Purpose: This was a descriptive study to compare the characteristics and outcomes among deceased cancer patients that had do-notattempt-resuscitation (DNAR) agreements in hospice and oncology wards. Methods: Subjects of this study were 250 patients with terminal cancer who died after making DNAR agreements from January 1, 2015 to December 31, 2016 . The subjects were divided into two groups those that died in hospice (n=150) and those that died in oncology wards (n=100). Data were collected from August 2017 to February 2018. Two trained investigators independently reviewed the medical records for subjects using survey tools developed by the researchers (r =.81). Results: Among the general characteristics between two groups, there were differences in age (t=-2.54, p=.012) and education (χ=5.96, p=.015). Among the disease related characteristics, there were differences in surgery history (χ=10.09, p=.001), chemotherapy history (χ=11.08, p=.002) and symptoms (t=5.14, p<.001) between the two groups. Among the treatment related characteristics, there were differences in chemotherapy at death (χ=9.61, p=.002), radiotherapy at death (χ=5.60, p=.018) and cardiopulmonary resuscitation (χ=6.15, p=.029) between the two groups. Among nursing related characteristics, there was a difference in the frequency of nursing intervention after DNAR consent (t=3.72, p<.001) between the two groups. Among outcomes related characteristics, there were differences in consciousness state at the DNAR consent (χ=64.82, p<.001), self-written consent (χ=18.26, p=.001), hospitalization days (t=2.31, p=.022) and the cost per day of hospitalization (t=-4.81, p<.001) between the two groups. Conclusion: This study found that the patients, families, and medical staff need to change their perception of hospice and palliative care to prevent medical expenses from rising due to unnecessary life-support treatment while preparing for death with family members.

하게 되므로 암 환자의 생존율 증가와 더불어 암 생존자의 건강 관 련 삶의 질(health-related quality of life, HRQoL)에 대한 관심이 부각 되고 있다. HRQoL은 건강 수준 또는 질병 정도에 의해 영향을 받는 삶의 질로 일상생활, 주관적 경험에 관한 신체적, 사회적, 감각적 그 리고 주관적 행복을 포괄하는 광범위하고 다면적인 개념이다. Purpose: This study aimed to examine the effects of knowledge of Human Papilloma Virus (HPV) and health beliefs related to HPV vaccination on the intention of cervical cancer prevention behavior in Nurses. Methods: Data were collected by conducting a survey of 222 nurses at a single tertiary hospital in J city from September 26 to October 02, 2019. The data were analyzed in SPSS WIN version 23.0 using descriptive statistics, t-test, ANOVA, Pearson’s correlation coefficient and Multiple linear regression. Results: The mean score for HPV knowledge was 7.98. Regarding health beliefs about HPV vaccination, the mean score for the level of perceived benefit was 3.08, and for perceived sensitivity was 2.09. The factors influencing the intention of cervical cancer prevention behavior were perceived benefits, severity, contraception, clinical career, and experience with HPV testing. Conclusion: The results of the study indicate that it is necessary to develop and apply education programs that enhance cervical cancer and HPV knowledge. Health beliefs related to HPV vaccination should be integrated when developing nursing intervention to improve the intention of cervical cancer prevention behavior in nurses.
目的:本研究旨在探讨人乳头瘤病毒(HPV)知识和与HPV疫苗接种相关的健康信念对护士宫颈癌预防行为意向的影响。方法:于2019年9月26日至10月2日对J市某三级医院222名护士进行问卷调查。采用SPSS WIN version 23.0对数据进行描述性统计、t检验、方差分析、Pearson相关系数和多元线性回归分析。结果:HPV知识平均得分为7.98分。关于HPV疫苗接种的健康信念,感知获益水平的平均得分为3.08分,感知敏感性的平均得分为2.09分。影响宫颈癌预防行为意向的因素包括获益感知、严重程度、避孕、临床职业和HPV检测经验。结论:研究结果表明,有必要制定和应用教育计划,提高宫颈癌和人乳头瘤病毒的知识。在制定护理干预措施时,应整合与HPV疫苗接种相关的健康信念,以提高护士宫颈癌预防行为的意愿。

