Dry Needling for Chronic Lumbar Radiculopathy with Knee Osteoarthritis in Elderly 干针治疗老年人慢性腰根病合并膝骨性关节炎

Abstract Background: Studies have shown about 83% of the elderly in pain are experiencing chronic pain at a given time. Hence it is important to explore some advance, effective treatment techniques in their rehabilitation Case Report: A 67 year old women came with complaints of radiating low back pain and pain in the medial knee joint line since 10 years. She was conservatively managed with rest and medications all these years with no referral to physiotherapy. She was thoroughly assessed and her pain mechanism was ascertained. Accordingly, electrical and thermal modalities were used and exercises were given for the first three sessions, attaining no relief of symptoms. Hence, she was started on nerve and myofascial dry needling with consent, post which she achieved good relief. Conclusion: It is important that we do not limit the rehabilitation of geriatric population to the traditional forms and should explore the advance techniques.

Freezing of Upper Limbs in Parkinson’s Disease: A Systematic Review 帕金森病上肢冻结:一项系统综述

Abstract Aim To identify how FOUL is investigated in patients with PD. Methods The Preferred Reporting Items for Systematic Reviews guideline was used, and the search was carried out in four databases, with no limit on publication date. Results 21 original articles were included. They used a variety of terms for upper-limb freezing. Some tasks evaluated only the movement of the limbs; others used associated objects. The tasks most often used to assess FOUL were flexion and extension of the index finger, finger tapping (index finger on thumb), and Funnel Task. In these tasks, movements of small amplitude/high frequency and the presence of a dual-task elicited more FOUL episodes. Conclusions This review highlights the need for development and validation of FOUL detection and assessment in clinical practice. The use of an appropriate assessment will prevent false-negative results and allow the phenomenon to be identified and treated.

Sleep Quality, Body Mass Index and Waist-to-Hip Ratio in Older Adults 老年人的睡眠质量、体重指数和腰臀比

Abstract Introduction Sleep quality and weight are inversely correlated in young adults (i.e., as weight increases, sleep quality decreases), but results are still inconsistent among older adults. Objective To examine the association between body mass index (BMI) and the waist-to-hip ratio (WHR) with sleep quality in older adults. Method 513 participants >60 years old were recruited. Data on BMI, WHR, age, gender, alcohol consumption, depression, comorbidities, and sleep (Epworth Sleepiness Scale (ESS) and Pittsburgh Sleep Quality Index (PSQI)) were collected. Univariate and multivariate linear regression methods were used to evaluate the association between BMI, WHR and PSQI. Results 503 participants were included. There was a significant association between BMI (β = 0.10 95%CI: 0.04 to 0.15) and PSQI in the linear analysis, but not confirmed in the multivariate regression. No significant associations were observed between WHR and PSQI. Conclusion Neither BMI nor WHR were related to sleep quality in the older population.

Effect of Walking Training with Textured Insole Socks in Older Adults 纹理鞋垫袜子对老年人步行训练的影响

Abstract Aims To evaluate the effects of textured insole socks on balance, gait parameters, and fall risk in older adults at risk of falling. Methods Thirty older adults at risk of falls participated and were randomly allocated equally to the intervention (texture insole socks) or control (unmodified socks) groups. Both groups were asked to walk on a 10-meter walkway for 2000 steps/day, 3 days/week for 4 weeks. Outcomes were measured using the Mini-Balance Evaluation Systems test, the modified 30-second chair stand test, and Gait Analyzer application. Results Textured insole socks increased balance and decreased the risk of falls in older adults to the same extent as unmodified socks. The beneficial effect on balance lasted at least for 2 weeks. No significant changes were observed in gait parameters for either group. Conclusions These interventions may be viewed as low-cost instruments for improving balance and lowering the risk of falls in older adults.

Understanding Identity Negotiation of Parkinson’s Disease and Occupational Engagement Using Narrative Inquiry 运用叙事探究了解帕金森病的身份协商与职业投入

Abstract Aims Many older adults with Parkinson’s disease experience challenges negotiating identity, often with negative implications for wellbeing. This study explores how older adults with Parkinson’s disease negotiate identity post-diagnosis, specifically addressing how they recount their management of identity and its impact on their occupational engagement. Methods Using a constructivist narrative approach, three participant sessions were conducted with five older adults with Parkinson’s disease. Line-by-line and thematic coding was completed. Results Three major themes emerged across narratives including: (a) Attempts to convey, maintain, and negotiate identity; (b) Resisting a disabled identity and (c) The centrality of occupation and social roles in negotiating identity. Conclusion Common themes of personal and social identity that threaten or support an older adult with PD’s decisions about occupational engagement were identified. Results of this study can be used by occupational therapists to better understand the influence of identity on occupational engagement.

