Research progress on infantile spasm 婴儿痉挛的研究进展

Infant spasm is the highest incidence of infantile epilepsy syndrome, the incidence is 0.16/1 000-0.42/1 000, the proportion of male and female children is 6/4.Infantile spasms starts in early childhood, and is hidden, the symptoms of infantile spasms need to be distinguished from other diseases.In the presence of isolated or series of abnormal movements with or without developmental retardation orregression, timely EEG is necessary.And according to the EEG, doctors could give the corresponding drugs to the children as soon as possible to improve the prognosis.Through the correlative literature in recent 10 years, we summarized the causes, the clinical characteristics and the iconography of isnfantile spasms. 婴儿痉挛是婴儿癫痫综合征发病率最高的疾病,发病率为0.16/1 000-0.42/1000,男女儿童比例为6/4。婴儿痉挛始于儿童早期,而且是隐性的,婴儿痉挛的症状需要与其他疾病区分开来。在存在孤立或一系列异常运动的情况下,无论是否有发育迟缓或消退,及时的脑电图都是必要的。根据脑电图,医生可以尽快给孩子服用相应的药物,以改善预后。通过近10年来的相关文献,总结了非纤维性痉挛的病因、临床特点和影像学特点。关键词:婴儿痉挛;头部MRI;脑电图;抗癫痫治疗

The therapeutic effect of glucocorticoids on children with sepsis: A Meta-analysis 糖皮质激素治疗儿童脓毒症的疗效:荟萃分析

Objective To evaluate the effect of glucocorticoid therapy in children with sepsis. Methods Databases including PubMed, EMbase, The Cochrane Library, Wanfang Data, VIP and CNKI were searched from inception to July 2019 to collect randomized controlled trials(RCTs)about glucocorticoid for children with sepsis.Two independent researchers screened the literature, extracted data, and assessed the risk of bias in the included studies.Data results were analyzed by RevMan5.3 software for Meta-analysis. Results A total of 7 RCTs, involving 1 359 patients were included.The results of Meta-analysis showed that, compared with the control group, the corticosteroid treatment group did not improve mortality(OR=1.09, 95%CI 0.61, 1.97, P=0.76), length of stay(MD=-3.13, 95%CI -8.29, 2.03, P=0.23), PICU stay time(MD=-2.33, 95%CI -5.03, 0.37, P=0.09), mechanical ventilation time(MD=-2.01, 95%CI -4.66, 0.65, P=0.14), in hospital infection rate(OR=1.58, 95%CI 0.27, 9.21, P=0.61). Conclusion The existing evidence shows that glucocorticoid adjuvant therapy can not effectively reduce the mortality, hospital stay, PICU stay time, mechanical ventilation time, hospital infection rate of children with sepsis, but it is limited by the number and quality of the included studies, which needs further large-scale, multicenter, double-blind, randomized controlled trials to verify. 目的探讨糖皮质激素治疗儿童脓毒症的疗效。方法检索PubMed、EMbase、The Cochrane Library、万方数据、VIP、中国知网等数据库,收集自成立以来至2019年7月关于糖皮质激素治疗儿童脓毒症的随机对照试验(RCTs)。两名独立研究人员筛选文献,提取数据,并评估纳入研究的偏倚风险。数据结果采用RevMan5.3软件进行meta分析。结果共纳入7项rct,共1 359例患者。meta分析结果显示,与对照组相比,皮质类固醇治疗组未改善患儿死亡率(OR=1.09, 95%CI 0.61, 1.97, P=0.76)、住院时间(MD=-3.13, 95%CI -8.29, 2.03, P=0.23)、PICU住院时间(MD=-2.33, 95%CI -5.03, 0.37, P=0.09)、机械通气时间(MD=-2.01, 95%CI -4.66, 0.65, P=0.14)、院内感染率(OR=1.58, 95%CI 0.27, 9.21, P=0.61)。结论现有证据表明,糖皮质激素辅助治疗不能有效降低脓毒症患儿的死亡率、住院时间、PICU住院时间、机械通气时间、医院感染率,但受纳入研究数量和质量的限制,需要进一步开展大规模、多中心、双盲、随机对照试验进行验证。关键词:糖皮质激素;脓毒症;孩子;荟萃分析

Infection is the most common disease in children and a major problem that plagues clinicians.Imaging examination is an important tool for the diagnosis and evaluation of infectious diseases, providing evidences for early diagnosis.Although the imaging can not identify the pathogen, it can provide important clues for the determination of the pathogen, narrow the scope of the suspected pathogens, and provide a basis for the clinician′s empirical treatment. Emergency rescue and transportation scheme for pediatrics in the background of flood disasters 洪涝灾害背景下的儿科应急救援与运输方案

