Three dimensional (3D) models mimic the features of native tissue environment. Thus, morphology and signalling of cells from 3D culture are often more physiological than routine two dimensional (2D) cell culture. It is also known that the cell-secreted products have paracrine effect on other cells growth. In this experimental study, we optimised the nasal fibroblast culture on a 3D cell culture system and studied the effects of secretome from the 3D culture (3DCM) onto fibroblast growth and cytoprotection. Nasal fibroblast was isolated from human nasal turbinates. The suitable microcarrier was selected by culturing the fibroblasts in passage 3 on various types PolyGEM™ polystyrene microcarriers. Then, the cells were cultured on selected microcarrier using a 3D culture system and the conditioned medium (CM) was collected. 3DCM were supplemented to fibroblasts to study for attachment, proliferation, and cytoprotective effects against cytotoxicity of Centella asiatica. Bicinchonic Acid Assay (BCA) was performed to quantify protein amount in CMs. Sodium dodecyl sulphate-polyacrylamide gel electrophoresis (SDS-PAGE) was performed for preliminary profiling and comparison of 2DCM and 3DCM protein profile. Our study showed that the 3DCM did not significantly enhance cell attachment and proliferation. The secretome of both 2DCM and 3DCM found to have significant cytoprotective effect onto nasal fibroblast against cytotoxicity of C.asiatica extract. 3DCM had higher protein concentration than 2DCM. SDS-PAGE showed three exclusive proteins in 3DCM and four exclusive proteins in 2DCM. Future study should be conducted on utility of nasal fibroblast secretome on cytoprotection against harmful agents in environment and cytotoxicity of natural products.

Human Immunodeficiency Virus (HIV) epidemic remains a significant burden in Malaysia. Stigma related to HIV and its effect on the quality of life (QOL) of persons living with HIV (PLHIV) remains under-reported. The aim of the present study was to assess self-perceived stigma amongst PLHIV attending an urban community clinic and its influence on their QOL. Data was collected using HIV Stigma Scale and WHO-QOL HIV BREF Scale. The overall stigma experienced by PLHIV in this community was higher than previous studies (mean ± SD; 103.37 ±18.14). Majority participants had fear disclosing their disease status, while personalized stigma or the experience of prejudice and rejection was the least experienced. The overall QOL was low and was significantly impaired in social relationship domain (mean ± SD; 12.72 ± 3.59). However, their ability to perform daily activities was not affected by the illness (mean ± SD; 14.48 ± 2.91). PLHIV with higher spiritual values demonstrate lower perception of negative self-image and inferiority (r= -0.54). This finding was unique to PLHIV in this study and suggested the importance of spirituality and personal beliefs on their self-esteem. In conclusion, stigma remains as a significant problem among PLHIV in this community. Primary care offers the best platform to promote a holistic management of PLHIV, where the integration between counselors, religious experts, family and non-governmental associations could come together. The management of PLHIV is unique in every community, hence individualized approach based on cultural norms and beliefs could assist in the overall management of PLHIV.

Acanthamoeba spp. is a free-living amoeba commonly found in the environment. It is the causative agent of Acanthamoeba keratitis (AK) and granulomatous amoebic encephalitis (GAE). This amoeba is also a host to various bacteria including pathogenic ones such as Mycobacterium, Legionella, Pseudomonas and Methicillin-resistant Staphylococcus aureus (MRSA). In light of this information, a study was undertaken to detect these bacterial endosymbionts in Acanthamoeba spp. isolated from air-conditioning outlets in wards and operating theatres in Universiti Kebangsaan Malaysia Medical Centre (UKMMC). The presence of these bacteria was screened using primer pair targeting each genus and further confirmed by sequencing analysis. Twenty-nine (80.56 %) Acanthamoeba isolates were found to contain targeted bacterial endosymbiont with at least one genus of bacteria per isolates. Mycobacterium spp. (82.76 %) were the most common bacteria detected, followed by Legionella spp. (65.52 %) and Pseudomonas spp. (62.07 %). No MRSA were detected in any isolates used in this study. Most of the Mycobacterium endosymbionts were non-tuberculous mycobacteria, while only two were part of Mycobacterium tuberculosis complex group. We conclude that most Acanthamoeba have the potential to host various pathogenic bacteria. However, the implication on the pathogenicity of both organisms remains unclear and further investigations are needed.

