Acute kidney injury and COVID-19 in young adults in intensive care 重症监护青年急性肾损伤与COVID-19

Objective To describe acute kidney injury prevalence in young adults diagnosed with COVID-19 admitted to the Intensive Care Unit. Methods This is a retrospective, quantitative and analytical study. The sample consisted of young adults (20 to 40 years old) admitted to Intensive Care Units, diagnosed with SARS-CoV-2 infection between March and December 2020. Data were obtained through electronic medical records, and kidney injury acute was defined by the creatinine value, according to the Kidney Disease Improving Global Outcomes guidelines criteria. Statistical significance was p≤0.05. Results A total of 58 young adults were hospitalized, 63.8% of whom were male. Hypertension was present in 39.6%, obesity in 18.9%, and diabetes mellitus in 8.6%. Acute kidney injury was identified in 55.1%, with stage 3 predominating in 43.1% of them. In these patients, the use of mechanical ventilation and vasoactive drugs was significant in 92% as well as respiratory organ dysfunction (80%), followed by renal organ dysfunction (76%). Risk factors such as kidney transplantation or chronic kidney disease and obesity increased by 12.3 and 9.0 times, respectively, the chances of developing acute kidney injury. Conclusion This study demonstrated a high kidney injury prevalence in young adults and its association with previous comorbidities. Obesity, kidney transplantation and chronic kidney disease increased the chance of young adults to develop acute kidney injury, resulting in outcomes in favor of morbidity and mortality.

Alta hospitalar de pacientes adultos e idosos: elaboração e validação de checklist 成人和老年患者出院:检查表的编制和验证

Objective: To describe content elaboration and validity of a checklist for preparing adults and older adults for hospital discharge. Methods: This is a methodological study, developed from May 2020 to September 2022 (in two stages), for checklist elaboration and validity. The Delphi technique was used, with assessment by an expert committee for content validity. To calculate the degree of agreement, the agreement rate and the Content Validity Index (CVI) were used. Results: A checklist with 17 items that help organize hospital discharge was prepared and validated. The checklist was prepared based on the compilation of results obtained from interviews with multidisciplinary team professionals, who worked in inpatient units, an integrative review on transition of care at hospital discharge of adult patients and reading of articles on the use of discharge checklist. In the first stage of validity, a mean was obtained for the instrument’s agreement rate, scope (94%) and relevance (91%). At the end of the second round, the mean CVI calculation was obtained (clarity: 0.95; relevance: 0.96). Conclusion: The checklist was validated as to its content by consensus by an expert committee, and can be used by care teams or hospital discharge management

Implementation of a knowledge translation and exchange intervention for pain management in neonates 新生儿疼痛管理的知识翻译和交流干预的实施

Objective To describe the implementation process of a multifaceted knowledge translation and exchange intervention to improve pain management practices, and to evaluate the adoption of this intervention by health professionals during painful procedures in neonates. Methods A quasi-experimental before-and-after study developed in a neonatal unit. The Evidence-Based Practice for Improving Quality intervention guided by the conceptual framework The Promoting Action on Research Implementation in Health Services was implemented in two stages (preparation and implementation). Its adoption was measured by clinical indicators related to pain management presented through descriptive statistics. Results After discussion on existing practices in the unit that needed to be changed, synthesis of current scientific evidence and local context data, members of the unit’s Research and Practice Council developed and implemented coherent and achievable goals for the change of practice in pain management, selected knowledge translation and exchange strategies, determined the target audience and indicators, and implemented the interventions. There was a 32.8% reduction in the number of painful procedures performed, an increase of 26.6-50.7% in the use of pain assessment scales and of 25.1% in the administration of oral glucose. Conclusion The multifaceted Evidence-Based Practice for Improving Quality intervention is complex, and has processes that demand knowledge and skills, commitment from the various actors involved, availability of time and financial investment. The analyzed indicators showed that the intervention resulted in positive changes in clinical practice in the management of pain in neonates.

A total of 123 nursing diagnosis/ outcome statements and 215 nursing intervention statements, distributed by Callista Roy’s adaptation model of nursing modes were assessed. To assess the degree of agreement regarding the meaning and clinical use among experts, the Content Validity Index (CVI) was used, accepting statements that presented an index ≥ 0.80

Objective: To evaluate the prevalence of risk for Burnout Syndrome among health professionals in areas of care for patients with COVID-19, as well as to verify possible associations of the syndrome with the perceived social support and food insecurity of these workers. Methods: Analytical cross-sectional study, with workers from intensive care units (ICU) and wards caring for patients with COVID-19, in a university hospital. Data collection was carried out from September to October 2021, using the instruments: Maslach Burnout Inventory (MBI-HSS), Multidimensional Perceived Social Support Scale and Food Insecurity Experience Scale (FIES). Statistical analysis used Poisson regression models and multiple Poisson regression, considering statistically significant differences and associations if p<0.05. Results: A total of 75 workers from three wards (48%) and from one ICU (52%) participated in the survey, and the professionals are mostly female (89.3%), trained as health care technicians (66.7%).
