PARANEOPLASTIC PEMBIGUS: CLINICAL AND DIAGNOSTIC FEATURES AUTHORS 副肿瘤性庞贝氏症:临床和诊断特征 作者

Paraneoplastic pemphigus is an autoimmune dermatosis following against the background of neoplasia. It can occur before cancer is diagnosed, during or shortly after treatment, and even during remission of the underlying disease. Typically, paraneoplastic pemphigus only refers to the deposition of IgG. The article is devoted to paraneoplastic IgA/IgG pemphigus, which is one of the rare forms of paraneoplastic pemphigus in which deposition of IgA together with IgG is observed. We have described a clinical case of a patient who developed various rashes during the period of remission of chronic lymphocytic leukemia. Diagnosis of paraneoplastic IgA/IgG pemphigus based on clinical and anamnestic data, the results of a diagnostic skin biopsy, immunofluorescence test, consultation with a hematologist and lab tests. Difficulties in diagnosing paraneoplastic pemphigus are due to the polymorphism of rashes and the variety of clinical and laboratory parameters, which may depend on concomitant oncological pathology. A diagnostic search requires an assessment of the clinical picture of the disease, a carefully collected anamnesis, and a complete diagnostic algorithm for patients with bullous dermatoses

Clinical Signs and Laboratory Tests for the Differential Diagnosis ofAndrogenic and Post-COVID-19 Alopecia in Women 鉴别诊断女性雄激素性脱发和 COVID-19 后脱发的临床体征和实验室测试

Background: Pathological hair loss in women has emerged as a prevalent cause for seeking specialized dermatological and cosmetic services. The most common type of this condition is androgenetic alopecia, which arises due to hormonal and micronutrient imbalances.Furthermore, during the COVID-19 epidemic, there has been a notable increase in the number of female patients with pathological hair loss as a complication of the disease, with some individualsexperiencing alopecia the sole indication of asymptomatic COVID-19. Aims: The aim of the study wassearch for objective criteria for the differential diagnosis of androgenetic alopecia and post-COVID alopecia in women based on informative trichological and laboratory markers. Materials and methods: The including criteria for androgenetic alopecia (AGA) group were elevated dihydrotestosterone (DHT) levels, clinical indications of alopecia corresponding to initial stages of the condition, and a past experience of continuous observation untilFebruary 2020. For the post-COVID alopecia (COVID) group, inclusion criteria were a confirmed diagnosis of COVID-19 using RT-PCR and the presence of alopecia symptoms emerging within a year post-infection. Evaluation of quantitative characteristics of hairs was conducted was carried out based on trichogram and phototrichogram data, followed by image processing using a computer diagnostic program. Key indicators for hair growth were identified in patients' plasma, including DHT, vitamins B9 (folic acid), B12, D (25(OH)-D3 form), E, as well as calcium (Ca), iron (Fe), magnesium (Mg), selenium (Se), copper (Cu), and zinc (Zn). CART algorithm (Classification and Regression Trees) based on recursive partitioning of available data with selection of informative predictors and formation of a tree-like hierarchical structurewas applied to determine criteria to differentiate between patients with androgenetic and post-COVID alopecia. Results: Analysis of trichograms and phototrichograms revealed that unlike androgenetic alopecia, which primarily impacts hair follicles in the telogen and anagen phases in the androgen-dependent zone, post-COVID hair loss presents as a diffuse telogen effluvium pattern, involving the androgen-dependent (parietal) area of the scalp. Notably, patients with post-COVID alopecia exhibited elevated dihydrotestosterone levels compared to reference values, with no significant differencein comparison to AGA. While there were no variations in vitamin and certain trace element levels (Fe, Ca, Mg, Zn), individuals in COVID group have demonstrated a statistically significant reduction in copper content (46.4% lower than AGA; p=0.006) alongside an increase in selenium levels (24.7% higher than AGA; p=0.003). Conclusions: The performed study for the first time presents objective criteria for the differential diagnosis of androgenetic and post-COVID alopecia in women. The data obtained show a diffuse pattern of telogen effluvium after recovering from COVID-1