ABSTRACT Tissue engineering has emerged as a new promising field that allow in vitro construction of whole transplantable tissue. Recently, bone marrow stem cells (BMSCs) fulfil the requirements as appropriate cell source that is renewable for cartilage tissue engineering since they were able to form hyaline-like cartilage in vitro and in vivo when cultured in media supplemented with specific growth factors. This study aimed to examine the potential of BMSCs chondrogenesis in vitro and in vivo. BMSCs were isolated from sheep and divided into a test group and control group. Control group was cultured in medium supplemented with 10% fetal bovine serum (FBS). Test group was cultured in medium supplemented with 1% FBS, 1% ITS, 5ng/mL TGF-s3, 50ng/mL IGF-1, 40 ng/mL L-proline, 100 nM dexamethasone and 50 μg/mL ascorbic acid-2 phosphate. The experiment was carried out on athymic nude mice. Cell aggregates were formed in test group and indicated the early chondrogenesis in cell culture. Later, cells-fibrin constructs were made and implanted subcutaneously into nude mice for 5 wks, then explanted for histological examination and glycosaminoglycans (GAG) quantification. Test group constructs showed higher cartilage matrix synthesis as confirmed by Safranin O staining and GAG production. These results demonstrated the effectiveness of low serum media supplemented with TGF-s3 in promoting chondrogenesis in BMSCs. Biceps brachii (BB) is one of the muscles which are subjected to many variations. Recently, there is a lot of information about variations in origin but also some cases are reported on variations in insertion of BB. Presented herein was a case of BB muscle having both variant origin and insertion in the same muscle. The variant origin of BB was long and large at its third head (10.2 cm width and 4.9 cm breadth) which arises from mid-shaft of the humerus. More significantly, the medial portion of the head covers and conceals the lower portion of median nerve and brachial artery as an arch which continues medially to be inserted to the medial epicondyle of the humerus. We discuss in this report, the possible mechanism for neurovascular compression by this abnormal third head of BB muscle and the variation of BB from its origin and insertion. Understanding these variations is highly significant in surgical procedures around elbow joint and upper arm and it also pave the way for the differential diagnosis in nerve entrapment syndrome.

The incidence of envenoming from king cobra, Ophiophagus hannah in human is relatively rare. Its venom acts on the postsynaptic region of the neuromuscular junction causing descending flaccid paralysis. Locked-in syndrome is a clinical state of inability to provide motor response in a conscious patient. Many reported cases of locked-in syndrome following neurotoxic snake-bite mimics brain death. We report a case of a middle aged man who presented with progressive neurological deficit following a king cobra bite over his right arm. He had local and systemic neurotoxic envenoming. His condition deteriorated, and was intubated and ventilated in the emergency department. He received a total of 33 vials of the Ophiophagus hannah monospecific antivenom and subsequently recovered well with no neurological deficit. Retrospectively, he was able to recall the events and while he was lying paralysed and intubated under minimal sedation in the intensive care unit. He described it as a terrifying and painful experience. This case highlights the rare presentation of locked-in syndrome following a systemic envenoming from a king cobra bite. It is important to differentiate neurotoxic snake envenoming lock-in syndrome from brain dead. Patients are unable to respond to physical pain and require adequate analgesia. A patient suffering this highly distressing experience may require psychological support.

It is important to differentiate neurotoxic snake envenoming lock-in syndrome from brain dead. Patients are unable to respond to physical pain and require adequate analgesia. Sickle cell disease (SCD) in pregnancy is uncommon in Malaysia. We present a case of sickle cell disease in pregnancy with maternal and fetal complications. The patient presented with acute pain crisis and hemolysis in the third trimester. Despite thromboprophylaxis, she developed deep vein thrombosis. The pregnancy was further complicated by severe pre-eclampsia and intrauterine growth restriction which require preterm caesarean section. The baby was admitted to Neonatal Intensive Care Unit due to prematurity and low birth weight. Multidisciplinary approach in managing pregnant patient with SCD is essential in achieving good obstetrics outcome.

Extramedullary plasmacytoma is a rare complication from multiple myeloma. We report a 56-year-old lady with underlying multiple myeloma who developed swelling over the left eye. It caused a non-axial proptosis, exposure keratopathy and visual acuity of counting fingers. A tissue biopsy revealed infiltration of tissue fragments with neoplastic plasma cells positive for CD138 and Kappa light chain restrictions consistent with plasmacytoma. Following radio-chemotherapy, the mass shrunk tremendously but her visual outcome remained poor.

Hyperbaric oxygen therapy (HBOT) was established to increase oxygenation and antimicrobial effect that potentially improve the healing of chronic ulcer. Present study aim to assess the effects of HBOT in chronic diabetic foot ulcer (DFU). A total of sixty patients classified according to Wagner 1, 2 or 3 chronic diabetic foot ulcers, were recruited and subsequently divided randomly into two groups; HBOT and control group. All patients underwent the standard treatment for DFU, but for the HBOT group, underwent 20 HBOT sessions, each lasted 80 – 90 mins at 2.5 atmospheres absolute (ATA). White cell count (WCC) and C-reactive protein (CRP) levels were taken during inclusion, at second and fourth week of treatment. Wound sizes were documented at each follow up until six months follow up. SF-36 at one-month post hyperbaric oxygen therapy was used to measure the health-related quality of life. Reduction of WCC and CRP in HBOT group were significant throughout the treatment (p=0.046 and p=0.039, respectively). A total of 26 patients (86.7%) from the HBOT group achieved complete ulcer healing at six months’ follow-up, while 18 patients (60%) in the control group’s ulcer healed completely. Patients treated with HBOT had significantly better mental and physical health constituent of quality of life. It must be emphasised that HBOT is an adjunctive therapy to the standard management of chronic DFU in accelerating wound healing for a better quality of life.
