Physical and Cognitive Training to Enhance Intensive Care Unit Survivors' Cognition: A Mapping Review. 增强重症监护病房幸存者认知能力的体能和认知训练:一项研究综述。

Purpose: The aim of this study was to examine current literature regarding effects of physical or cognitive training and simultaneous (dual-task) physical and cognitive training on cognition in adults surviving an intensive care unit (ICU) stay.

Design: Systematic mapping.

Methods: A literature search was conducted to examine effects of physical and/or cognitive training on cognitive processes.

Results: Few studies have targeted adults surviving ICU. Independently, physical and cognitive interventions improved cognition in healthy older adults with and without cognitive impairment. Simultaneous interventions may improve executive function. Small sample size and heterogeneity of interventions limited the ability to make inferences.

Conclusion: Literature supports positive effects of single- and dual-task training on recovering cognition in adults. This training could benefit ICU survivors who need to regain cognitive function and prevent future decline.

Relevance to practice: With the growing number of ICU survivors experiencing cognitive deficits, it is essential to develop and test interventions that restore cognitive function in this understudied population.

Stress and Resilience Among Healthcare Workers During the COVID-19 Pandemic: Consideration of Case Studies. COVID-19大流行期间医护人员的压力和复原力:对案例研究的考虑

Abstract: The COVID-19 pandemic has caused a significant increase in stress for frontline healthcare workers, including rehabilitation workers. Contributing factors include disrupted workflows, heavier workloads, increased time restraints, and fear of contracting/passing the virus. Prolonged high stress levels can produce adverse health outcomes when unaddressed. Resilience can mitigate the negative effects of prolonged stress. Four healthcare workers relate their experiences from the frontlines of the pandemic, discussing their strategies to build resilience and maintain health. Highlighted strategies include mindfulness (the purposeful act of paying attention to the present moment without judgment), gratitude (the practice of being grateful for the positive things in life), self-care (the maintenance of a healthy lifestyle using physical, psychological, and emotional tools), and social support (the sense of belonging that comes from being cared for and valued). These strategies reduce negative outcomes produced by elevated stress levels and promote resilience in frontline healthcare workers.

Development and Validation of the Stroke Symptom Cluster Scale Among Korean Stroke Survivors: Mixed-Methods Design. 韩国中风幸存者中风症状聚类量表的开发与验证:混合方法设计。

Purpose: This study aimed to develop and validate the Stroke Symptom Cluster Scale for Korean Adults (SSCS-K) for stroke survivors.

Design: An exploratory sequential study with a mixed-methods design was performed.

Methods: In the development stage, a qualitative study with in-depth interviews was conducted with 27 stroke patients. The validation stage involved analyzing the psychometric properties from 288 stroke patients.

Findings: The SSCS-K comprising 65 items in six dimensions (mobility, cognition, sensory, mood, communication, and swallowing difficulty) demonstrated acceptable internal consistency (Cronbach's α = .92-.94). Confirmatory factor analyses with a six-factor solution showed that it explained 62% of the variance in stroke symptoms. The concurrent validity was confirmed with the Stroke-Specific Quality of Life Scale (r = .38-.83, p < .001).

Conclusion: The SSCS-K has strong psychometric properties as a measure to assess clustered symptoms in stroke survivors during their long-term rehabilitation.

Clinical relevance: Rehabilitation nurses may consider the SSCS-K as a useful tool for assessing symptoms of stroke survivors.

Purpose: The aim of this study was to describe and analyze dosing patterns for patients with ITB treatment over time and to identify possible subgroups demonstrating diversity in patterns.

Design: A retrospective design.

Methods: For 81 patients from six different hospitals, baclofen doses from the first 2 years of treatment were identified using medical records. Line graphs of each patient's doses were analyzed and grouped based on similarities in dosing pattern.

Findings: The analyses of the dosing patterns resulted in four different subgroups classified as stable, slow increase, rapid increase, and fluctuating.

Conclusion: The results highlight the clinical challenge of predicting dose development over time.

Clinical relevance to rehabilitation nursing: This study provides rehabilitation healthcare professionals with a better understanding of intrathecal baclofen dose development. Illustrations of the four subgroups can be used as an educational tool for patients, family, and caregivers.

Designing and Evaluating a Goal-Setting Tip Sheet for Stroke Family Caregiver Health. 设计和评估卒中家庭照顾者健康目标设定提示表。

Purpose: The aim of the study was to design and evaluate a goal-setting tip sheet to improve stroke family caregiver health as part of the Telephone Assessment and Skill-Building Kit program.

Design: A multimethod design using open-ended questions and rating forms was used in this study.

Methods: In two studies, the goal-setting tip sheet was evaluated by caregivers (N = 12; N = 6), researchers (N = 10; N = 3), clinicians (N = 8; N = 4), and technology experts (N = 4; N = 4). Rigorous qualitative content analysis procedures were used with descriptive statistics for ratings.