Clinical experience in managing patients with lymphogranuloma venereum. 管理淋巴肉芽肿患者的临床经验。

The article presents two clinical cases of lymphogranuloma venereum with classic manifestations of the disease. In the first case, the stages of lymphogranuloma venereum can be traced: the appearance of a primary affect (ulcer on the penis), the development after a few weeks of regional (inguinal) lymphadenitis and proctitis (anorectal syndrome). On the background of prescribed antibiotic therapy according to the regimen: doxycycline at a dose of 100 mg twice a day for 21 days, the clinical manifestations completely resolved. The second case is interesting in the development of the clinical picture of lymphogranuloma venereum from the second stage of the disease (inguinal syndrome) and the ineffectiveness of treatment with doxycycline, which subsequently required surgical intervention. In addition, in this case, Chlamydia trachomatis DNA was initially detected in a urethral scraping without evidence of urethral inflammation, which is rare for this disease. To treat a patient with “groin syndrome” as a clinical manifestation of lymphogranuloma venereum, a fluoroquinolone antibiotic, levofloxacin, was successfully used according to the regimen: 500 mg twice a day for 28 days.

Skin microbiome changes in patients with atopic dermatitis and psoriasis 特应性皮炎和银屑病患者皮肤微生物组的变化

Aims: The main aim of our study was identification of skin microbiome changes in patients with atopic dermatitis and psoriasis. The focus of our research were microorganisms of Staphylococcus spp. genus. Materials and methods: The study included 34 patients with atopic dermatitis and 41 patients with psoriasis. The enrollment period lasted for 3 months. The material for cultivation was taken from 4 skin areas: lesion and normal skin of dry body skin sites (medial surface of forearm for patients with atopic dermatitis and extensor surface of elbow joint for patients with psoriasis) and scalp. Further evaluation was carried out using cultural and molecular biological research methods. The obtained data were statistically processed. Results: The study included 34 patients with atopic dermatitis and 41 patients with psoriasis. Mean SCORAD for atopic dermatitis group was 51,5912,75 and mean PASI for psoriasis group was23,7214,14. Mean SCORAD for atopic dermatitis patients who carries toxigenic S.аureus strains was 57,439,75, in group without toxigenic S.аureus strains mean SCORAD was statistically significant lower - 37,908,63 (р=0,054). Mean SCORAD for atopic dermatitis patients who carries S.epidermidis commensal strain was 51,312,75, difference between this group and toxigenic S.аureus strains group of patients was insignificant (р=0,18). Mean PASI for psoriasis patients who carries toxigenic S.аureus strains was 26,412,76, mean PASI for psoriasis patients who carries S.epidermidis commensal strain was 20,99,07. Difference between S.epidermidis group and toxigenic S.аureus strains group of patients was statistically insignificant (р=0,53) with tendency to more severe disease in group of toxigenic S.аureus strains carriers. Conclusions: This study reveals some aspects of skin colonization by microorganisms belonging to Staphylococcus spp genus in patients with atopic dermatitis and psoriasis. We identify some correlations between severity of inflammatory disease and characteristics of microorganisms strain.

