The aim is to evaluate the anatomical and functional outcomes of retinal detachment (RD) surgery in the pediatric population in a tertiary eye hospital in Nepal. This study is a hospital-based, retrospective cross-sectional study, including patients under 18 years of age, diagnosed with rhegmatogenous RD (RRD) for 5 years from January 2015 to December 2019, who underwent RD surgery. Twenty patients were included in the study (mean age of 10.75 ± 2.63). Male patients were 19 (95%). A history of trauma was seen in 13 (65%) patients. Phakic RRD was observed in 18 (90%) patients and the rest 2 (10%) patients were pseudophakic. Type B proliferative vitreoretinopathy changes were observed in 3 (15%) patients, type C was observed in single patient (5%). Scleral buckling was performed in 7 (35%) patients, while 12 (60%) patients underwent three-port pars plana vitrectomy (PPV) and silicon oil instillation and the remaining single patient (5%) underwent PPV with gas exchange. Lensectomy was performed in 8 (40%) patients. Retinal attachment with a single surgical procedure was seen in 8 (40%) patients, while 12 (60%) patients required multiple surgeries. The mean duration of silicon oil removal was 5 ± 2.78 months (1–11 months). At the end of 6 months, 11 (55%) patients had improved visual acuity of 6/60. Six months postoperatively, 18 (90%) patients had attached retina. Visual improvement and retinal reattachment could be achieved with early diagnosis and intervention in pediatric RRD.

The objective was to report a rare case of ocular sporotrichosis in Fundação Banco de Olhos de Goiás. A 29-year-old pregnant woman presented with Parinaud’s oculoglandular syndrome. She referred to contact with a sick cat. The patient was submitted to a conjunctival culture and skin nodule biopsy, which revealed sporotrichosis. This report discussed this rare case of sporotrichosis presentation in the city of Goiania, Goiás state, Brazil.

Orbital cellulitis is a rare cause of exudative retinal detachment (ERD). We hereby present a case of orbital cellulitis associated with ERD from North India. A 42-year-old man presented with features of orbital cellulitis in the left eye for 3 days. An ERD lesion was found in the superior-temporal quadrant of the retina of the same eye. Investigations were performed; however, the cause of orbital cellulitis could not be found. Systemic antibiotics were started, leading to improved orbital cellulitis and resolution of ERD. Orbital cellulitis can be treated with systemic antibiotics. The cure of underlying disease leads to the resolution of ERD.

Parkinson’s disease, a neurodegenerative disease, involves motor and nonmotor components. Vision, a nonmotor component, has been studied by the analysis of the retinal layers by optical coherence tomography (OCT), and the main finding is the thinning of the retinal nerve fiber layer (RNFL). The aim of this study was to verify the influence of age on the thickness of retinal nerve fiber in patients with mild-to-moderate stages of Parkinson’s disease. Healthy individuals and patients with Parkinson’s disease were included in this prospective, cross-sectional, and observational study. The patients were recruited according to the clinical criteria from the neurology clinic of two major hospitals, with the description of the Hoehn and Yahr scale score. Healthy subjects were recruited from the general clinic and hospital staff. A comprehensive ophthalmic examination was performed previously on the spectral-domain OCT. There was a significant effect of age in the thickness of the RNFL in patients with Parkinson’s disease, in the right eye in the lower quadrant (P = 0.026) and in the left eye in the upper (P = 0.029) and temporal (P = 0.037) quadrants, as well in the control group in the upper (P = 0.003) and lower quadrant (P = 0.024). This case series could demonstrate that in mild-to-moderate stages of the Hoehn and Yahr Scale, there is an asymmetric correlation between aging and the decrease in superior, inferior, and temporal RNFL between the eyes of patients with Parkinson’s disease, and this finding precedes the decrease in visual acuity.

The purpose of the study is to evaluate whether there is a significant diurnal variation in the thickness of corneal epithelium and stroma, in healthy volunteers, using corneal optical coherence tomography (OCT). During sleep, closing the eyelids impairs the diffusion of oxygen through the corneal epithelium, resulting in the production of lactic acid, which can interfere with corneal thickness in the 1st min of wakefulness. OCT is an objective and noninvasive accurate method for evaluating corneal epithelium and storm thickness. A prospective and analytical study investigated the thickness and regularity of the cornea and its epithelium in the eyes (n = 30) of individuals without comorbidities. The volunteers underwent corneal OCT examination in two different periods: between 30 and 60 min after waking up and in the late afternoon of the same day. The study was carried out at Médicos de Olhos S.A, Curitiba, Brazil, between August and October 2023. For statistical analysis, the T-Student test was used, considering P < 0.0001 as statistically significant. Epithelium variability index, in OD (P = 0.965) and in OS (P = 0.516), did not present a statistically significant difference. The difference in thickness between epithelium minimum and maximum, OD (P = 0.880) and OS (P = 1.0), also did not show a statistically significant difference. It was possible to verify that the epithelial and stromal thickness of the cornea did not show significant diurnal variations. Furthermore, the regularity of epithelial thickness remained constant, as indicated by the epithelial variability index value.

