Examination of risk appraisal during the return to sport following anterior cruciate ligament reconstruction 前交叉韧带重建后恢复运动的风险评估检查

Objectives

To describe features of risk appraisal after anterior cruciate ligament reconstruction (ACLR).

Study design

Secondary analysis.

Methods

161 patients with ACLR, cleared to initiate return to sport, were tested (initial and follow-up 6 weeks later). Patient characteristics, ACL-Return to Sport after Injury (ACL-RSI) score and limb symmetry index (LSI) on 4 single-leg hop tests were collected. ACL-RSI confidence domain and scores on risk appraisal items (#2: reinjure knee, #10: surgery/rehabilitation; both with respect to playing sports) were calculated. Risk appraisal items were analyzed for: change over time using effect sizes; correlations with age, sex, confidence domain score, and hop LSIs; and differences based on meeting readiness-for-sport criteria (ACL-RSI ≥70, hop LSIs ≥90 %).

Results

Risk appraisal item scores increased at follow-up (δ = 0.22, 0.26); higher scores were correlated with younger age (ρ = −0.25, −0.39), higher confidence domain score (ρ = 0.67, 0.60) and higher timed hop LSI (ρ = 0.24, 0.27). Patients meeting ACL-RSI/hop LSI or ACL-RSI criteria had significantly higher risk appraisal item scores than those meeting hop LSI or no criteria (p < 0.001).

Conclusions

Risk appraisal may evolve following ACLR. Patients with high confidence, and those exhibiting psychological readiness alone or in combination with physical readiness, tend to perceive lower risk with returning to sport.

Trunk strength and endurance testing in field-athletes: A reliability study 田径运动员躯干力量和耐力测试:可靠性研究

Objectives

Despite the important role of the trunk muscles in athletic performance and the onset of injury, consensus is lacking as to the best methods to assess trunk strength and endurance in athletic populations in the field. This study aimed to examine the intra-rater reliability of a battery of trunk strength and endurance tests.

Design

Test-retest reliability study.

Setting

Isometric trunk strength using a hand-held dynamometer (HHD) and isometric trunk endurance using the McGill protocol and novel ‘long-lever’ tests were evaluated.

Participants

Twenty-three male field-athletes.
Main Outcome Measures: Intra-class coefficient (ICC) and standard error of measurement (SEM).

Results

For relative reliability, good to excellent results were found for all HHD strength tests (ICC = 0.70–0.91). Relative reliability for trunk endurance tests showed excellent results for the McGill protocol (ICC = 0.80–0.89) and moderate to excellent reliability for the long-lever tests (ICC = 0.57–0.96). The V-Sit endurance test and the isometric lateral flexion strength test showed lower absolute reliability, with relatively high SEM values.

Conclusions

Therefore, we recommend practitioners assess athlete trunk function by using a HHD to evaluate isometric flexion, extension and rotation trunk strength and the Biering-Sørenson, and long-lever lateral flexion and flexion tests to assess trunk endurance.

Footwear recommendations for Runners: Influencing factors and professional perspectives 跑鞋建议:影响因素和专业观点

Objective

Runners value footwear advice provided by experts. It is therefore essential to understand the nature of these recommendations. This study identified factors influencing individualised running footwear recommendations and measured the relative importance placed on footwear characteristics related to comfort and injury management.

Design

Cross-sectional survey.

Setting

Online.

Participants

Podiatrists, physiotherapists, running coaches, and footwear retailers working in New Zealand.

Main outcome measures

Quantitative survey questions covering footwear recommendation habits, and beliefs regarding the functional capacity of running footwear.

Results

A total of 264 responses were received. Most participants identified comfort (96 %) and injury prevention (86 %) as important considerations in footwear selection. Retailers were more likely than other groups to emphasise the importance of arch height and foot motion (p < 0.001). Cushioning was rated the most influential characteristic for both comfort (99 %) and injury management (94 %). Most participants (91 %) thought medial posting influenced injury management, whilst shoe/upper width (95 %) and forefoot stiffness (93 %) were frequently linked to comfort. Significant differences emerged regarding beliefs about footwear functionality: 82 % of retailers believed that footwear could reduce pronation, compared to 55 % of podiatrists, 42 % of physiotherapists, and 29 % of coaches (p < 0.001).

Conclusions

Podiatrists and retailers provide individualised footwear advice more frequently and confidently and are more likely to base their recommendations on reducing pronation than physiotherapists and coaches.

Objectives

To examine associations between anthropometric properties and neck strength with linear and rotational accelerations during heading.

Design

Observational cohort study.

