By training young people a range of practical medical skills and engaging them in volunteer activity, they are expected to express themselves and achieve outcomes that have an impact on their own lives and the lives of others in society. Purpose: to determine the attitudes and motivations of students studying medical specialties who want to volunteer using their professional training received at Higher Medical School during the pandemic. Material and Methods: A survey; Mathematical and statistical processing of the obtained data; Applying an established methodology for selection of willing and motivated students; Non-formal education. Results: Total surveyed students 282 - the largest number are those studying the Nurse specialty (48.2%). 57.4% of study participants had experience volunteering. 52.7% would work as a volunteer with Covid-19 patients. 11% were already volunteering Covid-19 at healthcare facilities. The main reason why students are most likely to volunteer in the Covid-19 sectors is personal moral principles (49.2%). The main way students would spread information in society about the nature and principles of voluntary student work is through social networks (75.2%). Conclusions: More than half of the respondents had experience as volunteers. Students are motivated and willing to help Covid-19 patients. Personal motivation and opportunities to improve skills and acquire new knowledge and competencies are the main reasons students prefer volunteering during a pandemic. Volunteer work has a positive influence on the professional development of students, a good impact on their professional identity creation and is essential for their future work as healthcare professionals.

The nurse's work with children with disabilities requires narrowly profiled knowledge to ensure adequate medical and health care for children in the process of deinstitutionalization. Purpose: The purpose of the research is to establish the attitudes of nurses regarding the increase of their professional competence for working with children with disabilities in the process of deinstitutionalization. Methods: An anonymous survey was conducted of 109 nurses working in medical and social care homes (HMSCC) in the cities of Burgas and Stara Zagora. For this purpose, a questionnaire consisting of 34 questions of open and closed type was developed. Results: The survey found 68% of respondents believed that a nurse should have a continuous drive to learn new knowledge about caring for children with disabilities. The respondents (75%) indicate that dealing with emergency situations is a mandatory professional competence for those working with children with disabilities, another 55% consider that the ability to emotionally assess the child's condition is very important when performing their professional activity. The respondents considered that a personal contribution of the nurse (outside of professional duties) is necessary for the improvement of care for children with disabilities [t(107) = 5.814; p-level < 0.001]. Conclusion: The survey found a positive attitude of the respondents to participate in postgraduate studies. The respondents express a desire to become involved as trainers of parents to provide real knowledge and skills in order to deal with and solve the child's health problems that arise at home.

Background - hyperbaric oxygen therapy (HBOT) is a clinical method of treatment with oxygen while under increased atmospheric pressure. It has been proven as an independent therapeutic factor for a number of diseases, and in the case of COVID-19, its potential as an adjuvant therapy is still being studied. Case report – in the case of a 61-year-old man with severe pneumonia resulting from infection with COVID-19 and persisting hypoxemia and dyspnea after the acute phase. Prescribed continuous oxygenation in home conditions due to deteriorating general condition and quality of life. Undertaken additional hyperbaric oxygen therapy resulting in significant improvement after the 20th session. Conclusion - the connection between HBOT and improved paraclinical and imaging indicators, in this case, suggests conclusions of a therapeutic effect of HBOT in hypoxemia as a post-COVID symptom.

Obstructive Sleep Apnoea (OSA) with hypoxemia, oxidative stress, low-grade inflammation and multiple hormonal metabolic changes affect the bone metabolism. This leads to an increased fracture risk in patients with OSA. Purpose: The aim of the study was to explore the connection between fracture risk and newly-diagnosed OSA. Materials/methods: 130 patients with newly diagnosed OSA and 67 controls without OSA were included in the study. Anthropometric, laboratory, instrumental and study tests and fracture risk assessment under the FRAX program were performed. Results: There isn’t a statistical difference in the age and gender in the OSA group compared to the controls (p>0.05). A statistically significant difference between the OSA group and the control group was found with regard to the body mass index (BMI), visceral fat mass ratio (VFR), neck circumference, Epworth sleepiness scale (ESS), Vitamin D levels, beta-crosslaps and osteocalcin (р<0.0001 for all). Reduction of bone mineral density (BMD) was found in patients with OSA. On average, three risk factors for fracture were found in patients with OSA compared to an absence or only one risk factor in the control group (р<0.0001). Conclusion: Patients with OSA are at an increased fracture risk due to disturbed bone metabolism. They have lower Vitamin D levels, reduction of BMD and 3 risk factors for high FR. This requiresan assessment of fracture risk and its eventual reduction in patients with OSA.

Objectives: The aim of our study is to analyse clinicohistopathological features, recurrence, metastasis and mortality rates of SCCs in Pleven, Lovech region, Bulgaria. Methods: We investigated 355 patients with histologically confirmed SCCs for the period 2016 – 2022 and carried out a detailed histopathological analysis of 100 tumors. They were divided into two groups according to their macroscopic diameter: with low risk for recurrence and metastases (<20mm) and high – risk (>20mm). We studied the localisation, histological subtype, tumor cell differentiation, microscopic depth of invasion, perineural and lymphovascular invasion, stromal lymphocyte infiltration. Results: We revealed that 73,3% of the patients with SCCs reported severe sunburns, and 1,13% of them underwent immunosuppressive treatment. Head and Neck localisation is the most common – 72,7%. We established a mean histological depth of SCCs of 5,92mm, lymphovascular invasion in 7% and perineural invasion in 5% of the cases. We found that tumors with a macroscopic diameter >20mm have a greater depth of invasion in comparison to SCCs with a smaller diameter. We found an incidence of local recurrence of 4,8%, lymph node metastases at 5,1%, far metastases in internal organs at 2,25% and mortality at 1,97%. Conclusion: Clinicohistopathological features of SCCs with related to recurrence, metastasis and mortality are localisation on the scalp, ears or lips, tumor size >20mm, depth of invasion >6mm, poor tumor cell differentiation, perineural and lymphovascular invasion, low or absent stromal lymphocyte infiltration.