Hidradenitis suppurativa is a common disease that significantly reduces the quality of life of patients of reproductive age; its exact prevalence is unknown in most countries due to late and poor diagnosis. In order to clarify the prevalence of HS, an analysis of the structure of newly identified cases of this disease was carried out in the State Scientific Center for Disease Control and Prevention of the Ministry of Health of the Russian Federation for the period 2008-2021. inclusive. In order to study the characteristics of the course of HS, its impact on behavior and receipt of medical care from the patient’s point of view, an anonymous quantitative online survey of patients was conducted. Results: During the specified period, 765 cases (0.36% of the total number of all newly diagnosed cases) were diagnosed in the section “L73 Other diseases of the hair follicles” in the State Scientific Center for Disease Control and Prevention. The diagnosis of GG (ICD code L73.2) was first established in only 59 cases (about 8% of identified cases using code L73). 1035 respondents took part in the survey. The survey revealed that patients with HS are worried about: 66% - swelling, 55% - pain, 38% - purulent discharge and/or blood, 25% of patients noted itching that bothers them. The majority of patients rated the severity of pain as moderate (29%) and slight pain (29%). 44% of patients continue to search for a primary care physician; 18% of patients are observed by one doctor (dermatologist and/or surgeon); 9% of patients visited 2-3 doctors before receiving the necessary help. Conclusion: All over the world, the “search” for a specialist who will correctly diagnose, prescribe adequate treatment and accompany its long process is still a complex and unsolved task. In order to improve the quality of medical care for patients with HH, it is necessary to develop and implement domestic guidelines, as well as the development and implementation of educational programs for interested specialists.

FEATURES OF PSORIASIS TREATMENT WITH COVID-19 INFECTION 用Covid-19感染治疗牛皮癣的特点

Background: Today, one of the difficult tasks in modern dermatology is the management of patients with chronic dermatoses, including psoriasis, due to a new coronavirus infection. The general condition of the patients, as well as concomitant therapy for the underlying disease, may subsequently produce severe side effects and complicate the treatment of concomitant skin pathology in patients in this category. Aims: The purpose of our study was to optimize the treatment of patients with psoriasis against the background of COVID-19 infection. Materials and methods: Our study included 34 patients with psoriasis who were hospitalized at the Zangiota Clinical Hospital, specializing in patients with COVID-19 infection from 2020 to 2022. Also, all patients were divided according to the severity of the underlying disease according to the “Temporary recommendations for the management of patients infected with COVID-19”, fifth version of 2020. The severity of the psoriatic process was assessed using the PASI index. Results: If patients have a psoriatic process, it is more rational to use biological drugs for the treatment of COVID-19 infection due to the absence of exacerbation of the psoriatic process in dynamics. Conclusion: Treatment of patients with COVID-19 infection with casivirimab + imdevimab showed a positive effect on the dynamics of skin elements and the absence of relapses during three months of observation. While standard therapy for coronavirus infection gives a good regression of skin elements on the 10th day of therapy, after a month there is a progression of the skin process, the appearance of itching and new elements of psoriasis.

Abstract:background - comparative assessment of the effect of different options for spa treatment: balneotherapy and balneotherapy in combination with intravenous laser blood irradiation in patients with psoriasis vulgaris on the level of IL-6 and IL-17, dermatological and psycho-emotional status, assessment of quality of life. Materials and methods. The naturalistic comparative study involved 108 patients with ordinary psoriasis, stationary stage (men – 57.4%, women – 42.6%; the average age was 36.2 [24.5;47.9] years) who underwent spa treatment, which were subsequently divided into 2 groups: 52 patients who received balneotherapy along with the traditional complex of SCL were included in the 1st group, 56 patients who received complex treatment were included in the 2nd group. balneotherapy in combination with VLOK along with traditional ... The effectiveness of SCL was evaluated using the PASI index, the HARS and HDRS scale, the SF-36 questionnaire. The dynamics of IL-6 and IL-10 levels in blood plasma were evaluated. The total duration of the study was 6 months and 48 weeks: the treatment phase was 14 days and the follow–up phase was 3 and 6 months after the completion of SCL. Results. After 14 days of SCL in the group in which balneotherapy was performed in combination with VLOK, a statistically significant decrease in the PASI index was more pronounced compared to the group in which balneotherapy was performed (p). Balneotherapy did not have a pronounced effect on psychoemotional status. On the contrary, balneotherapy in combination with VLOK contributed to a statistically significant decrease in the final indicators on the HARS and HDRS scales. The decrease in IL-6 and IL-17 levels was statistically significant in both groups and there were no differences between them. The complex use of balneotherapy and VLOK in comparison with the group of balneotherapy was accompanied by a more pronounced improvement in QOL. The combined use of balneotherapy and VLOK in the follow-up phase showed a long-term positive effect: 6 months after the completion of SCL, the number of patients who had clinical remission was statistically significantly higher compared to the group in which balneotherapy was performed (87.4% vs. 44.7%). Conclusions:The advantage of the combined use of balneotherapy and ILBI compared with balneotherapy in patients with psoriasis vulgaris on CL was shown. The complex use of balneotherapy and ILBI of significant indicators of inflammatory biomarkers, improvement of the dermatological and psycho-emotional state, quality of life parameters and good overpopulation. More pronounced effectiveness of the complex use of balneotherapy and ILBI compared to balneotherapy of the quality, diversity of pathophysiological mechanisms of psoriasis and the presence of several targets for pathogenetic therapy.

