Rehabilitation protocol with or without mobile health in overweight and obeseknee osteoarthritis patients - a pilot study 超重和肥胖骨关节炎患者有无移动健康的康复方案——一项试点研究

Knee osteoarthritis (OA) is a degenerative and weight bearing joint disease that resulted from wear and tear of articular cartilage. It is more common in the overweight and obese knee OA patients. The objectives of pilot study were to assess the feasibility, acceptability and challenges of study design, study setting and tools of lower limb rehabilitation protocol among the knee OA patients who are overweight and obese. Twelve overweight and obese knee OA patients attending a Teaching Bay in the Pakistan were enrolled in the study and completed 6 training sessions over 2 weeks. The participants were divided randomly into Intervention Group with Mobile Health (IGW-mH), Intervention Group without Mobile Health (IGWO-mH) and the Control Group (CG). Both intervention groups were provided fifteen days of lower limb rehabilitation protocol but the CG did not. Two text messages per day for three days a week were sent to IGW-mH as a reminder to carry on their training session and instructions of daily care. All participants of intervention groups showed willingness to randomization and adherence to training sessions. The results indicated that eligibility criteria, recruitment rate and randomization procedures were feasible and there were no adverse events from training sessions. Possibilities of using natural mineral waters in the treatment of patients with non-alcoholic fatty liver disease 使用天然矿泉水治疗非酒精性脂肪肝患者的可能性

Non-alcoholic fatty liver disease (NAFLD) is a chronic pathology that is increasingly diagnosed in different countries. The pathogenesis of NAFLD associated with insulin resistance, abdominal obesity, atherogenic dyslipidemia, arterial hypertension, endothelial dysfunction, impaired adipokine secretion, that is, components of metabolic syndrome. Treatment of NAFLD should be comprehensive and lengthy, although there are no standardized approaches to the treatment of NAFLD. Meanwhile, drug therapy carries certain risks: the development of serious adverse reactions with prolonged use. The work highlights modern views on the etiology, epidemiology, pathogenesis, drug and non-drug treatment of non-alcoholic fatty liver disease (NAFLD). Experimental and clinical studies on the mechanism of biological action, the effects of using different in composition and mineralization of mineral waters on the course of NAFLD had presented. Based on experimental and clinical studies, ideas about the specificity of the action of mineral waters of various balneological types and mineralization on the clinical course of NAFLD at different stages of the disease, the effect on the functional state of the liver, and the dynamics of lipid and carbohydrate metabolism had detailed. It is concluded that mineral waters could be successfully used in the complex treatment of NAFLD patients.

Multimodal therapeutical-rehabilitative approches in a complex case of pathology including possibly evolving discariotic type- case report. 复杂病理病例的多模式治疗康复方法,包括可能演变的椎间盘型病例报告。