Children′s unique physiological, developmental and psychological attributes make them one of the most vulnerable groups in large-scale flood disasters.Formulating a reasonable and feasible emergency preparedness plan for flood disasters can improve their ability to cope with flood disasters and optimize the outcomes of children suffering from flood disasters.The emergency preparedness plan for children′s flood disaster should cover the allocation and utilization of various resources during disasters, regional cooperative rescue and evacuation of children, ethical problems in treatment under special circumstances, psychological counseling of children, family members and medical personnel.Relevant staff of pediatric emergency rescue need to consider all aspects and make reasonable plans so that the rescue work of children affected by disasters can be carried out in an orderly manner. Epidemiological and clinical characteristics of novel coronavirus infection in children: Thoughts on the diagnostic criteria of suspected cases outside Hubei Province/ 中国小儿急救医学 儿童新型冠状病毒感染的流行病学和临床特征——对湖北省外疑似病例诊断标准的思考中国小儿急救医学

Objective To improve the diagnostic criteria of suspected cases through investigating the epidemiological and clinical manifestations of confirmed cases of new-type coronavirus infection in children. Methods We retrospective analyzed the epidemiological and clinical manifestations of 6 children with new coronavirus infection diagnosed in Chongqing Three Gorges Central Hospital from February 3, 2020 to February 15, 2020 . Compared with the diagnostic criteria of suspected cases,we summarized the problems encountered in the application of this standard in clinical work and try to put forward Suggestions for improvement. Results Among the 6 children with confirmed cases: 5 males and 1 female; 3 from Hubei Province and 3 from Wanzhou ; 6 cases of clustered onset of the family; Visiting nature: 3 cases of suspected case income, 3 cases of community or outpatient screening . Three cases with fever and / or respiratory symptoms, one of which had symptoms of diarrhea; all children's blood routine and lymphocyte counts were within the normal range; chest CT imaging except for cases No. 1 and No. 5 were in line with typical new coronavirus pneumonia signs. In addition, the remaining 3 patients had abnormal imaging but did not have the characteristics of new coronavirus pneumonia, and 1 case was normal. Comparison results:Only case 1 of all cases fully met the diagnostic criteria, and the remaining cases did not meet the diagnostic criteria of early suspected cases. Conclusion In order to improve the accuracy and practicality of the diagnosis of suspected cases in children, it is recommended to refine and standardize the diagnostic criteria of some suspected cases. Analysis of risk factors for neonatal transfusion-associated necrotizing enterocolitis 新生儿输血相关坏死性小肠结肠炎危险因素分析

Objective To investigate the clinical relevant factors associated with transfusion associated necrotizing enterocolitis (TANEC) in order to reduce the incidence of neonatal necrotizing enterocolitis (NEC). Methods The clinical data of neonates admitted to the First Hospital of Lanzhou University and received blood transfusion therapy from January 2017 to June 2018 were collected, including perinatal factors, basic conditions of children, and comorbidities.According to the occurrence or absence of NEC within 48 hours after transfusion, the patients were divided into TANEC group and no-TANEC group.The clinical data of the two groups were compared. Results Univariate analysis showed that there were statistical differences (P<0.05) in the mode of delivery, gestational age, birth weight, neonatal sepsis, patent ductus arteriosus(PDA), neonatal respiratory distress syndrome (NRDS), and anemia.The logistic multivariate analysis revealed that gestational age(P<0.05, OR=0.772, 95%CI: 0.684-0.871), the birth weight(P<0.05, OR=0.236, 95%CI: 0.079-0.711)were protect fators of TANEC, the degree of anemia(mode 1: P<0.05, OR=3.129, 95%CI: 1.003-9.756; mode 2: P<0.05, OR=3.449, 95%CI: 1.024-11.609)and sepsis (mode 1: P<0.05, OR=6.327, 95%CI: 1.732-23.720; mode 2: P<0.05, OR=8.154, 95%CI: 2.122-31.336)were significant risk fators for TANEC. Conclusion The factors leading to the occurrence of TANEC are various.When transfused, the greater the gestational age, the higher the birth weight, the lower the risk of developing NEC.The more severe the combination of neonatal sepsis and anemia, the higher the risk of developing TANEC.Clinically, comprehensive measures should be taken to prevent neonatal anemia.According to the specific conditions of the children and the degree of anemia, a reasonable clinical strategy should be formulated to avoid blood transfusion to reduce the incidence of TANEC and improve the prognosis of the children. 目的探讨输血相关性坏死性小肠结肠炎(TANEC)的临床相关因素,以降低新生儿坏死性小肠结肠炎(NEC)的发病率。方法收集2017年1月至2018年6月兰州大学第一医院住院并接受输血治疗的新生儿的临床资料,包括围生期因素、患儿基本情况及合并症。根据输血后48小时内有无NEC的发生分为TANEC组和无TANEC组。比较两组患者的临床资料。结果单因素分析显示,分娩方式、胎龄、出生体重、新生儿脓毒症、动脉导管未闭(PDA)、新生儿呼吸窘迫综合征(NRDS)、贫血等差异均有统计学意义(P<0.05)。logistic多因素分析显示,胎龄(P<0.05, OR=0.772, 95%CI: 0.684 ~ 0.871)、出生体重(P<0.05, OR=0.236, 95%CI: 0.079 ~ 0.711)、贫血程度(模式1:P<0.05, OR=3.129, 95%CI: 1.003 ~ 9.756;模式2:P<0.05, OR=3.449, 95%CI: 1.024 ~ 11.609)和脓毒症(模式1:P<0.05, OR=6.327, 95%CI: 1.732 ~ 23.720;模式2:P<0.05, OR=8.154, 95%CI: 2.122 ~ 31.336)是TANEC的显著危险因素。结论导致TANEC发生的因素是多种多样的。当输血时,胎龄越大,出生体重越高,发生NEC的风险越低。新生儿败血症和贫血合并越严重,发生TANEC的风险越高。临床应采取综合措施预防新生儿贫血。根据患儿的具体情况及贫血程度,制定合理的临床策略,避免输血,以降低TANEC的发生率,改善患儿预后。关键词:新生儿;贫血;输血;输血相关的坏死性小肠结肠炎;脓毒症