Grover's disease: modern ideas about transient and persistent acantholytic dermatosis. 格罗弗病:关于一过性和持续性棘层溶解性皮肤病的现代观点。

Grover's disease is a papulo-vesicular pruritic dermatosis characterized by a transient, persistent, or asymptomatic course. We have analyzed the publications found in the PubMed and RSCI scientific literature databases between 1970 and 2022. The clinical manifestations of Grover's disease, possible localization, etiological factors, and the histological and dermatoscopic picture of the disease are described. According to the literature, the putative associations with disease onset are highly variable and include insolation, pregnancy, hemodialysis, chemotherapy, sweating, a number of dermatoses, and a history of malignancy and organ transplantation. Seasonality of the process has also been described. To date, 9 histologic patterns of the disease have been described, the most common being acantholytic dyskeratosis. In this review, we have included our observations of patients with Grover's disease.

Clinical experience of outpatient treatment of bacterial skin infections in children 门诊治疗儿童皮肤细菌感染的临床经验

Data regarding the role of cutaneous microbiome in physiological and pathological conditions, pharmacological properties of fusidic acid and its clinical use for the treatment of skin infections are presented in the article. Incidence of skin infections in children treated in children's city polyclinic №1 of Izhevsk city through 2019-2022 are analyzed. Outcomes of the treatment of these patients with topical fusidic acid preparations are presented. Due to high activity against typical cutaneous bacterial pathogens including MRSA strains of S. aureus, favorable safety profile and pharmacokinetic profile when topically applied along with low resistance of the key pathogens fusidic acid preparations are the first line treatment of primary and secondary pyodermas.

Incidence of skin infections in children treated in children's city polyclinic №1 of Izhevsk city through 2019-2022 are analyzed. Outcomes of the treatment of these patients with topical fusidic acid preparations are presented. Valproic acid-induced Stevens-Johnson syndrome 丙戊酸诱发史蒂文斯-约翰逊综合征

A 27-year-old transgender female at the stage of gender transition with bipolar disorder for which she received valproic acid developed multiple petechial rashes on the chest, upper and lower extremities, local erythematous lesions with forming blisters, large-platelet peeling on the face, neck, chest and inguinal area, erosion on the oral mucosa. She was diagnosed with drug eruption, Stevens-Johnson syndrome. The diagnosis was verified on the basis of complaints, medical history, clinical picture, laboratory findings. Treatment included dexamethasone, infusion therapy, vancomycin, cyclosporine A, valproic acid withdrawal. Against the background of the therapy, skin condition was improved: there was the epithelization of ulcerative defects and the absence of new elements.
一名 27 岁的变性女性,正处于性别转换阶段,患有双相情感障碍,曾服用丙戊酸治疗,胸部、上肢和下肢出现多处瘀点状皮疹,局部红斑伴水疱形成,面部、颈部、胸部和腹股沟区出现大面积血小板脱落,口腔黏膜糜烂。她被诊断为药物性糜烂、史蒂文斯-约翰逊综合征。诊断是根据主诉、病史、临床表现和实验室检查结果核实的。治疗包括地塞米松、输液治疗、万古霉素、环孢素 A、丙戊酸停药。在治疗的背景下,皮肤状况有所改善:溃疡性缺损上皮化,没有新的元素。