Introduction Having the patient’s consent and The Teaching Emegency Hospital “Bagdasar-Arseni” Ethics Committeee N.O 20270 from 26.06.2019, the current case report presents the case of a female patients with both hemiplegia following a thalamic vascular accident and a long history of neglected auricular melanoma. The management of a patient diagnosed with melanoma is a complex one, involving wide local excisions with safety margins, with sentinel lymph node biopsy. Auricular melanomas have recently evolved from radical procedures involving the amputation of the involved organ, to much less radical procedures, which help save more of the patient’s tissue and functionality. (1) Another important factor that threatens the rehabilitation process in the case of hemiplegic patients is the presence of clinical depression, both as a pre-existing comorbidity and as a common psychiatric complication of stroke. (2) Depression jeopardizes the patient’s quality of life and increases mortality. (2) There is also relevant date supporting the hypotheses that depression history is associated with melanoma risk, although no effect on survival was observed. (3) Materials and Methods A 70 years old pacient, which suffered right thalamic vascular accident in december 2018 was admitted in our Neuromuscular clinic division with the following comorbidities: basal-cell carcinoma (BCC), diagnosed 20 years ago, with slow evolution. When admittedin our division, the pacient presented with a moderately large ulcerated tumor in the temporal region and the left auricular pavilion – with surgical indication. She was also diagnosed with arterial hypertension stage III, chronic, cardiac insufficiency class III NYHA, chronic cervicalgia and lumbosacralgia, class II obesity, clinical depression. She was admitted into our clinic for hemiplegic motor deficit, sensibility disorders, severe locomotor and self-grooming dysfunction. Results The patient improved on most of the assessment scales/scores implemented in our Clinic’s Division Motor FIM (Functiona Independence Measure) from 35/91 to 38/91, FAC(Functional Ambulation Categories) from 0/5 to 1/5, GOS-E (The Extended Glasgow Outcome Scale) from 4/8 to 5/8. The most important improvemet in our patent’s evolution was her ability to start walking again. During her admission into our Clinic, she was also briefly admitted into the hospital’s Plastic Survery Divison, where she received the necessary surgical trateament for the melanoma. Conclusions Following a complex neuro-recovery program developed by a multidisciplinary team made of doctors, kinesio-therapists, middle and allied health personnel, the patient had an extremely good evolution (during a short period of time) - attested on the scales and also - on a psycho-cognitive and behavioral level.Key words: neuromuscular rehabilitation, stroke, hemiplegia, melanoma, basal-call carcinoma,

The role of the balneo-physical-kinetic treatment on the ligamentous stability in osteoarthritis of the knee balneo运动疗法对膝关节骨性关节炎韧带稳定性的影响

Osteoarthritis of the knee is one of the disabling conditions with a major impact on the quality of life of the patients. Existing therapies focus on reducing pain and disability caused by the disease. The existence of scores that evaluate the different aspects of patients quality of life are often used in monitoring the treatment and its results.The study is performed on a batch of 21 patients, between June 2017 and June 2018. They performed a complex rehabilitation program being evaluated every six months by applying the WOMAC questionnaire and performing specific maneuvers.The study has shown that by performing complex rehabilitation treatments the patients ability to perform some actions as well as their quality of life is improved and the effect following repeated treatments every six months is maintained during two treatments successive treatment.

Efficacy of intra-articular hyaluronic acid injections in the rehabilitation programme of Knee Osteoarthritis 关节内注射透明质酸治疗膝关节骨关节炎的疗效观察

AbstractKnee osteoarthritis (KOA) is a disease of wear and tear, leading to disability and ultimate surgical treatment. KOA has multiple etiologies and is considered a disorder of physiological processes that is manifested by progressive lesions of articular cartilage caused by chondrolysis, synovial membrane inflammation and subchondral osteophytes production. Modalities that limit the progression of the KOA and rehabilitation are a constant interest for clinicians and medical researchers. Various physical therapies and pharmacological strategies are currently used for pain management, with no clear benefit in terms of inhibiting disease progression. In this context, the use of exogenous hyaluronic acid (HA) (that is a vital component of the synovial fluid and involved in lubricating all synovial joints) has been explored as viable alternative to alleviate knee pain and temporarily restore knee function. In the following, we will review the use of HA injections associated with the physiotherapeutic recovery plan in the treatment of KOA. The therapeutic effects of the HA is still a matter of debate in the field, with no consensus being reached so far and thus, difficult to evaluate. However, much research has to be done to understand the therapeutic efficacy of HA alone or in combination with physical therapies and the role that other factors may play in unraveling its beneficial effects on the KOA. In this context, the use of exogenous hyaluronic acid (HA) (that is a vital component of the synovial fluid and involved in lubricating all synovial joints) has been explored as viable alternative to alleviate knee pain and temporarily restore knee function. In the following, we will review the use of HA injections associated with the physiotherapeutic recovery plan in the treatment of KOA. The therapeutic effects of the HA is still a matter of debate in the field, with no consensus being reached so far and thus, difficult to evaluate. The role of dance therapy in the rehabilitation of Parkinson disease patients 舞蹈疗法在帕金森病患者康复中的作用

