Materials and Method: The retrospective study reviewed 1981 patients aged 65–104 years who were bedridden and followed at University of Health Sciences, Antalya Training and Research Hospital Family Medicine Clinic from 2016 to 2018. Due to the effects on studied test parameters, patients with anemia were excluded from the study, and 898 patients included. The baseline variables red cell distribution width, mean platelet volume, platelet-to-lymphocyte ratio, and neutrophil-tolymphocyte ratio were compared between survivors and non-survivors.
材料与方法:回顾性研究2016 - 2018年在安塔利亚培训与研究医院健康科学大学家庭医学门诊随访的65 ~ 104岁卧床患者1981例。由于对研究试验参数的影响,贫血患者被排除在研究之外,共纳入898例患者。在幸存者和非幸存者之间比较基线变量红细胞分布宽度、平均血小板体积、血小板与淋巴细胞比率和中性粒细胞与淋巴细胞比率。

Materials and Method: 61 stable patients (ages of 65-84) were included. Demographic data, body mass index, comorbidities, annual number of exacerbations were recorded. Pulmonary function test, six-minute walk test and Tilburg Frailty Test were performed in all cases. Patients were classified into groups A,B,C&D according to GOLD chronic obstructive lung disease guideline by annual exacerbation rate and dyspnea score. Neutrophil lymphocyte ratio, high sensitive C-reactive protein, IL-6, 8OH-dG, IL-18 levels were studied in serum.
材料与方法:纳入61例病情稳定的患者(年龄65 ~ 84岁)。记录人口统计数据、体重指数、合并症、年加重次数。所有病例均行肺功能试验、6分钟步行试验和Tilburg虚弱试验。根据GOLD慢性阻塞性肺疾病指南按年加重率和呼吸困难评分将患者分为A、B、C&D组。检测血清中性粒细胞比例、高敏c反应蛋白、IL-6、8OH-dG、IL-18水平。

THE EFFECT OF SURGICAL TREATMENT ON SURVIVAL IN GERIATRIC PATIENTS WITH STAGE I SMALL-CELL LUNG CANCER 手术治疗对老年I期小细胞肺癌患者生存的影响

Results: A total of 1248 patients were included in the study. 28.9% of the patients did not receive any treatments, 47.2% of the patients received non-surgical treatment and 23.9% of the patients received surgical treatment. The 5-year overall survival rates of the untreated group, non-surgical group and surgical group were 7%, 10% and 32%, respectively (p <0.0001). According to the results of multivariable analysis, surgical treatment was an important factor in increasing overall survival when compared to the non-surgical treatment.

Humanity worldwide experienced an unprecedented, tremendous change since the end of 2019 and some scholars use the term “new normal” for our current state A coronavirus disease outbreak occurred in December 2019, which was originated from Wuhan city of Hubei province in the People’s Republic of China and erupted into an international public health emergency in a month It is clear that the elderly population is at a significantly higher risk of the severe outcome of COVID-19 and has the greatest risk of mortality Since the early stages of the pandemic several measures adopted including social distancing, promoting staying home, cancellation of mass gatherings, and school closures, larger containment processes (such as entire towns or cities) to control the spread of the disease, and mitigate the negative consequences At the same time, these measures have significant effects on the health of the elderly An action in solidarity is needed to prevent the further community spread of the virus, protecting older people living alone in the community, as well as supporting all health and social care workers In these difficult times, the elder people should not neglect or underestimate their own health problems and cling to life They should keep in mind that;they are very valuable to their loved ones Additionally, during the post-pandemic period, permanent problems may arise in adults in the older age group, which differ greatly from adults It is necessary to be ready to deal with this, which depends on solving foreseeable problems © 2020, Geriatrics Society All rights reserved

PATHOLOGICAL MANDIBULAR ANGLE FRACTURE: SPONTANEOUS HEALING IN A CASE OF OSTEOMYELITIS AFTER THIRD MOLAR EXTRACTION 病理性下颌角骨折:第三磨牙拔除后骨髓炎自行愈合1例

