Congenital heart disease is now the most common reason for women to attend a high-risk joint cardiac–obstetric clinic. With advances in medical care and surgical techniques, most children born with congenital heart disease reach childbearing age and have a good prognosis for long-term survival with a good quality of life. Pregnancy remains a major life event for these women, posing additional risks with the potential for severe morbidity and mortality. The hemodynamic changes induced by pregnancy can unmask undiagnosed congenital heart disease or exacerbate pre-existing conditions. Therefore, specialist care is essential for a successful pregnancy outcome. Women with complex congenital heart disease embarking on pregnancy need the support of a multidisciplinary team, including cardiologists, obstetricians, anaesthetists, midwives and adult congenital heart disease clinical nurse specialists. Clinical nurse specialists in particular play a crucial role in coordinating, supporting, educating and advocating for the mother and fetus through preconception, pregnancy, the puerperium and beyond.

Evidence-based standards are an important means for ensuring safe and effective care for birthing women. The provision of one-to-one care for women in labour is one such standard, which should be the norm in Dutch maternity care. However, no audit measures of this standard are available. This study examined the use of Birthrate Plus, a validated instrument for the measurement of patient acuity. This tool has the added benefit of allowing measurement of birth characteristics in relation to staffing numbers, providing a basis for auditing the standard of one-to-one care for women in labour in hospitals in the Netherlands. This pilot study used the Birthrate Plus tool to retrospectively examine birth characteristics and staffing levels in a 4-month period, in five hospitals in the Netherlands. The review of 11 582 patient cases found that most births in the sample were classified as occurring in the higher acuity levels of Birthrate+ tool. Examination of staffing levels showed that when comparing actual staffing levels with recommended levels, hospitals had a shortfall of between 47% and 64%. The Birthrate Plus tool could be useful in auditing staffing levels in Dutch maternity care. Analysis of the data provided by hospitals in the sample showed that none were able to meet the Dutch standard of providing consistent one-one-one care.

Mental health can influence the likelihood that a woman will initiate and continue breastfeeding behaviour that is associated with improved infant and maternal health. Mental health outcomes are associated with variables such as optimism and coping. The aim of this study was to explore the relationship between coping, optimism and mental health, and their impact on breastfeeding duration. A total of 612 women who had breastfed completed an online questionnaire comprising the hospital anxiety and depression scale, cognitive emotional regulation questionnaire, brief coping orientation to problems experienced inventory and life orientation test. Data were analysed using correlations, ordinal linear regression and mediation analysis. There was a direct relationship between breastfeeding duration and depression but no direct relationship between duration and anxiety. A mediation analysis showed no mediating between breastfeeding duration and depression. Optimism did mediate the relationship between anxiety and duration, with optimism being associated with reduced anxiety and increased duration. The findings highlight the importance of support for mothers while breastfeeding to improve mental health outcomes and foster coping strategies, such as optimism.

Anaemia is a multifactorial condition and understanding its prevalence and associated factors is crucial for effective healthcare planning and interventions. The aim of this study was to explore the prevalence of anaemia during pregnancy and its potential risk factors in Oman. A retrospective approach was used to identify anaemia cases by examining medical records taken from 9321 women who gave birth at the hospital over 3 years. Chi-squared tests were used to establish differences in risk factors using a case-control design, with a 1:2 ratio. The crude prevalence of anaemia was 31.2%, dropping to 4.9% after excluding women with chronic and/or acute diseases. Significant differences were found in nationality (χ2=4.738, P=0.030), body mass index (χ2=6.392, P=0.041) and gestational age (χ2=4.329, P=0.037) for women with anaemia compared to those without. These findings underscore the significance of assessing and managing anaemia in pregnancy, while considering the interaction of demographic and clinical variables in formulating anaemia prevention strategies.

Working mothers may find it difficult to maintain exclusive breastfeeding; this challenge can be addressed through expressing breast milk. However, barriers to expressing breast milk include perceived insufficient milk supply and a lack of knowledge on pumping, storing and giving breast milk to babies. This study's aim was to identify the effectiveness of expressed breast milk assistance, through the ASIPku mobile application, among working mothers in North Sulawesi, Indonesia. This quasi-experimental study involved 100 participants (intervention: n=50, control: n=50), selected using cluster randomised trials. Questionnaires were used to assess exclusive breastfeeding and use of the app in the intervention group. Chi-squared tests were used for data analysis. Assistance using mobile applications had a significant effect on exclusive breastfeeding among working mothers in North Sulawesi (P<0.001). Digital mentoring was effective in improving the rate of exclusive breastfeeding. By providing information on expressing breast milk, these apps can be used to support women and make exclusive breastfeeding more accessible to working mothers.

Eggs are a natural whole food providing an important array of nutrients that can be challenging to find in other everyday foods. Although evidence shows the benefits of egg consumption for mother and child, consumption rates remain low. The aim of this article was to review latest literature and guidance on egg consumption and examine barriers to consumption during the critical first 1001 days of life. This narrative review included an electronic search of PubMed, Google Scholar and Clinical Trials.Gov. Human studies published in English between 2019 and 2024 were screened for eligibility. Overall, 14 key studies were included that were specific to egg intake/consumption during pregnancy, breastfeeding and/or infancy/early childhood. Six sets of relevant dietary guidelines were identified and reviewed. Current egg consumption is low, despite previous concerns about food safety and allergy having been overturned in official advice. Including eggs in the diet is an easy and cost-effective way to improve diversity and digestibility of nutrients when dietary requirements are higher. Their consumption has the potential to bridge nutrient gaps, help prevent allergy, augment breast milk composition and contribute to child development and growth.

Exclusive breastfeeding duration remains suboptimal among Chinese mothers. Understanding the factors that influence exclusive breastfeeding is essential for developing targeted interventions to promote breastfeeding practices. This study aimed to identify determinants of exclusive breastfeeding in the first 6 months after birth in Xuzhou City, China. This prospective study enrolled 178 mothers, using structured questionnaires for data collection. Subsequent analysis used multiple linear regression to explore factors associated with exclusive breastfeeding duration. On average, participants exclusively breastfed for 67 days. Influencing factors included breastfeeding intention, perception of insufficient milk and maternity leave. Targeted interventions have the potential to prolong exclusive breastfeeding duration, such as ensuring women develop accurate perceptions of their breast milk supply and extending maternity leave. Understanding the factors that influence exclusive breastfeeding is essential for developing targeted interventions to promote breastfeeding practices. This study aimed to identify determinants of exclusive breastfeeding in the first 6 months after birth in Xuzhou City, China. This prospective study enrolled 178 mothers, using structured questionnaires for data collection. Subsequent analysis used multiple linear regression to explore factors associated with exclusive breastfeeding duration. On average, participants exclusively breastfed for 67 days. Influencing factors included breastfeeding intention, perception of insufficient milk and maternity leave. Targeted interventions have the potential to prolong exclusive breastfeeding duration, such as ensuring women develop accurate perceptions of their breast milk supply and extending maternity leave.