Sleep disruption is prevalent in patients of pediatric intensive care unit(PICU). A variety of factors can cause sleep disruption in critical children, which may have an impact on disease recovery and long-term prognosis.Currently, there is still no systematic study on sleep disruption in critical children, and the available evaluation tools on sleep in PICU are limited.The diversified sleep management programs may be effective in promoting sleep. Delirium is an acute, fluctuating state of brain dysfunction characterized by a change in consciousness and a reduced ability to concentrate, maintain, or transfer, which can lead to cognitive or perceptual disorders, hallucinations and sleep-arousal disorders.It cannot be better explained by pre-existing, occurring or developing diseases.At present, its pathogenesis is not clear, and is related to a variety of factors.Early and effective identification and evaluation is the premise, and the clinical practice strategy of prevention and treatment of pediatric delirium needs to be further studied. Progress in the etiology, diagnosis and treatment of cholangitis after biliary atresia Kasai surgery 胆道闭锁开赛手术后胆管炎的病因、诊断和治疗进展

Biliary atresia is a serious congenital malformation that threatens the life of newborns.At present, the treatment of biliary atresia mainly relies on Kasai portoenterostomy which is also named hepatoportoenterostomy to correct the dysplastic biliary system.Cholangitis is the most common and intractable complication after Kasai portoenterostomy.The pathogenesis is still unidentified.Many factors including ascending infection of intestinal bacteria, abnormal development of intrahepatic bile duct, surgical injury, reflux of intestinal contentscan and so on can affect the occurrence and development of the disease.The initial time and frequency of cholangitis can affect the postoperative primary liver function, so it is especially important to diagnose and treat it timely.The diagnosis of post-Kasai cholangitis is lack of specificity, mainly based on clinical manifestations, biochemical abnormalities and surgical history.For intractable cholangitis should consider completing radioisotope hepatobiliary imaging, percutaneous cholangiography and single or double-balloon enteroscopy.The main focus of prevention and treatment lies in preoperative care, surgical improvement and postoperative care, as well as drug prevention during disease follow-up. Evolving concepts in guidelines for analgesic and sedative treatment 镇痛和镇静治疗指南概念的演变

In 2018, the Society of Critical Care Medicine(SCCM) and the Chinese Society of Critical Care Medicine(CSCCM) drawn up guidelines for analgesic and sedative treatment in adult patients in the intensive care unit successively.In 2019, the emergency group of Chinese Pediatric Society, Chinese Medical Association published "the experts′ consensus on sedation and analgesia for children in pediatric intensive care unit of China" . The new guidelines have been revised and updated on the basis of the previous ones.Especially, some new concepts have been gradually implemented.This paper seeked to summarize these new concepts and aid clinicians in clinical decision-making in intensive care unit.
