led to a need for comprehensive stroke centers (CSC) capable of providing advanced and multidisciplinary care. Articles with information about guidelines, and other relevant clinical and research reports were investigated to establish the concept of certification for a CSC. Experts recommended by stroke-related societies gathered to form a consensus through public hearings, several meetings, and academic conferences. Herein, we aimed to write the policy statement for CSC certification. We described key elements of CSC in Korea. These areas include healthcare personnel with specific expertise in a number of specializations for stroke care, specific infrastructures and advanced diagnostic techniques including neuroimaging capabilities, surgical and endovascular techniques and neurocritical care for complex stroke care, and programmatic elements such as a stroke registry, education, and quality improvement activities. CSCs should have a comprehensive team that includes neurologists, neurosurgeons, vascular surgeons, neurointensivists, neuroradiologists, rehabilitation specialists who perform cutting edge diagnosis and treatment for two or more complex acute stroke patients simultaneously in an all-in-shop model without additional transfer to other hospitals.

Tetanus is a bacterial infection by Clostridium tetani. Its neurotoxin causes spastic paralysis and autonomic dysfunction. Intrathecal infusion of baclofen has been suggested as a pertinent treatment for generalized spasm. Our case describes a patient who had a severe generalized form of tetanus, and was effectively treated with intrathecal baclofen infusion. He showed cerebral and brainstem dysfunction during baclofen infusion, which were reversed without sequelae when baclofen was discontinued.

We report a rare and unique presentation of a patient with early and levodopa-responsive Parkinson’s disease (PD) who exhibited clinical features similar to posterior cortical atrophy (PCA). The imaging evidence suggested the coexistence of PD and PCA, without the presence of amyloid pathology. Although the present case lacks pathologic evidence, this case represents a distinct manifestation of PD characterized by a combination of parkinsonism and early prominent posterior cognitive deficits, reminiscent of the visual variant of Alzheimer’s disease.

Recurrent meningitis is defined as more than one episode of meningitis that is separated by an entire recovery period. Causes of recurrent meningitis include anatomical defects, immune deficiency, and parameningeal infections. A thorough investigation of the underlying causes of recurrent meningitis is required for proper treatment. Zolpidem is widely used for insomnia and is known to have relatively few side effects. There have been several reports of side effects, but only a few for withdrawal symptoms. A 41-year-old woman developed convulsions 4 days after discontinuing zolpidem. She was taking more than 10 times the standard dosage. After electroencephalography monitoring without antiseizure medications, she showed no more seizures. This case highlights the danger of zolpidem withdrawal and warns about the easy accessibility of zolpidem for individuals.