Benefit of Ruesi Dadton on Oxidative Stress and Physical Performance: Quasi-Experimental Study Ruesi-Dadton对氧化应激和身体机能的益处:准实验研究

Abstract This study aimed to investigate the effect of Ruesi Dadton (RD) group exercise on oxidative stress (OS) levels and health conditions in healthy elderly individuals. This quasi-experimental study evaluated 23 healthy elderly participants, with evaluations at baseline, and 4 and 12 weeks after RD program initiation. RD consisted of 15 postures performed three times a week. OS was measured through levels of malondialdehyde (MDA) and 8-hydroxy-2-deoxyguanosine (8-OHdG), and activities of superoxide dismutase (SOD), glutathione peroxidase (GPx), and catalase (CAT). Blood pressure (BP) and balance were also observed. The results revealed that RD yielded significantly improved balance and CAT activity and decreased MDA level at 4 weeks. At 12 weeks, significant improvements in BP, balance, and SOD, CAT, and GPx activities and decreases in MDA and 8-OHdG levels were observed. These results suggest that RD can improve BP and balance by decreasing OS and increasing antioxidant levels in healthy elderly individuals.

Therapeutic Regime to Ameliorate Quality of Life in Patients with Parkinson’s Disease 改善帕金森病患者生活质量的治疗方案

Abstract Aim As various physiotherapeutic approaches are available to improve either motor or non-motor symptoms, the study was conducted for evaluating the effectiveness of Institution based Rehabilitation protocol to improve quality of life in patients with Parkinson’s Disease (IRQOL-PD) to focus on both types of symptoms simultaneously. Method The non-randomized controlled trial pilot study recruited 42 participants with Parkinson’s disease (age between 35-80 years) for 5 weeks of treatment sessions to evaluate the validity, reliability, and effectiveness of the designed protocol. The Unified Parkinson’s Disease Rating Scale (UPDRS) and Hoehn and Yahr scale (H&Y scale) were used as outcome measures. Result The IRQOL-PD with content validity index (0.9), intra-rater reliability (Intra-class Co-relation Coefficient = 1 and Cronbach’s alpha = 1) and inter-reliability (Pearson correlation coefficient > 0.90 and <0.50 for UPDRS and H&Y respectively) established the statistical and clinical improvement in experimental group related to UPDRS grading (p-value = 0.008). Conclusion IRQoL – PD protocol was effective to delay the progression of disease without any requirement of special equipment.

Abstract Aims To assess if Pain Neuroscience Education (PNE) delivered to older adults can positively influence pain knowledge and pain beliefs. Methods A total of 55 older adults attended a 30-minute PNE lecture. The primary outcome measures of pain knowledge, fear avoidance, beliefs regarding pain and aging as well as self-reported pain were obtained both pre- and post-lecture. Results Significant improvement in knowledge was found with a mean score on the neurophysiology of pain questionnaire test improving by 11.07% from pre to post-test (p = 0.002). A greater shift in knowledge was observed in the older adult group (70 and above) compared to the younger group (50–69). Conclusions A brief PNE lecture to older adults positively influences pain knowledge, and beliefs regarding pain and aging. This study indicates that PNE can be understood by older adults and may be a viable non-pharmacological treatment for older adults experiencing pain.

Purposes of Going out and Subjective Well-Being in Older Adults with Impairments 老年人外出与主观幸福感的目的

Abstract Aims To examine the association between diverse purposes of going out and the participant’s subjective well-being and their orientation toward going out. Methods We conducted a self-administered questionnaire survey to assess the association between orientation toward going out, purposes of going out, and the Philadelphia Geriatric Center Morale Scale (PGCMS) score in adults > 65 years. Results A regression analysis of 138 responses using a multistage adjustment approach showed no significant association, only with diverse purposes of going out (b: −1.74; 95%CI: −4.17, 0.69). In contrast, the interaction term was significantly associated with the PGCMS score (b: 3.28; 95%CI: 0.38 6.17). Conclusions Having diverse purposes of going out was associated with a higher level of subjective well-being in individuals with an orientation toward going out, suggesting the importance of increasing the variety of purposes of going out.

Predicting First Time Falls: Validating a Novel Algorithm in Long Term Care 预测第一次跌倒:验证长期护理中的新算法

Abstract Aim To determine the predictive validity of the 1stFall algorithm in long-term care (LTC) residents across four Canadian provinces. Methods This retrospective cohort study included all clients admitted to LTC between 2006-2017 with no history of falls in the past 30 days. The outcome was occurrence of a fall and logistic regression analysis was performed to assess predictive validity. Results A total of 199,997 LTC residents were studied (71% were >80 years old, 66% women, and 17% had severe cognitive impairment). For the total sample, clients in the 2nd, 3rd, 4th and 5th risk categories had 1.15, 1.58, 2.66, and 3.76 times greater odds of falling than the 1st category, respectively. Similar trends were observed across provinces. Conclusions 1stFall was developed to predict the risk of a first-time fall event in individuals with no history of a recent fall. 1stFall identified LTC residents at risk of a first-time fall, supporting its use in routine care.