Introduction: Parkinson disease is the second most common neurodegenerative disease which requires a complex approach including pharmacological and non-pharmacological treatment. While kinesiotherapy is a must in Parkinson’s disease patients, dance therapy in rehabilitation programs is not often taken into consideration. Dance therapy is a challenging non-pharmacological treatment which merges physical activity with musical synchronization, socialization , disease acceptance and creativity with daily activities. The aim of this study is to evaluate the effect of dance therapy on the quality of life, functionality and decrease of depression in Parkinson patients.Material and method: We developed a case control study to evaluate the benefit of dance therapy in Parkinson Patients. The study group consisted in 10 patients from “Puterea Sperantei Project”. The intervention consisted in 40 minutes/day of dance therapy during two weeks. First control group consisted of 10 patients admitted to Cluj-Napoca Rehabilitation Hospital for two weeks. The second control group consisted in 5 patients enrolled in the “Puterea Sperantei Project” which did not attended dance classes. All three groups followed standard therapy: kineto-therapy and ergo-therapy. To assess the effect of dance therapy we used: Beck scale, Parkinson Disease Questionnaire (PDQ-39) and a satisfaction questionnaire which we developed, for the dance therapy. Results and discussions: In the study group we registered an improvement of depression symptoms (p=0,017) and an increase in the life quality (p=0,005). In the hospital control group and in the “Puterea Speratei Project” control group there was no decrease in the depressions score (p >0,05). However, the quality of life increased also in the control group enrolled in the Rehabilitation Hospital, probably due to medication change. The mean of satisfaction questionnaire for dance therapy in the study group was 39,70 from a maximum of 48 points. Conclusions: Dance therapy is a holistic non-pharmacological therapy which improves the depression index, the quality of life and functionality of Parkinson disease patients. This initial results raise the possibility of including dance therapy as an alternative of non-pharmacological therapy in the rehabilitation programs for Parkinson Disease Patients.

Asthma is defined by The Initiative for Asthma (GINA 2018) as a heterogeneous disease, which include chronic airway inflammation and a history of respiratory symptoms. In the last decades asthma had a rise in prevalence, becoming one of the most frequent diagnosed diseases in the world. The main goals of asthma management are to achieve good symptom control, minimize the risks of exacerbations, decrease rescue medication intake, improve the quality of life by decreasing respiratory system inflammation and ameliorating the patient’s lung function. Beside effective medications, asthma continues to impair quality of life for most patients. Due to the difficulty of controlling symptoms and exacerbations, the need of developing complementary options of treatment is increasing in order to achieve an optimum control and a lower risk of acute episodes or fatal events. Pulmonary rehabilitation is suggested for asthma patients when adequate medical therapy poorly control the symptoms and mental, physical or social consequences of illness persist during the daily life. The following non-drug therapy components are included in the rehabilitation program: physical training, comprehensive smoking cessation program, comprehensive patient education, respiratory physiotherapy, psychosocial support and comprehensive nutritional counseling. These complementary therapies have been proven to improve muscle strength, exercise capacity and symptomatology. Also, it has been associated to fewer exacerbations and a lower use of rescue medication, leading to a better quality of life. AbstractLower lumbar discopathies are in continuous growth among both young people and elderly. Causes that have led to an increase in the incidence of this pathology include: lack of exercise, nutrition, excessive use of computers and mobile phones, etc. The increased incidence of pathology led us to identify an attractive kinetotherapeutic treatment protocol that initially reduced the symptoms of this condition, and subsequently allowed the patients’ complete recovery.The present study was conducted on 33 patients diagnosed with lower lumbar discopathy. They received two evaluations, and between these, the patients performed a kinetic program consisting of elastic band exercises along with general neuroproprioceptive muscle relaxation techniques.The data obtained were inventoried and processed using statistical programs. The comparison of the initial and final results, as well as the data obtained from statistical processing showed an improvement of the symptomatology and the orientation of patients towards a complete recovery. Role of rehabilitation in prevention of early thromboembolic complications in hemorrhagic stroke 康复在预防出血性脑卒中早期血栓栓塞并发症中的作用