The objective of this study was to evaluate the prevalence of premalignant and malignant lesions in patients clinically diagnosed with pterygium who underwent excision procedures performed by 1st-year residents in ophthalmology at a teaching hospital between 2018 and 2021. A cross-sectional analysis was conducted on 274 medical records of patients diagnosed with pterygium who underwent excision procedures from 2018 to 2021 at Hospital PUC-Campinas in Campinas, São Paulo, Brazil. Fifty-four medical records were excluded due to missing essential data. Among the 220 records included for analysis, 239 histopathological results were considered, as 19 patients underwent pterygium excision in both eyes. Out of 239 histopathological results analyzed, 236 (98.74%) were compatible with pterygium, 2 (0.8%) with epithelial dysplasia, and 1 (0.4%) with actinic keratosis. A histopathological study is essential for the accurate differential diagnosis of pterygium. Although the frequency of ocular surface squamous neoplasia is low, premalignant and malignant lesions must be ruled out. Correct diagnosis must be made to allow proper follow-up of the patients and prevent life-threatening lesions.

Drusen are yellow deposits between the basal lamina of the retinal pigment epithelium and the inner collagenous layer of Bruch’s membrane. Soft drusen, generally characterized by a medium to large size and poorly demarcated boundaries, increases the risk for advanced age-related macular degeneration (AMD). The prevalence of soft drusen in histopathological sections of the macular region, obtained from enucleated and eviscerated eyes, will be correlated in this study. This is a prospective cross-sectional study performed at the MUHC-McGill University Ocular Pathology and Translational Research Laboratory (McGill University, Montreal, Canada). A total of 158 eyes were evaluated between 2011 and 2019. Of these, 50% were enucleated and 50% eviscerated eyes. Cases were divided by age in decades into both groups, eviscerated and enucleated eyes. Inclusion criteria were patients aged 50 years or older, identification by histological criteria of macular area, histopathologically preserved macular area, presence of sufficient residual tissue for additional sections, and concrete patient data. The histopathological review was performed in digitized H and E slides (ZEISS Axio Scan.Z1) to assess the presence of soft drusen. The results in both groups, enucleated and eviscerated eyes, were similar, as well as the histopathological criteria used in the analysis of soft drusen. Prevalence of AMD in the referred population was comparable to that described in both studied groups.

Diabetic papillopathy (DP) is a rare ocular manifestation of diabetes mellitus. The exact etiology is not well understood, but in some cases, it may be associated with a rapid correction of the blood glucose level. There are limited case reports describing the use of intravitreal anti-vascular endothelial growth factor antibodies which have shown effectiveness in reducing disc swelling. In this case, we present the case of an 80-year-old diabetic woman diagnosed with DP during a diabetic retinopathy (DR) screening. Her visual acuity (VA) was counting fingers on her right eye and 20/160 on her left eye. At the fundoscopy, an exuberant bilateral hemorrhagic disc edema was observed, along with a nonproliferative DR. This edema was also evident in the fluorescein angiography and was documented through the optical coherence tomography. Brain computed tomography normal. She was treated with ranibizumab with significant improvement of the optic disc edema and the VA.

Primary large B-cell lymphoma of the retina, vitreous, and central nervous system (CNS), is an intraocular tumor with an unspecific and insidious clinical presentation. Pars plana vitrectomy (PPV) and optical coherence tomography (OCT) are useful diagnostic tools for this malignant process. The aim of this study is to evaluate the diagnostic efficacy of PPV for these intraocular lesions under a modified diagnostic protocol with a clinical pathological correlation with OCT imaging. A total of 115 samples were collected after a vitrectomy procedure (aspiration or vitrectomy cassette). The samples were centrifuged, and the precipitates were collected. A cell block was prepared and analyzed with multiple stains and an immunohistochemistry (IHC) panel, including B- and T-cell markers, as well as light chain markers, to establish the monoclonal nature of the tumor. Of the 115 samples, 9 (7.83%) were diagnosed with large B-cell lymphoma of the retina, vitreous, and CNS. Diagnostic vitrectomies for the large B-cell lymphoma of the retina, vitreous, and CNS is an excellent tool for the diagnosis of this entity. A negative diagnostic PPV with a strong suspicious OCT image, where the neoplastic cells are located between the retinal pigmented epithelium and Bruch’s membrane, the latter procedure should be either repeated or a chorioretinal biopsy be performed. In contrast, a positive vitrectomy using the IHC panel for large B-cell lymphoma of the retina, vitreous, and CNS is pathognomonic of this condition. In addition, the OCT is an important tool to help in the diagnosis of this difficult entity. The diagnostic PPV provided suitable vitreous samples to all patients with undetermined uveitis and/or intraocular tumor suspicion. A morphological and immunohistichemical (IHC) analysis enabled a conclusive diagnosis of retina, vitreous and CNS lymphoma in all patients submitted to the procedure.