Introduction: Complex multi-fragmentary fractures of the proximal humerus are a rare type of injury, which represent a major challenge, even to the most experienced shoulder surgeons. Hemiarthroplasty is commonly accepted as the first surgical choice of treatment for three or four-part fractures of the proximal humerus. Case Report: A 62-year-old female who has sustained complex multi-fragmentary trauma of the proximal humerus presented to the clinic after a low-energy trauma. The patient was managed with a long-stemmed shoulder hemiarthroplasty and cerclage wiring of the fracture area. A good clinical and radiological outcome was observed 5 months post-operatively. Conclusion: We report the functional, anatomical and radiological outcome of a case of a rare type of proximal humeral fracture that was managed with a long-stemmed hemiarthroplasty and cerclage wire and has shown a good early clinical outcome.

Introduction: The main advantage of perforative techniques is their atraumatic, non-invasiveness and possibly being done under arthroscopic control. Purpose: This study aims to compare two methods of operative techniques in the treatment of chondral lesions of the knee. Materials and methods: The clinical study included 237 symptomatic patients with chondral defects from the period of 2010 – 2018. Patients were divided into 2 groups. The first group included 49 (32.11%) patients with arthroscopic micro-fractures. The second group included 54 (35.49%) patients with arthroscopic subchondral perforation (drilling). The term of follow-up was 36 months. Each patient had at least 4 documented follow-up visits. Results: Results from grading scales – Lysholm, KOS, MRI – scales, as well as our own scale for evaluating knee function (PS) scale, were documented in each patient's separate file. The average pre-operative Lysholm score was 41.5 ± 6.6 in the microfracture group and 43.0 ± 5.6 in the perforation group. At the 36th-month average, Lysholm's score increased to 86.9 ± 7.3 in the microfracture group and to 86.1 ± 6.6 in the perforation group. During the study, it was also found that a correlation exists between the received results and the age of the patient, the size of the chondral defect, and the tracking period. Conclusion: The effectiveness of osteoperforative techniques relies on many factors: the age of the patient, the size of the defect, and the tracking period. In the end, mesenchymal stimulation of chondrogenesis results in positive mid-term results in patients with small-sized chondral lesion.

Materials and methods: The clinical study included 237 symptomatic patients with chondral defects from the period of 2010 – 2018. Patients were divided into 2 groups. The first group included 49 (32.11%) patients with arthroscopic micro-fractures. The second group included 54 (35.49%) patients with arthroscopic subchondral perforation (drilling). The term of follow-up was 36 months. Each patient had at least 4 documented follow-up visits. Results: Results from grading scales – Lysholm, KOS, MRI – scales, as well as our own scale for evaluating knee function (PS) scale, were documented in each patient's separate file. The average pre-operative Lysholm score was 41.5 ± 6.6 in the microfracture group and 43.0 ± 5.6 in the perforation group. At the 36th-month average, Lysholm's score increased to 86.9 ± 7.3 in the microfracture group and to 86.1 ± 6.6 in the perforation group. During the study, it was also found that a correlation exists between the received results and the age of the patient, the size of the chondral defect, and the tracking period. Conclusion: The effectiveness of osteoperforative techniques relies on many factors: the age of the patient, the size of the defect, and the tracking period. PURPOSE: The article aims to determine the most common problematic aspects and challenges ahead of the medical professionals working in emergency medical care in the Republic of Bulgaria. For that reason, scientific developments, reports and publications of researchers and experts have been studied and analyzed. MATERIALS AND METHODS: A survey was conducted via an interview and direct anonymous questionnaire throughout the first quarter of 2019 amongst 325 respondents. RESULTS: The medical professionals providing emergency medical care face numerous challenges that require instant and adequate measures. Options for overcoming the difficulties that medical professionals face daily are sought at all levels of the hierarchy. A lot of work is done in the direction of a real interaction between all the subjects. CONCLUSIONS: The issues of the EMC require complex, radical measures.

Isotretinoin is an effective treatment for severe and resistant forms of acne. It has proven potential for inducing dose-dependent psychiatric side effects, especially in some vulnerable groups of patients. Objective: The aim of the present article is to report a case of a 22-year-old woman with a family history of schizophrenia who was diagnosed with acne. After unsuccessful antibiotic treatment of acne, a panic disorder accompanied by depressive symptoms manifested. Isotretinoin 20 mg/day was used as а second step of the dermatological treatment for a period of four months, along with psychotropic treatment for panic disorder – sertraline 50 mg/day and flupentixol 1,5 mg/day. When the dose of Isotretinoin was increased to 40 mg/day, paranoid delusions occurred. The manifestation of psychotic symptoms required discontinuation of all medications and initiation of antipsychotic therapy with quetiapine. The psychotic symptoms completely disappeared when the dose of quetiapine was increased to 300 mg/day. Conclusion: The presented case emphasizes the need for increased vigilance about the manifestation of psychiatric symptoms, especially in patients with a family history, current or past medical history of mental disorder. In such cases, consultation with a psychiatrist before starting treatment with Isotretinoin is recommended in order to better clarify the benefits and the risks of the treatment. The endogenous origin of the psychosis was discussed in the differential diagnostic plan, which requires long-term monitoring of the mental state.
