BACKGROUND Estrogens play a key role in the skin: they are associated with an increased production of melanin, proliferation of melanocytes, increased skin thickness and increased cutaneous vascularization. Spitz and Reed nevi are acquired melanocytic lesions that generally develop during childhood or adolescence, a period of changes in sex hormones background. Our study project aimed at investigating, through immunohistochemical analysis, the expression levels of ERß receptors and their expression patterns (cytoplasmic or nuclear) in Spitz and Reed nevi. METHODS In our study we collected a total of 86 melanocytic lesions of patients: of these, 16 were common nevi, 23 were Spitz nevi, 18 were Reed nevi and 29 were melanomas. Expression curves for estrogen receptors were constructed using the Kaplan-Meier method and compared using a log-rank test.Statistical analysis was performed using MedCalc®. Immunohistochemical analysis on all histological sections of nevi and melanomas was performed to evaluate the expression levels of of ERß and their expression patterns (cytoplasmic or nuclear). The agreement between the operators was calculated using Fleiss κ values. RESULTS Correlation between immunoreactivity for the β-estrogen receptor and the sex of patients with Spitz and Reed nevi: immunoreactivity was higher in male patients. Correlation between β-estrogen receptor immunoreactivity and patient age for Spitz and Reed nevi: No statistically significant correlation was observed. Correlation between immunoreactivity for the β-estrogen receptor and histotype: Spitz and Reed nevi showed a high intensity, while in common nevi and in melanomas the immunoreactive was low. Correlation between receptor immunoreactivity for β estrogens and Breslow thickness in melanomas: Breslow thickness of non-immunoreactive melanomas for ERβ was much higher than those showing high immunoreactivity for this reeptor. CONCLUSIONS Spitz and Reed nevi express a higher immunoreactivity for estrogens than common nevi and melanomas, especially those with a high Breslow thickness; and immunoreactivity is higher in younger age groups.

BACKGROUND Hidradenitis suppurativa (HS) is a chronic, inflammatory disease that presents with nodules, abscesses and fistulae affecting the apocrine gland-bearing skin. Since few years, ultrasonography is used to better characterize HS skin lesions but comparison between clinical and sonographic scores has been made only in small series of patients. The aim is to assess concordance between clinical and sonographic scores in a larger cohort of HS patients. METHODS We conducted a retrospective observational study on 140 HS patients comparing two clinical score systems, Hurley and HS-Physical Global Assessment (PGA), with two ultrasonographic scores, SOnographic Score of Hidradenitis Suppurativa (SOS-HS) and ultrasonographic HS-PGA, a novel ultrasound scoring system set up by our group. We used the weighted Cohen's Kappa statistic (k) to evaluate the agreement. RESULTS Agreement between Hurley staging and SOS-HS was found in 61.4% of patients (weighted kappa 0.477), while agreement between clinical and ultrasonography HS-PGA scores was found in 35% of patients (weighted kappa 0.278), both findings suggesting a substantial disagreement. CONCLUSIONS Our study demonstrated a relevant disagreement between clinical and ultrasonography scores. Ultrasonography discovered non-clinically evident HS lesions, notably fistulae. Taken together, the above findings may support the view, previously suggested in expert panel reports, on the higher sensitivity of ultrasonography compared to clinics in HS.

BACKGROUND Psoriasis is a chronic, immune-mediated, inflammatory skin disease where interleukin-17 plays an important role in the underlying pathogenesis. IL-17A is a key driver of pro- osteoclastogenic process, that is abnormal in psoriatic patients. Aim of this study was to investigate in vivo the capability of secukinumab to influence the osteoclastogenesis in psoriatic patients reporting also our experience regarding the effectiveness and safety profile of this biological treatment. METHODS Efficacy and tolerability of secukinumab were evaluated after the induction period and 12 weeks. Moreover, to investigate how the cutaneous clinical improvement impacted also on the osteoclastogenic profile of psoriatic patients, the osteoclastogenic assay was performed in 5 secukinumab-treated psoriatic patients, not including psoriatic arthritis, before and after 5 weeks of therapy. RESULTS In line with clinical trials, our results confirmed the efficacy, speed of achievement and safety of secukinumab for the treatment of psoriatic patients. CONCLUSIONS Moreover, in our experiments, secukinumab has demonstrated speed of action also on osteoclastogenic process, reducing the number and activity of osteoclasts only after five weeks of treatment.

BACKGROUND Psoriasis is a common inflammatory skin disease characterized by uncontrolled proliferation of keratinocytes and recruitment of T lymphocytes into the skin. Possible triggers for psoriasis have been attributed to drugs or pathogens such as bacteria and possibly virus. Human endogenous retroviruses (HERVs) might play a role in triggering these anti-viral immune responses, since the role of HERVs in the pathogenesis of autoimmune diseases has generated considerable interest. Some studies have also reported an association of HERV-E and psoriasis. None of them investigate the HERV-E expression in peripheral blood of psoriasis. All these considerations have prompted us to perform a survey for HERV-E expression in PBMC from psoriatic patients. METHODS Peripheral blood mononuclear cells from 69 psoriatic patients were analyzed. Total RNA was extracted and amplified with reverse transcription polymerase chain reaction. Results were compared with those obtained in a cohort of 20 healthy donors. RESULTS HERV E was expressed in all samples analyzed but the level of expression was much lower in the psoriasis that in HC p<0.0001. CONCLUSIONS The reasons for the unexpected, low levels of HERV expression in psoriatic patients are unclear and might be in part a consequence of anti-viral defense mechanisms.
