Treatment indications for symptomatic versus asymptomatic florid cemento-osseous dysplasia in adult patients: A systematic review 成年患者有症状与无症状花斑状骨水泥骨发育不良的治疗指征:系统回顾

The objective of this study was to identify treatment indications for symptomatic and asymptomatic florid cemento-osseous dysplasia (FCOD) in adult patients and explore relationships between clinical variables and radiographic findings (PROSPERO # CRD42023411228). A systematic review was conducted by independent investigators using databases: PubMed, ProQuest, Embase, Web of Science, Dentistry and Oral Sciences Database (DOSS), and TRIP to identify studies on FCOD treatment options in adults. Inclusion criteria for this systematic review included: originally in English; open-access; published between 2001 and 2021. Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines and the Joanna Briggs Institute (JBI) Critical Appraisal checklist were used for reporting and quality assessment of each study. From the initial 122 studies, 11 fit criteria for this systematic review. Eight studies reported symptoms as clinical presentation of FCOD, 6 reported swelling, and 3 reported infection. Five studies recommended surgical treatment in symptomatic patients with anatomic structure changes around the lesions, such as presence of necrotic bone and secondary infection. Treatment of asymptomatic FCOD was contraindicated in 10 studies. Dental prophylaxis was recommended. Dental prophylaxis and monitoring were the most common management strategies for asymptomatic and symptomatic FCOD. Surgical curettage with stimulation of bleeding, or pulp vitality testing of specific areas with periapical inflammation were treatment options for symptomatic cases. Is there a difference in quality of photographs obtained using smartphones, compared to the photographs obtained via digital single-lens reflex camera? 使用智能手机拍摄的照片与使用数码单反相机拍摄的照片在质量上是否存在差异?

The purpose of this study was to compare the quality of photographs obtained with 3 different cameras: iPhone, Samsung, and digital single-lens reflex (DSLR), as assessed by oral and maxillofacial surgeons (OMS). This was an anonymous online survey study. The study population consisted of OMS in New Jersey, New York, Pennsylvania, and Massachusetts who were members of their state societies. The primary outcome variable was ratings on the quality of extraoral and intraoral photographs obtained with 3 different cameras. The predictor variable was the type of camera used to obtain the photographs: iPhone, Samsung, and DSLR camera. Descriptive statistics were computed. Spearman test was utilized to analyze correlation between the outcome variables. The majority of respondents were from full time private practice (n = 84, 77.06%), followed by full time academia (n = 12, 11.01%). The Spearman test revealed negligible correlation between extraoral and intraoral photographs on the best quality rating (0.15, = .13). Negligible correlation between extraoral and intraoral photographs were noted on the second best (0.15, = .11), and least quality rating (0.09, = .32). Study results revealed that there is no difference between the quality of photographs obtained using smartphones, compared with the photographs obtained via a DSLR camera. Future discussions on an ideal application on smartphones would allow for consent and transfer of high-quality photographs via a secure messaging system between health care providers to address ethical concerns of confidentiality. Surgical management of frontal bone and naso-orbito-ethmoidal fractures: a 7-year multi-center retrospective review 额骨和鼻眶外侧骨折的手术治疗:7 年多中心回顾性研究

Frontal and naso-orbito-ethmoidal (NOE) fractures are rare but important injuries due to their anatomical complexity and proximity to vital structures. This study aims to describe the patient factors, procedural factors, and postoperative outcomes in the surgical management of these fractures. Retrospective review of patients aged 18 years or older who were surgically treated for these fractures at two Australian tertiary hospitals (2014-2020). Patient factors (demographics, mechanism of injury, fracture pattern, concomitant injuries); procedural factors (operation timing, surgical approach); and postoperative outcomes (complications, revision surgeries) were recorded. 60 cases were included (41 frontal; 29 NOE; 10 combined). Mean age was 37 (frontal) and 39 years (NOE). Most cases were male (87.8%; 75.9%). The most common mechanism was traffic accidents (29.3%; 34.5%). The most common associated facial fractures were Le Fort fractures (46.3%; 89.7%). The most common associated systemic injuries were brain injuries (36.6%; 34.5%). Mean operation timing was 13 days (frontal) and 11 days (NOE). Coronal flap was most commonly used (68.3%; 82.8%). Postoperative complication rates were 39.0% (frontal) and 37.9% (NOE). Three cases required revision surgeries. Further larger longitudinal studies are required to build knowledge and improve patient outcomes.

This study aims to describe the patient factors, procedural factors, and postoperative outcomes in the surgical management of these fractures. Retrospective review of patients aged 18 years or older who were surgically treated for these fractures at two Australian tertiary hospitals (2014-2020). Patient factors (demographics, mechanism of injury, fracture pattern, concomitant injuries); procedural factors (operation timing, surgical approach); and postoperative outcomes (complications, revision surgeries) were recorded. 60 cases were included (41 frontal; 29 NOE; 10 combined). Mean age was 37 (frontal) and 39 years (NOE). Most cases were male (87.8%; 75.9%). The most common mechanism was traffic accidents (29.3%; 34.5%). The most common associated facial fractures were Le Fort fractures (46.3%; 89.7%). The most common associated systemic injuries were brain injuries (36.6%; 34.5%). Mean operation timing was 13 days (frontal) and 11 days (NOE). Coronal flap was most commonly used (68.3%; 82.8%). Postoperative complication rates were 39.0% (frontal) and 37.9% (NOE). Three cases required revision surgeries. What are the incidence and costs associated with interfacility emergency department transfer patients treated by oral and maxillofacial surgeons? 由口腔颌面外科医生治疗的机构间急诊科转院病人的发生率和相关费用是多少?

Interfacility emergency department transfers (IETs) for maxillofacial trauma and infections are prevalent in the United States, with significant implications for health care costs. Limited availability of oral and maxillofacial surgeons (OMS) exacerbates unnecessary transfers and associated expenses. This study aimed to determine the incidence and costs of OMS IET. A retrospective cohort study was performed for maxillofacial trauma and infection IET at the University of Illinois Health main hospital via electronic medical record query for terms “infection [or] trauma [and] transfer.” Inclusion criteria required presentation from January 1, 2022 to June 30, 2022; emergency department (ED)-ED transfer; and or consultation by OMS. Distance transferred, insurance type, location of treatment, weekend presentation, and associated costs were collected. One thousand ninety-nine records were identified, including 46 trauma IET and 122 infection IET. Costs ranged from $2,683,918.90 to $7,984,912.89, indicating ∼$1.5 billion annual expenditure across US OMS programs. Three trauma IET required urgent treatment; no infection IETs were “emergent.” Trauma IET averaged 20.7 + 17.1 miles and infection IET 22.0 + 17.4 miles for transfer distance. Among trauma IET, 28 were treated in the operating room (OR), 10 had outpatient follow-up, and 8 had ED treatment. Among infection IETs, 57 received ED treatment, 56 received OR treatment, and 9 received antibiotics/no treatment. Maxillofacial trauma and infection IET impose significant health care costs. Increased funding for OMS training may mitigate the shortage and improve patient care. Further research is needed for better triaging and reimbursement strategies.