Participants

Eleven male footballers from a UK academy.

Main outcomes

Independent variables included head and neck circumference, head mass, head-neck segment mass, and strength. Dependent variables were mouthguard-derived linear and rotational acceleration.

Results

A total of 11 male footballer recorded 259 heading events (24 ± 8 per player). Trivial associations were observed between neck length, head-neck segment mass, and side flexion strength with linear acceleration (r = −0.04–0.18, p = 0.001–0.638). There was trivial-to-small association between neck circumference, head mass and strength in forward flexion and extension with linear acceleration (r = 0.05–0.33, p = 0.001–0.602). Neck circumference, head mass, and strength in forward flexion and extension showed trivial-to-small associations (r = 0.05–0.33, p = 0.001–0.602). Neck length, circumference, and side flexion strength showed trivial-to-large associations with rotational acceleration (r = −0.17–0.70, p < 0.001–0.166), while head mass, head-neck segment mass, and strength in flexion and extension had trivial-to-small associations (r = 0.01–0.75, p < 0.001–0.955). Fixed effects explained 4.3–6.5% of the variance in linear and rotational head accelerations.

Conclusion

Neck strength and anthropometric factors showed mixed associations with head accelerations during heading, explaining a small proportion of the variance in linear and rotational accelerations.

Objective

To determine whether chronic ankle instability (CAI) individuals exhibit hip muscle strength deficits compared to healthy people.

Methods

A comprehensive search of five databases was conducted from inception to May 2025 according to PRISMA guidelines. Quality assessment of included studies was performed using the modified Downs and Black checklist, and meta-analysis was conducted using random-effects models with results reported as standardized mean differences (SMD).

Results

Ten studies involving 548 participants (271 CAI, 277 healthy controls) were included. Individuals with CAI demonstrated significantly decreased isometric hip abduction (SMD: 0.56, P < 0.001), extension (SMD: 0.62, P = 0.003), and external rotation strength (SMD: 0.59, P = 0.005) compared to controls. No significant differences were found in isokinetic strength at 60°/s. Sensitivity analysis showed unstable results for hip external rotation strength, while other outcomes remained stable.

Conclusion

Individuals with CAI demonstrated significant deficits in isometric hip abduction and extension strength compared to healthy people. Hip muscle strength assessment should be incorporated into routine examination of CAI individuals.

Objective

The role of hip strength in the development of running related injuries is unclear. The aim of this study was to assess if runners who develop a running-related injury demonstrate less isometric hip strength than uninjured runners and to evaluate if runners demonstrating isometric hip weakness go on to experience more injuries than those with normal strength.

Design

Prospective cohort study.

Setting

University Campus.

Participants

Seventy-nine (29 male and 50 female) collegiate cross-country runners.

Main outcome measures

Running-related injury.

Results

Runners who sustained an injury demonstrated 13.6 % (p = 0.035) less hip abductor strength and demonstrated 16.6 % (p = 0.035) greater asymmetry in hip abductor strength. Runners identified as having a weakness in hip strength did not demonstrate a greater proportion of injuries than runners noted to have normal strength. The relative risk of sustaining a running-related injury was not greater in runners identified as having hip weakness.

Conclusions

Despite a theoretical link between hip weakness and the development of running-related injury, no difference in injury proportions was found in this cohort. However, differences in hip strength were identified between injured and uninjured runners. This may indicate that hip strength may mediate other factors that influence a runner's overall risk profile.

Objectives

This study aimed to investigate the associations of selected lower limb biomechanical variables with pain severity, disability, and postural balance in female athletes with Patellofemoral Pain Syndrome (PFPS).

Design

Cross-sectional study.

Setting

Biomechanics Laboratory.

Participants

165 female athletes were assigned to the PFPS group and 160 as a healthy controls group.

Main outcome measures

Dynamic knee valgus (DKV) was measured via the Kinovea software program, tibial torsion was measured via a universal goniometer, and the navicular height was measured via a height caliper tool. Postural balance under dynamic stress was quantified via the Biodex Balance System, Pain severity via the numerical pain rating scale (NPRS), and disability via the anterior knee pain scale (AKPS).

Results

DKV and tibial torsion showed significant differences between groups. DKV positively correlated with pain (r = 0.451) and postural balance (r = 0.244, 0.341, and 0.351 for balance scores), and negatively with disability (r = −0.432). Tibial torsion negatively correlated with disability (r = −0.339). Navicular height had no significant correlation with the measured variables.