Purpose: The purpose of this study was to identify the effects of repeated audiovisual information on the reduction of uncertainty and anxiety in patients undergoing thyroidectomy. Methods: A non-equivalent control group pretest-posttest design was used. The study included 32 patients in the experimental group and 32 in the control group. Preoperative information using video-audio media was provided two times for 5 minutes and 45 seconds each in the experimental group, while the control group only received routine care. The data collection period was from November 1, 2017 to February 23, 2018. The data were analyzed using χ² test, t test, and independent t-test with SPSS/WIN 24.0. Results: Significant differences were observed between the two groups in pre- and post-uncer-tainty scores (t = -2.06, p= .044) and anxiety scores (t = -2.06, p= .044). Purpose: This study was conducted to examine the role of video education on cancer patient advance directive writing. Methods: To evaluate the effects of video education on cancer patient advance directive knowledge, attitude, writing intention and writing, this study employed a non-equivalent control group and non-synchronized design as a quasi-experimental study. The target population of this study were adults aged ≥50 years old, diagnosed with cancer in South Korea. The proximity population were also cancer patients who visited a university hospital. The control group (n=31) were educated verbally using a leaflet and the experimental group (n=30) were educated by a video program developed by the National Bioethics Policy Institute for 10 minutes, supplemented with a leaflet. The advance directives video program form provided by the Korean Ministry of Health and Welfare was used. Results: The experimental group showed a positive change in knowledge level (F=52.82, p<.001, effect size=0.96), attitude (F=12.21, p<.001, effect size=0.46), the intention to write (F=18.22, p<.001, effect size=0.56) advance directives and a higher proportion of writing (χ=13.87, p<.001) than the control group. Conclusion: The results of this research show that providing information through video is a more effective educational medium for retention of knowledge, attitude, writing intention, and writing of cancer patient’s advance directives.

Methods: To evaluate the effects of video education on cancer patient advance directive knowledge, attitude, writing intention and writing, this study employed a non-equivalent control group and non-synchronized design as a quasi-experimental study. The target population of this study were adults aged ≥50 years old, diagnosed with cancer in South Korea. The proximity population were also cancer patients who visited a university hospital. The control group (n=31) were educated verbally using a leaflet and the experimental group (n=30) were educated by a video program developed by the National Bioethics Policy Institute for 10 minutes, supplemented with a leaflet. The advance directives video program form provided by the Korean Ministry of Health and Welfare was used. Results: The experimental group showed a positive change in knowledge level (F=52.82, p<.001, effect size=0.96), attitude (F=12.21, p<.001, effect size=0.46), the intention to write (F=18.22, p<.001, effect size=0.56) advance directives and a higher proportion of writing (χ=13.87, p<.001) than the control group. 치료의 발전으로 이환율 및 사망률이 전반적으로 감소추세에 있 어, 유방암의 5년 상대생존율은 92.7%로 높은 생존율을 보인다. 이는 유방암 증상의 조기발견과 조절, 그리고 치료로 인한 합병증 예방이 점차 중요해졌음을 시사한다. 유방암 치료로 수술, 항암화학요법, 생물학적 요법, 방사선요법 등이 적용되는데, 항암화학요법은 유방암 치료에 가장 많이 적용되 고 있는 치료법이다. 유방암의 항암화학요법은 adriamycin과 cytoxan을 병용(AC 항암화학요법)하거나 taxotere와 cytoxan을 병용 (TC 항암화학요법)한 항암화학요법이 많이 사용되고 있다. 최근 AC 항암화학요법은 American Society of Clinical Oncology (ASCO), National Comprehensive Cancer Network (NCCN), Multinational Association of Supportive Care in Cancer (MASCC) 등에서 고위험 오 심 유발 항암화학요법으로 재분류되어 AC 항암화학요법을 시행 하는 환자에서 오심 간호중재가 중요한 문제로 대두되게 되었다. Purpose: The objective of this study was to determine the effects of self-efficacy and depression on the sense of family coherence in both cancer patients and their caregivers using the Actor-Partner Interdependence Model (APIM) analysis. Methods: A total of 274 patients were registered in the study, including 137 cancer patients who were undergoing chemotherapy in a day care ward and 137 primary caregivers. The data was collected from December 30, 2017 to July 30, 2018. Data were analyzed by the data analysis method using SPSS/WIN 20.0 (SPSS Korea Data Solution Inc) and AMOS 21.0 (SPSS Korea Data Solution Inc). Results: Self-efficacy of cancer patients had a direct effect on patient depression (β=-.24, p=.008). Self-efficacy of primary caregivers also had a direct effect on patient depression (β=-.51, p<.001). Factors significantly affecting cancer patient FSOC were cancer patient self-efficacy (β=.34, p<.001) and cancer patient depression (β=-.42, p<.001). Factors significantly affecting primary caregiver FSOC (Family Sense of Coherence) were cancer patient self-efficacy (β=.13, p=.026) and caregiver depression (β=-.29, p=.008). Conclusion: It is thought that self-efficacy and depression should be controlled to improve the family cohesion of cancer patients and their primary caregivers.