Findings: Qualitative strengths and areas for improvement refined the goal-setting tip sheet. From 1 = strongly disagree to 5 = strongly agree, ratings averaged from 3.25 to 5.00 for strategies, accuracy, feasibility, and acceptability.

Conclusions: The goal-setting tip sheet was found to be accurate, feasible, and acceptable. Future research is needed to determine the impact on caregiver health.

Clinical relevance: Stroke caregivers often neglect their own health; goal setting has the potential to improve caregiver health.

Introducing Standardized Assessment and Management of Oral Cares in a Rehabilitation Unit: Patient Outcomes and Experiences. 在康复病房引入口腔护理的标准化评估和管理:患者的结果和经验。

Purpose: The aim of this study was to improve patient oral hygiene outcomes in a rehabilitation unit by implementing a nursing education package and oral hygiene assessment tool.

Design: A case-control design with 50 rehabilitation patients was performed.

Methods: Nursing staff received education and training in applying the Modified Oral Health Assessment Tool. Clinician assessment of patient oral hygiene occurred on admission and at days 5-7. Each patient reported their perceptions of oral hygiene and comfort prior to hospitalization, while in the hospital, and after transfer to the rehabilitation unit.

Findings: Oral hygiene rating scores improved significantly from admission to the rehabilitation unit to days 5-7 (p = .00). The mean score of patient perceived cleanliness improved from hospital admission to admission to the rehabilitation unit.

Conclusion: Oral hygiene was improved following admission to a rehabilitation unit with a consistent and individualized approach to oral hygiene.

Clinical relevance: The introduction of a consistent and individualized approach to oral hygiene demonstrates positive patient outcomes and high patient acceptance.

{"title":"Introducing Standardized Assessment and Management of Oral Cares in a Rehabilitation Unit: Patient Outcomes and Experiences.","authors":"Maria Schwarz,&nbsp;Inger Kwiecien,&nbsp;Blaise Hamlet,&nbsp;Anne Coccetti,&nbsp;Ursula Monsiegneur,&nbsp;Benjamin Tickle,&nbsp;Elizabeth Cardell","doi":"10.1097/rnj.0000000000000301","DOIUrl":"https://doi.org/10.1097/rnj.0000000000000301","url":null,"abstract":"<p><strong>Purpose: </strong>The aim of this study was to improve patient oral hygiene outcomes in a rehabilitation unit by implementing a nursing education package and oral hygiene assessment tool.</p><p><strong>Design: </strong>A case-control design with 50 rehabilitation patients was performed.</p><p><strong>Methods: </strong>Nursing staff received education and training in applying the Modified Oral Health Assessment Tool. Clinician assessment of patient oral hygiene occurred on admission and at days 5-7. Each patient reported their perceptions of oral hygiene and comfort prior to hospitalization, while in the hospital, and after transfer to the rehabilitation unit.</p><p><strong>Findings: </strong>Oral hygiene rating scores improved significantly from admission to the rehabilitation unit to days 5-7 (p = .00). The mean score of patient perceived cleanliness improved from hospital admission to admission to the rehabilitation unit.</p><p><strong>Conclusion: </strong>Oral hygiene was improved following admission to a rehabilitation unit with a consistent and individualized approach to oral hygiene.</p><p><strong>Clinical relevance: </strong>The introduction of a consistent and individualized approach to oral hygiene demonstrates positive patient outcomes and high patient acceptance.</p>","PeriodicalId":49631,"journal":{"name":"Rehabilitation Nursing","volume":null,"pages":null},"PeriodicalIF":1.3,"publicationDate":"2021-09-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"38711168","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":4,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Grief and Loss Among Veterans With Spinal Cord Injury: A Qualitative Study. 脊髓损伤退伍军人的悲伤和损失:一项定性研究。

Purpose: The objective of this study was to understand veteran perspectives of grief/loss associated with their spinal cord injury (SCI).

Design/methods: Qualitative descriptive design using semistructured interviews with veterans with SCI (n = 15) was performed using interview questions informed by the literature. Analysis was conducted using a deductive/inductive approach.

Results: The 15 participants' mean age was 64 years; six had paraplegia, eight had tetraplegia, one had unknown level of injury, six were ambulatory, and nine were not ambulatory. The grief/loss experience was individual and constant, consisting of seven phases: experiencing shock, releasing emotion, withdrawing, focusing on losses, problem solving, realizing strength and motivation, and accepting a new life. Veteran coping strategies used in each phase of the grief experience were also identified, as well as triggers that increased or decreased the grief/loss experience.

Conclusions/clinical relevance: A better understanding of veterans living with SCI grief experience, coping strategies, and triggering events can help healthcare providers support veterans with SCI.
