Analysis of Salivary Levels of IL-8 and IL-34 in Periodontitis Patients: An Analytical Cross-Sectional Study 牙周炎患者唾液中 IL-8 和 IL-34 的水平分析:一项横断面分析研究

Introduction: The aim of this study was to determine the salivary levels of interleukin-8 (IL-8) and interleukin-34 (IL-34) among patients with periodontitis and healthy individuals. Methods: Thirty participants with periodontitis and 30 healthy individuals were enrolled in this study. Enzyme-linked immunosorbent assay (ELISA) was used to determine salivary levels of IL-8 and IL-34, blindly. Statistical analysis was carried out by unequal variances t-test (Welch’s t-test) and the Pearson correlation coefficient using R software. Result: Statistically significant differences were found between participants with periodontitis and healthy subjects regarding salivary levels of IL-8 (P = 0.012) and IL-34 (P < 0.001). There was a non-significant (P > 0.05) correlation between salivary levels of IL-8 and IL-34 and periodontal parameters, including plaque index, bleeding on probing, probing pocket depth, and clinical attachment loss. Conclusion: Within the limitations of this study, the salivary levels of IL-8 and IL-34 were higher among participants with periodontitis in comparison with healthy subjects. In the periodontitis group, salivary levels of IL-8 and IL-34 were not correlated with clinical indicators of periodontitis.

Endodontic Microsurgery for a Mandibular First Molar With a Large Lateral Canal in Mesial Root: A Case Report 下颌第一磨牙中上根大侧根管的根管显微手术:病例报告

Introduction: The lateral canal that occurs in the mesial root of the mandibular first molar is relatively rare. Herein we reported a rare case about a large lateral canal located in the middle of the mesial root of mandibular molar. Case report: A 38-year-old woman presented with symptomatic apical periodontitis and J-shaped bone defect similar to vertical root fracture on the mesial root of tooth #30. Endodontic microsurgery was chosen to make a clear diagnosis and treatment. The procedure was carried out with apicoectomy, root-end and lateral canal preparation, and retrograde filling. At the 1-year follow-up, clinical examination and the radiograph showed complete healing. Discussion: The untreated mesiobuccal root canal with large lateral canal was found to be the cause of the J-shaped bone defect.

Assessment of the Impact of Adhesive and Wires Types on the Tensile Bond Strength of Fixed Lingual Retainers Used in Orthodontics: An In Vitro Study 评估粘合剂和钢丝类型对正畸中使用的固定舌侧保持器拉伸粘接强度的影响:体外研究

Introduction: We aimed to assess the impact of adhesive and wires types on the tensile bond strength of fixed lingual retainers. Methods: A total of 160 intact bovine teeth were collected, cleaned, stored in 25% sodium hypochlorite, and randomly assigned to two groups based on the adhesive type: a two-step adhesive and a one-step adhesive. Each group was further divided into four subgroups based on the type of lingual retainer wire, which included (A) 8-strand braided stainless steel wire, (B) three-strand titanium retainer wire, (C) stainless steel chain, and (D) fiber-reinforced retainer. A tensile bond strength test was conducted using a universal testing machine at a controlled speed of 10 mm/min. Result: The 8-strand braided stainless steel wire and stainless steel chain bonded by one-step self-priming adhesive showed significantly higher tensile bond strength (P < 0.001). The adhesive wire significantly affected the tensile bond strength (P < 0.001). Conclusion: Within the limitations of this in vitro study, it can be concluded that stainless steel wire and chain bonded by one-step self-priming adhesive showed higher tensile bond strength.

Three-Dimensional Culture of Human Dental Pulp Tissue: A Preliminary Experimental In Vitro Model for Regenerative Endodontic Procedures 人类牙髓组织的三维培养:牙髓再生程序的初步体外实验模型

Introduction: Dental pulp regeneration is fundamental in dentistry and endodontics; however, few in vitro experimental models are available to study its biological process. This study aimed to develop a three-dimensional (3D) culture model of human dental pulp-like tissue mimicking the possible complexity of human pulp tissue. This new and innovative human-like tissue model could be used for testing drugs and materials, particularly those involved in regenerative endodontics. Methods: Vital pulp tissue samples were obtained from human third molars (n = 4) immediately after extraction and cultured in a 3D fibrin matrix to create a sustainable ex vivo experimental model. The angiogenesis degrees and the nitric oxide levels were evaluated following the culture of pulp-like tissues in the fibrin matrix for 21 days. The expression of Transforming growth factor- beta (TGF-β1), TGF-β2, TGF-β3, and their relevant receptors, tumor necrosis factor (TNF) and vascular endothelial growth factor A (VEGFA) was evaluated using a reverse transcription polymerase chain reaction (RT-PCR) method. Results: Pulp tissue angiogenesis was initiated, and completed on days 7 and 21, and pulp-like tissue cells expressed TGF-β1, TGF-β2, TGF-β3, and their relevant receptors, TNF and VEGFA. Conclusion: This model provided a precise observation of dental pulp angiogenesis at early stages.