Conclusions

Lower limb biomechanics, especially DKV and tibial torsion, should be prioritized in performance screening for female athletes. This study highlights the importance of addressing biomechanical correction strategies into rehabilitation programs for female athletes with PFPS.

Objectives

To explore the effect of an isometric weighted Copenhagen adduction (WCA) exercise programme and an isometric unweighted Copenhagen adduction (CA) exercise programme on hip adduction strength, abduction strength and adduction:abduction strength ratio.

Design

Randomized controlled trial, with flat outcome structure.

Setting

Rugby training facility.

Participants

Thirty-one male rugby players.

Main outcome methods

Between-group differences in hip adduction strength, abduction strength and adduction:abduction ratio were assessed using independent-sample t-tests. Within-group differences in hip strength were assessed using paired-sample t-tests.

Results

15 participants were assigned to the CA group and sixteen to the WCA group. Post-intervention, a significant between-group difference in hip adduction:abduction strength ratio was observed (0.09 Nm/kg, p = 0.032) with large effect size (ES = 0.812). No significant between-group differences were evident for hip adduction strength (0.21 Nm/kg, p = 0.058) or hip abduction strength (0.01 Nm/kg, p = 0.856). The WCA group demonstrated significant increases in hip adduction (0.22 N/kg, +10 %, p = 0.008) and hip adduction:abduction ratio (0.06 N/kg, +7 %, p = 0.036), but not hip abduction (0.08 N/kg, +4 %, p = 0.055), following the 8-week programme. No significant changes in hip strength were observed in the CA group.

Conclusions

A low-dose 8-week adductor exercise programme using the isometric WCA exercise elicits significant increases in hip adduction:abduction strength ratio when compared to an isometric CA exercise.

Objective

Establish test re-test reliability of three tests assessing hamstring function during hip extention and investigate their associations with maximal sprinting speed.

Design

Cross-sectional.

Setting

Biomechanics laboratory.

Participants

Twenty healthy, physically active men (age = 26 ± 5 years; height = 1.81 ± 0.07 m; body mass = 81 ± 7 kg).

Main outcome measures

Participants performed a maximal sprinting speed test on a non-motorised-treadmill (NMT) and three tests assessing hamstring function (hamstring scooter, hamstring push and hamstring switch), during test and re-test sessions seven days apart. Hamstring scooter peak horizontal force was measured using the NMT, while hamstring push and hamstring switch peak hip extension forces were measured using force plates (ForceDecks Lite, Vald Performance, Brisbane, Australia). We calculated each test's intraclass correlation coefficient (ICC) with 95% confidence intervals (95% CI) within (ICC2,1) test and re-test sessions, respectively, and between (ICC2,k) test and re-test sessions, along with Pearson's correlation coefficient (r-value) with maximal sprinting speed.

Results

Test re-test reliability ranged from moderate to excellent for all tests. The hamstring scooter was the only test with a statistically significant association (p < 0.01; r = 0.76) with maximal sprinting speed.

Conclusions

The tests described in this study all had moderate to excellent test re-test reliability, but only the hamstring scooter was associated with maximal sprinting speed.

Hickey","doi":"10.1016/j.ptsp.2025.07.005","DOIUrl":"10.1016/j.ptsp.2025.07.005","url":null,"abstract":"<div><h3>Objective</h3><div>Establish test re-test reliability of three tests assessing hamstring function during hip extention and investigate their associations with maximal sprinting speed.</div></div><div><h3>Design</h3><div>Cross-sectional.</div></div><div><h3>Setting</h3><div>Biomechanics laboratory.</div></div><div><h3>Participants</h3><div>Twenty healthy, physically active men (age = 26 ± 5 years; height = 1.81 ± 0.07 m; body mass = 81 ± 7 kg).</div></div><div><h3>Main outcome measures</h3><div>Participants performed a maximal sprinting speed test on a non-motorised-treadmill (NMT) and three tests assessing hamstring function (hamstring scooter, hamstring push and hamstring switch), during test and re-test sessions seven days apart. Hamstring scooter peak horizontal force was measured using the NMT, while hamstring push and hamstring switch peak hip extension forces were measured using force plates (ForceDecks Lite, Vald Performance, Brisbane, Australia). We calculated each test's intraclass correlation coefficient (ICC) with 95% confidence intervals (95% CI) within (ICC<sub>2,1</sub>) test and re-test sessions, respectively, and between (ICC<sub>2,k</sub>) test and re-test sessions, along with Pearson's correlation coefficient (r-value) with maximal sprinting speed.</div></div><div><h3>Results</h3><div>Test re-test reliability ranged from moderate to excellent for all tests.
