Lung Ultrasound using a Handheld Device to Diagnose COVID-19 in the Emergency Department 在急诊科使用手持设备进行肺部超声诊断COVID-19

The Coronavirus Disease 2019 (COVID-19) pandemic has critically struck healthcare systems and burdened emergency services. To date, there is no accurate and rapid point-of-care diagnostic test. This study aimed to investigate Lung Ultrasound (LUS) against Reverse Transcription-Polymerase Chain Reaction (RT-PCR) test in suspected COVID-19 patients presenting to the emergency department. In 20 eligible patients (mean age ± standard deviation, 49 ± 15 years), 12 had a positive RT-PCR test and undergone an LUS examination over 12 lung zones using a handheld ultrasound device. Each zone was semiquantitatively scored according to the Lung Ultrasound Scoring System (LUSS) from 0 to 3 based on the severity of findings (pleural line irregularity, B-lines, consolidations) and documented the presence of light beam artifacts. A second blinded reader scored the images to investigate interreader reproducibility. The LUSS score had a modest diagnostic performance at 66.6% [95% Confidence Interval (CI), 34.9–90.0%] sensitivity and 75.0% (95% CI, 34.9–96.8%) specificity. The light beam artifact was more prevalent and sensitive to COVID-19 patients with 81.8% (95% CI, 48.2–97.7%) sensitivity and 75.0% (95% CI, 34.9–96.8%) specificity. LUS had an almost perfect interreader reproducibility for LUSS (Kendall’s W = 0.961; 95% CI, 0.894–0.985) and light beam artifact (Cohen’s κ = 0.890; 95% CI, 0.683–1.00). Overall, LUS using handheld devices can offer a safe, reproducible, rapid, and feasible first-line tool for detecting COVID-19 patients in emergency departments. The light beam artifact was more sensitive and specific to COVID-19 patients and can be useful for effectively triaging suspected cases.

Drug Repurposing of Lactoferrin Combination in a Nanodrug Delivery System to Combat Severe Acute Respiratory Syndrome Coronavirus-2 Infection 纳米药物传递系统中乳铁蛋白组合的药物再利用以对抗严重急性呼吸综合征冠状病毒-2感染

A drug repurposing approach for Food and Drug Administration (FDA)-approved drugs and preclinical entity by using cheminformatics and chemical transformational method, with the objective of discovering safer novel potent inhibitors that are selective for COVID-19, is reported. We examined the action of lactoferrin against Severe Acute Respiratory Syndrome Coronavirus-2 (SARS-CoV-2). Lactoferrin was tested not only as therapy but also as a nanocarrier. Among the different repurposed drugs approved to help control the COVID-19 pandemic, we focused on nitazoxanide in a nanodrug delivery form. Lactoferrin activity improved after it was used in combination with nitazoxanide [half-maximal inhibitory concentration (IC 50 ) = 2.72, 1.34 with Selectivity Index (SI) = 25 and 32, respectively]. These results will help us enhance the activity of lactoferrin as a nanocarrier for a variety of selected drugs. In addition, the antiviral activity of the spirooxindole scaffold showed interesting results against SARS-CoV-2 as well as Middle East respiratory syndrome (MERS)-CoV with an IC 50 of 0.03 and 0.001 mM, respectively. The molecular modeling study revealed that nitazoxanide has a high similarity to arbidol. Docking outputs emphasize the importance of anilide functionality in drug activity because of its ability to form essential HB main protease and spike protein active sites. By understanding the binding mode, these data will help us design new bioactive drug candidates against SARS-CoV-2.

Hospital-acquired Respiratory-Tract Infections in the Teaching Hospitals of Sfax 斯法克斯教学医院院内获得性呼吸道感染调查

Hospital-acquired Respiratory-Tract Infections (HARTIs) are identified as the most frequent type of hospital-acquired infections. They can engender significant morbidity and mortality rates, generating a heavy economic burden, especially in the limited resources countries. In this perspective, this study aimed to determine the prevalence of HARTIs in the University Hospitals (UHs) of Southern Tunisia and to identify their main associated factors. It was a cross-sectional study conducted in the two UHs of Sfax governorate, Tunisia, from July 10 to 24th, 2017, including all hospitalized patients for at least 48 hours. It was a 1-day pass per department and a 1-week survey per UH. In total, 34 cases of HARTIs were notified among 752 surveyed patients, accounting for an overall prevalence of 4.5%. The prevalence of HARTIs in the Intensive Care Units (ICU) was 20.6%. Multivariate logistic regression analysis showed that developing a HARTI in non-ICU was independently associated with tobacco use [Adjusted Odds Ratio (AOR) = 2.83; 95% Confidence Interval (95% CI) = [1.10–7.27]; p = 0.03], central vascular catheter (AOR = 5.70; 95% CI = [1.29–25.15]; p = 0.022) and McCabe Index ≥1 (AOR = 7.38; 95% CI = [2.73–19.97]; p < 0.001). In ICU, only endotracheal tube was independently associated with HARTIs (AOR = 42.5; 95% CI = [4.97–64.13]; p = 0.001). This study illustrated the extent of HARTIs problem threatening the quality of care in Southern Tunisia. Identifying the risk factors of HARTIs in both ICUs and non-ICUs may help healthcare workers to ascertain the avoidability of these infections.

They can engender significant morbidity and mortality rates, generating a heavy economic burden, especially in the limited resources countries. In this perspective, this study aimed to determine the prevalence of HARTIs in the University Hospitals (UHs) of Southern Tunisia and to identify their main associated factors. It was a cross-sectional study conducted in the two UHs of Sfax governorate, Tunisia, from July 10 to 24th, 2017, including all hospitalized patients for at least 48 hours. It was a 1-day pass per department and a 1-week survey per UH. In total, 34 cases of HARTIs were notified among 752 surveyed patients, accounting for an overall prevalence of 4.5%. The prevalence of HARTIs in the Intensive Care Units (ICU) was 20.6%. Multivariate logistic regression analysis showed that developing a HARTI in non-ICU was independently associated with tobacco use [Adjusted Odds Ratio (AOR) = 2.83; 95% Confidence Interval (95% CI) = [1.10–7.27]; p = 0.03], central vascular catheter (AOR = 5.70; 95% CI = [1.29–25.15]; p = 0.022) and McCabe Index ≥1 (AOR = 7.38; 95% CI = [2.73–19.97]; p < 0.001). In ICU, only endotracheal tube was independently associated with HARTIs (AOR = 42.5; 95% CI = [4.97–64.13]; p = 0.001). This study illustrated the extent of HARTIs problem threatening the quality of care in Southern Tunisia.
