The Industrialization of "Liberal Medicine" in France: A Labor Quality Conventions Approach 法国“自由医学”的产业化:劳工素质公约的途径

This article seeks to analyze the evolution of the regulation of liberal medicine in France from the theoretical framework of the economics of convention. The recent introduction by the state of multiple management devices aimed at quantifying and evaluating the performance of physicians could be interpreted as a process of rationalization of medical practices. However, we propose to analyze the transformations in the regulation of liberal medicine as the transition from an inspired/domestic convention of healthcare quality to an industrial convention of healthcare quality. What is at stake is not improving the quality of care, but changing the conception of quality. Do doctors treat sick people or illnesses? This induces significant changes not only in the entire healthcare system but also in medical ethics. While the profession has historically been built against the market, it seems that the industrialization of healthcare opens the door to its commodification.

»Nur zwei Seiten derselben Medaille? Ethische Fragen und Diskurse uber COVID-19 und Ebola. Eine vergleichende Literaturanalyse«. Infectious diseases pose a continuing threat to human life. In the case of pandemics, they can also grow into massive challenges for society as a whole – not only from a medical but also from an ethical perspective. This article takes the current COVID-19 pandemic as the occasion for an empirical medico-ethical analysis. It explores the ethical dimensions and discourses on COVID-19 and the Ebola epidemics (West Africa, Democratic Republic of Congo). Additional attention is paid to the question whether and to what extent the ethical issues raised differ and how the possible disparities can be explained. Using a methodo-logical two-step approach (systematic literature review;qualitative content analysis), we were able to identify nine categories that map the ethical dimensions of recent outbreaks of these two diseases: (1) Prioritization of health, (2) Equitable access to resources, (3) Adequate information, (4) Health worker vulnerability, (5) Stigma and discrimination, (6) Research ethics, (7) Measures restricting freedom, (8) Global health justice, (9) Environmental ethics. © 2021, GESIS - Leibniz Institute for the Social Sciences. “Targeting Lifestyle" Conditions: What Justifications for Treatment? “针对生活方式”的疾病:治疗的理由是什么?

Patients suffering from “lifestyle” conditions are most often viewed as responsible for their illness, and so not considered to be a priority for healthcare resources. Instead, their treatment is financed on instrumental grounds: it is better to treat the condition now than to incur higher costs later of not doing so. An alternative register of justification at work in public healthcare policies is not motivated by instrumental considerations. Instead, it seeks to articulate an ethical case for prioritizing lifestyle conditions. Within this framework, we draw on the notion of vital need within the tradition of humanistic philosophy to argue that solidarity justifies the treatment of such conditions, exemplified here by obesity. We use the theoretical framework of economics of convention to present these two registers of justification at work in public healthcare policies. The importance of humanistic criticism prevents instrumental logic from being completely dominant.

 Abstract: »Medizin und Wirtschaftswissen: Die Relevanz der Karriere für das Studium der Transformationen im Gesundheitssystem« . This contribution highlights the relevance of biographical research for the economic sociology of the healthcare system. In particular, the economy of conventions could be enriched by an approach that is suitable for studying the actors of economic coordination as well as for analyzing the significance of economic knowledge within the framework of specific conventions. This contribution is based on an exploratory study on the economization of the healthcare system in Switzerland. Biographical interviews were conducted with 14 influential experts and senior managers. The careers of four physicians with a strong vocation to influence the public healthcare system are presented. In the concluding discussion, the findings are compared with economic sociology research on the healthcare system in the US and France. The conclusion states that the biographical perspective brings additional aspects into light that are not taken into account by more traditional concepts and methods of economic sociology.

The introduction of digital technology has sparked new debates about the value of community health workers in low-and-middle-income countries. This debate offers important insights into the conventions that are relevant in global public health. Community health workers, a workforce that was already celebrated during the 1970s Primary Health Care movement, are having a remarkable revival in recent years, and myriad actors seek to boost their impact through mobile devices. Our content analysis of the public health literature evaluating this impact reveals the centrality of attempts at reconciling equity and cost effectiveness concerns, and thus considerable normative tensions. Additionally, we find that discussions about “domestic” values such as privacy and gender roles come with a paternalistic undertone, calling for feminist and postcolonial engagement with the digitalization of community health work.