Late pathological fractures of the mandible associated with the extraction of an impacted third molar and osteomyelitis of the jaw feature rarely in the scientific literature. Indeed, most information on the subject is to be found in the form of case reports. Fractures of this kind may occur in regions where the bone has weakened because of an underlying pathological process, which has developed slowly. Predisposing factors of this complication are believed to be multifactorial but, in the older age group, reduced bone elasticity, bone atrophy, risk of osteoporosis, and potential of tooth ankylosis can weaken the mandible, possibly increasing the rate of fractures. In this study, we report a case of chronic osteomyelitis with a mandibular angle fracture in an elderly male as a rare complication of an odontogenic cystic lesion associated with the extraction of a mandibular third molar. Spontaneous healing occurred with the use of an antibiotic therapy until the second surgical intervention. Attention is focused on the possible risk indicators and preventive measures.

Materials and Method: After obtaining local ethical committee approval, the perioperative data of geriatric patients who had total thyroidectomy between January 2016 and December 2018 were evaluated. Patients’ age, sex, body mass index, American Society of Anesthesiologists score, preoperative systemic disease or medical comorbidities such as thyroid hormone dysfunction and electrolyte imbalance, difficult airway, operation time, preoperative vocal cord paralysis and thyroid malignancy were evaluated. Patients with respiratory or cardiovascular complications 30 days after surgery were evaluated about the effect of demographic data by using one –way ANOVA test.

Immunoglobulin G4-related sclerosing ureteral disease is a rare benign disorder characterised by fibrosis and lymphoplasmacytic infiltration in the ureter. A 70-year-old man with a single functional kidney and left flank pain was diagnosed with IgG4-related ureteral disease that presented as a unilateral ureteral mass. Left hydronephrosis and a 25 × 23 × 26 mm left midureteral mass were found. No malignancy was found on ureteroscopy and urinary cytology did not reveal any neoplastic cells. A 2 cm midureteral stenosis was found in the left ureter on retrograde pyelography. It was not a ureteral stricture but was the result of periureteral inflammation and fibrosis caused by immunoglobulin G4-related sclerosing disease. Initial endoscopic ablation-obliteration therapy was unsuccessful, and after 6 weeks the patient was treated by robotic ureteroureterostomy. Most plasma cells in the excised ureteral segment were IgG4-positive. Serum IgG4 was 273 mg/dL (normal range: 85–120 mg/dL). The histology of the ureteral segment resembled retroperitoneal fibrosis and the histopathology of the stricture included IgG4-positive cells, fibrosis and ureteritis. The patient was treated with oral azathioprine for 6 months. No evidence of recurrence was seen on ureteroscopy or abdominopelvic computed tomography at the 3-month or 1-year follow-up.

PARANASAL SINUS ANATOMICAL DIFFERENCES IN ELDERLY PATIENTS 老年患者鼻窦解剖差异

Introduction: Endonasal endoscopic sinus surgeries performed on elderly patients can be challenging due to anatomical variations, and can be studied using preoperative computed tomography. The aim of the present study was to evaluate paranasal sinus anatomical differences in elderly patients compared to a younger control group. Materials and Methods: We retrospectively evaluated paranasal computed tomography scans of 47 elderly patients (>65 years old) (Elderly group) and 47 younger patients (Control group) for midfacial skeletal size (interzygomatic buttress distance, nasion-basion distance), anatomical variations, dimensions, and paranasal sinus volumes. Results: The mean age of the Elderly group was 69.89 years (65-81 years) and the mean age of the Control group was 33.15 years (20-49 years). There was no significant difference in midfacial size between the two groups. The prevalence of Keros Type III olfactory fossa was significantly higher in the Elderly group than in the Control group (p<0.05). The Elderly group had a significantly lower mean maxillary sinus volume (p<0.01) and mean anteroposterior diameter of the sphenoid sinus (p<0.01) compared to the Control group. Furthermore, there was no significant difference in the maxillary sinus volume between the elderly edentulous and dentulous patients(p>0.05). Conclusion: Elderly patients have more Keros Type III olfactory fossa, which confers a higher risk of iatrogenic cerebrospinal fluid leakage during endoscopic sinus surgery. The preoperative detailed evaluation of computed tomography scans of elderly patients should include, but not be limited to, the ethmoid roof for deep olfactory fossa, and the sphenoid sinus for its narrow anterioposterior dimention.