Effectiveness of Four Different Obturation Techniques on the Penetration Depth of Bioceramic Sealers into the Dentin Tubules: An In Vitro Study 四种不同的封闭技术对生物陶瓷封闭剂渗入牙本质小管深度的影响:体外研究

Introduction: We aimed to assess the penetration depth of bioceramic sealers into the dentin tubules following different root canal obturation techniques included (A) warm vertical compaction, (B) carrier-based technique, (C) cold lateral compaction, and (D) single-cone obturation. Methods: This study utilized 40 extracted lower first premolars with developed apices and circular and straight root canals. The roots were eliminated to achieve an 11-mm length with a coronal flat measurement point. ProTaper Next rotary system was used for instrumentation. For obturation procedures, gutta-percha and Bio-C bioceramic sealer were employed, and the roots were randomly divided into four study groups, including (A) warm vertical compaction, (B) carrier-based technique, (C) cold lateral compaction, and (D) single-cone obturation. Depth of sealer penetration into the tubules was assessed using scanning electron microscopy. Result: We found significant differences in the penetration depth of bioceramic sealers based on obturation techniques (p < 0.001), location of dentin tubules (coronal, middle, or apical third) (p < 0.001), and the interaction between obturation techniques and location (p = 0.042). Conclusion: The warm vertical compaction and carrier-based technique showed superior penetration depth into the dentin tubules.

Introduction: In this study, we aimed to evaluate the impact of ultraconservative and traditional endodontic access cavities on the total percentage increase in the surface area of root canals. Methods: Endodontic instrumentation was performed on 30 simulated maxillary first premolars with two roots using traditional and ultraconservative access cavities via ProTaper Next rotary files. The pre- and postinstrumentation photographs of the root canals were analyzed blindly using Fiji software to determine the percentage increase in surface area. Results: A significant increase in the surface area of the root canal was observed with an ultraconservative endodontic access cavity compared to a traditional access cavity. The difference between the palatal and buccal roots was not significant, but there was a significant difference among the coronal, middle, and apical thirds of the root canal. Conclusion: Ultraconservative access cavities have a significant impact on the increase in surface area of the root canal after instrumentation.

Effect of the Rs2923234 and Rs1049112 Single Nucleotide Polymorphisms of Salivary Acidic Proline-rich Protein on Dental Caries in Young Children: An Analytical Cross-sectional Molecular Study 唾液酸性脯氨酸富蛋白Rs2923234和Rs1049112单核苷酸多态性对幼儿龋齿影响的横断面分子分析研究

Introduction: We aimed to use polymerase chain reaction (PCR) on genomic deoxyribonucleic acid (DNA) to detect the Db allele and the rs2923234 and rs1049112 single nucleotide polymorphisms (SNPs) of the salivary acidic proline-rich proteins (PRPs) to determine their relationship with dental caries in young children. Methods: DNA was extracted from saliva samples of preschool children aged 3 to 5 years. PCR primers designed around exon 3 of the PRH1 locus yielded a 416-base product representing Db for gel electrophoresis and a 519-base product representing the rs2923234 and rs1049112 SNPs for Sanger sequencing. The data were analyzed using a logistic regression model and a multilayer perceptron artificial neural network. Results: Forty children with severe caries and 40 caries-free children completed the study. The frequency of the Db gene was 16.3% in the entire study group. The rs2923234 SNP was a marginally significant (P = 0.053) predictor for the dependent variable (caries-free or severe caries). However, the rs1049112 (P = 0.407) and the Db allele (P = 0.442) were not significant predictors. Conclusion: The rs29232334 SNP could be considered a potential genetic predictor for caries susceptibility.

Guiding Groove: A New Hypothetical Strategy for Ostectomy and Fenestration in Endodontic Microsurgery 导向槽:一种新的假设策略,用于口腔内显微外科的Osteomy和开窗术

Introduction: How to save more bone for the teeth of patients with chronic periapical periodontitis with intact or small defects of the labiobuccal cortical plate in the periapical surgeries is a problem that many dentists are concerned about. The Hypothesis: We came up with a new strategy for periapical surgery by designing a modified osteotomy and fenestration method, which we named the “guiding groove” method. There are two ways to achieve this, one is to perform guided groove positioning under CBCT dynamic navigation, and the other is to design a guide plate that marks the position and direction of the guided groove based on CBCT data before surgery. Evaluation of the Hypothesis: The existence of the “guiding groove” locks in the angle and direction of the ultrasonic root end preparation and retrofilling apparatus, which not only minimizes the deviation of the root canal but also preserves more cortical plate on the labial and buccal sides of the teeth and thus improves the efficiency of treatment. This hypothesis has certain limitations. Firstly, the technical sensitivity of the operation is high, and proficiency in CBCT dynamic navigation technology is required. The accuracy requirements for navigation equipment are also relatively high. It will also increase the patient’s exposure to X-rays. For the guide plate designed before surgery that has already marked the position of the guide groove, it can only accurately avoid the deviation of the operating instrument in the proximal and distal directions, and it is difficult to avoid the deviation of the instrument in the buccal and lingual directions.

Assessment of the Salivary Level of Malondialdehyde in Localized Periodontitis Versus Generalized Periodontitis: An Analytical Cross-Sectional Study 局部牙周炎与广泛性牙周炎患者唾液丙二醛水平的评估:一项分析性横断面研究

Introduction: We aimed to compare the salivary level of malondialdehyde (MDA) in localized periodontitis versus generalized periodontitis. Methods: Nineteen healthy participants, 35 cases of generalized periodontitis, and 36 cases of localized periodontitis included in the study. The enzyme-linked immunosorbent (ELISA) assay was used to blindly quantify the amounts of MDA in saliva. Data analyzed using one-way ANOVA and Tukey’s post hoc test via R software. Results: Ninety subjects completed the study. There was a statistically significant difference among study groups (P < 0.001). The Tukey’s post hoc test showed a statistically significant difference for all pairwise compressions (P < 0.001) Conclusion: The salivary levels of MDA increase in periodontitis patients. Within limitations of this cross-sectional study, it could be concluded that salivary MDA levels may possibly be a potential biomarker to examine oxidative stress in periodontal disease.
