Anti-N-methyl-D-aspartate Receptor Encephalitis in Illicit Substance Use 非法药物使用中的抗n -甲基- d -天冬氨酸受体脑炎

Anti-N-methyl-D-aspartate receptor (NMDAR) encephalitis is a neuroinflammatory disease mediated by autoantibodies against the NMDAR, typically presenting with psychiatric symptoms, cognitive dysfunction, and motor dysfunction. These neuropsychiatric symptoms may be mimicked by drug abuse, and the development of anti-NMDAR encephalitis may be triggered by certain substance use. Here we report a case of anti-NMDAR encephalitis who developed neuropsychiatric symptoms after illicit substance use, the first report in Korea.

Quality of Acute Stroke Care within Emergency Medical Service System in Korea: Proposal for Severe Emergency Medical Center 韩国急诊医疗服务体系中急性脑卒中的护理质量:重症急诊医疗中心的建议

Background: Korea recently established 70 emergency medical service areas. However, there are many concerns that medical resources for stroke could not be evenly distributed through the country. We aimed to compare the treatment quality and outcomes of acute stroke among the emergency medical service areas.Methods: This study analyzed the data of 28,800 patients admitted in 248 hospitals which participated in the 8th acute stroke quality assessment by Health Insurance Review and Assessment Service. Individual hospitals were regrouped into emergency service areas according to the address of the location. Assessment indicators and fatality were compared by the service areas. We defined the appropriate hospital by the performance of intravenous thrombolysis.Results: In seven service areas, there were no hospitals which received more than 10 stroke patients for 6 months. In nine service areas, there were no patients who underwent intravenous thrombolysis (IVT). Among 167 designated emergency medical centers, 50 hospitals (29.9%) responded that IVT was impossible 24 hours a day. There are 97 (39.1%) hospitals that meet the definitions of appropriate hospital. In 23 service areas (32.9%) had no appropriate or feasible hospitals. The fatality of service areas with stroke centers were 6.9% within 30 days and 15.6% within 1 year from stroke onset than those without stroke centers (7.7%, 16.9%, respectively).Conclusions: There was a wide regional gap in the medical resource and the quality of treatments for acute stroke among emergency medical service areas in Korea. The poststroke fatality rate of the service areas which have stroke centers or appropriate hospitals were significantly low.

Reversible Burst-suppression Pattern of Acute Hyperammonemic Encephalopathy 急性高氨血症脑病的可逆性突发抑制模式

Acute hyperammonemic encephalopathy is generally associated with severe liver disease, manifesting with neuropsychiatric symptoms including confusion, lethargy, seizure, coma, and even death. Electroencephalogram (EEG) is a proven diagnostic, prognostic, and therapeutic tool in patients with acute and chronic encephalopathies. EEG changes in acute hyperammonemic encephalopathy are associated with background slowing in theta to delta ranges, accompanied by presence of triphasic waves. We reported a patient with acute hyperammonemic encephalopathy showing an unusual burst-suppression pattern, which was reversible with proper treatment.

The Etiology of Acute Stroke with Experience of Repeated Upper Extremity Deep Vein Thrombosis 反复上肢深静脉血栓形成的急性脑卒中病因学分析

In stroke patients, upper extremity deep vein thrombosis (UEDVT) is uncommon compared with lower extremity. Unlike the blood stasis in lower extremity, UEDVT has been developed by secondary cause. We reported a case of stroke patient with repeated UEDVT, presenting superficial venous congestion, who was finally diagnosed with pulmonary adenocarcinoma. The cause of stroke was non-bacterial thromboembolism formed at the mitral valve. Our case shows that unexpected UEDVT should be closely evaluated for higher coagulable status such as a malignancy.

Neurocysticercosis is the most common parasitic infection of the nervous system. However, the improvement in public health made the prevalence of neurocysticercosis low. Neurocysticercosis may have symptoms such as seizures, headache, and hydrocephalus, and calcified neurocysticercosis is generally known to be asymptomatic and inert. Also, status epilepticus associated with neurocysticercosis has been rarely reported. Therefore, we report a case of focal status epilepticus caused by calcified neurocysticercosis that invaded the subarachnoid space, which is uncommon pathophysiology of neurocysticercosis.

Background: This study aims to develop a well-founded program for the elderly in the community to provide a guide to continuous practice and to be used as basic data for its spread in the community.Methods: Among the elderly aged 60 or older living in Seongdong-gu, a total of 130 people agreed to participate in the study and met the criteria for selecting subjects, and 86 were the final subjects. The effects on cognitive function, depression, daily life activities, and quality of life before and after participating in the program were investigated and evaluated. Cognitive Impairment Screening Test, Subjective Memory Complaints Questionnaire, Short form of Geriatric Depression Scale-Korean version, Korean version of Instrumental Activities of Daily Living, and EuroQol-5 dimension were used as test tools. After the pre-test, they participated in the program twice a week for 14 50 minutes sessions, and the average score of the posttest was compared.Results: It was found that cognitive function improved and depression significantly decreased regardless of age after participating in the program than before participating in the program. There was a difference in cognitive function improvement according to the level of education, but depression was found to decrease equally, and health-related quality of life improved.Conclusions: Based on the results of this study, it is recommended that senior citizens aged60 or older continue to practice the 'Connect Memories' program to improve cognitivefunction.