Hemorrhagic strokes (ICH) affects mainly young active people, with increasing incidence in developing countries. Mortality is high in acute phase, and patients are prone to complications related to stroke itself and to coexisting medical conditions. Patients with ICH are at high risk in developing deep venous thrombosis (DVT) with secondary pulmonary embolism (PE). Prevention of venous thrombotic events in hemorrhagic stroke patients requires intermittent pneumatic compression and preventive doses of low molecular weight heparins (LMWH) in high-risk patients. If DVT and /or PE occurs, the therapeutic management should balance the risk of recurrent cerebral bleeding and the life-threatening risk of PE, making the decision to start anticoagulation challenging. We present a case of a young patient with a large hypertensive capsulo-lenticular hemorrhage, who was diagnosed with pulmonary embolism 21 days after stroke onset. The decision was for anticoagulant treatment initial with LWMH, and switch to direct oral anticoagulants (DOAC) after 10 days; strict control of vascular risk factors of the patients (hypertension, diabetes and obesity) was achieved. Rehabilitation treatment, delayed until day 21, was recommended with progressive intensity. Evolution of the patient was favorable, with complete hematoma resorbtion under DOAC treatment at 10 weeks follow-up and important motor recovery. Rehabilitation program was intensive during this interval, and strongly contributive to neurologic improvement.

Introduction & objectives: Although controversial, the Mozart effect, consisting of an improvement in cognitive abilities in healthy persons, has started to be studied in the case of patients with various neurological, even neurodegenerative diseases (dementia, treatment-refractory epilepsy). There are few data on the Mozart effect in Parkinson’s disease, but the positive results reported for other neurological pathologies have led us to study this subject. The aim of this study is to compare the results of the Mozart effect associated with specific kinetic treatment in the rehabilitation of Parkinson’s disease and the effect of standard kinesiotherapy, results reflected in the impact on the degree of disability, emotional state and quality of life.Material and method: This was a prospective case-control study, comprising patients with Parkinson’s disease, Hoehn & Yahr stages I-III, included in a motor neurorehabilitation program through the project “The power of hope for patients with Parkinson’s disease” Cluj-Napoca. The patients were assigned to two groups, a control group with specific physiotherapy and the other group receiving the same specific kinetic treatment associated with the Mozart effect, for 2 hours daily, over 14 consecutive days. The patients were assessed using the Schwab & England scale (for the degree of dependence) and the PHQ-9 scale (for depression) before and after therapy.Results: After 2 weeks of therapy, the patients in the study group had much more obvious, statistically significant improvements in both scores.Conclusions: Mozart’s music, as an adjuvant intervention to specific kinetic treatment in patients with Parkinson’s disease, had a beneficial effect, ameliorating the degree of disability and emotional status.

There are few data on the Mozart effect in Parkinson’s disease, but the positive results reported for other neurological pathologies have led us to study this subject. The aim of this study is to compare the results of the Mozart effect associated with specific kinetic treatment in the rehabilitation of Parkinson’s disease and the effect of standard kinesiotherapy, results reflected in the impact on the degree of disability, emotional state and quality of life.\u0000Material and method: This was a prospective case-control study, comprising patients with Parkinson’s disease, Hoehn & Yahr stages I-III, included in a motor neurorehabilitation program through the project “The power of hope for patients with Parkinson’s disease” Cluj-Napoca. The patients were assigned to two groups, a control group with specific physiotherapy and the other group receiving the same specific kinetic treatment associated with the Mozart effect, for 2 hours daily, over 14 consecutive days.
