The conventional treatment for periapical periodontitis faces challenges,especially in the cases with immature roots.This article reports a case of periapical periodontitis in an adult permanent tooth with an immature root,where erbium-doped yttrium aluminum garnet(Er:YAG)laser-activated root canal disinfection was employed to induce apical bleeding and pulp revascularization.A 22-month postoperative follow-up showed healing of the periapical lesion,narrowing of the apical foramen,and positive pulp vitality test results.

Objective To investigate the effect of health failure mode and effect analysis(HFMEA)in optimizing the management process of postoperative diabetes insipidus in children undergoing neurosurgery.Methods Based on HFMEA,a management flowchart for postoperative diabetes insipidus in children undergoing neurosurgery was created.Brainstorming was adopted to identify failure modes in the workflow,analyze risk factors,and develop improvement measures,thereby refining the management flowchart.The amelioration and prognosis of diabetes insipidus in these children before(October 2022 to November 2023)and after(January 2024 to February 2025)implementation of the management flowchart were compared.Results The HFMEA-based management process for postoperative diabetes insipidus in children undergoing neurosurgery alleviated the symptoms of diabetes insipidus regarding the number of diabetes insipidus in the pediatric intensive care unit(P=0.006),the average daily urine output in the pediatric intensive care unit(P=0.001),the proportion of electrolyte abnormalities at discharge/transfer(P=0.037),the duration of mechanical ventilation(P=0.007),and the length of stay in the intensive care unit(P=0.001).Conclusion The HFMEA-based management process for postoperative diabetes insipidus in children undergoing neurosurgery is beneficial to the optimization of the management process,the alleviation of postoperative diabetes insipidus,and the improvement of prognosis in these children.

As population aging aggravates,the prevalence of dementia is increasing,seriously impacting the daily lives of patients.Sensory stimulation therapy has emerged as a safe and effective non-pharmacological intervention for individuals with dementia.Consequently,exploring non-pharmacological treatments for Alzheimer's disease is crucial.By a review of the relevant literature,this paper provides an overview of sensory stimulation therapy in terms of the concept,applications,and effects on dementia,aiming to promote the broader implementation of this therapy in the care of individuals with dementia.

Objective To explore the functional mechanism of spinal cord fragile X mental retardation protein(FMRP)involved in neuropathic pain(NP)by using the sciatic nerve model of chronic compression injury(CCI).Methods First,to investigate the changes of spinal cord FMRP and β-catenin following the development of NP,this study compared the 50%mechanical withdrawal threshold(MWT)and thermal withdrawal latency(TWL)in CCI rats,as well as changes of FMRP and β-catenin in the spinal dorsal horn post-surgery,through random grouping.Immunofluorescence staining was performed on spinal cord tissue sections from CCI rats.Second,to further validate the alterations in pain behavior when the FMRP function was lost,we measured the 50%MWT,TWL,and FMRP and β-catenin in the spinal dorsal horn after FMRP knockdown in CCI rats.Finally,we measured the 50%MWT,TWL,and FMRP and β-catenin in the case of FMRP hyperfunction for validation.Results Compared with the baseline CCI group and the naive and sham groups after modeling,the CCI group after modeling showed decreases in 50%MWT and TWL(all P<0.001).After modeling,compared with the naive group and the sham group,the CCI group presented up-regulated expression of FMRP(P=0.027,P=0.022)and β-catenin(P<0.001,P=0.001)in the spinal dorsal horn.No co-localization of FMRP with astrocytes and microglia was observed in the spinal cord,while the co-localization with neurons was observed.Compared with the baseline,the CCI+FMRP knockdown group showed decreases in 50%MWT(P=0.015)and TWL(P=0.001)after modeling.After intrathecal injection of small interfering RNA(siRNA),the 50%MWT(P=0.020)and TWL(P=0.009)of the CCI+FMRP knockdown group were increased.Moreover,compared with the CCI group and the CCI+solvent group,the CCI+FMRP knockdown group showed increases in 50%MWT(both P<0.001)and TWL(P=0.005,P=0.006).After intrathecal injection of siRNA,the expression levels of FMRP(P=0.012,P=0.007)and β-catenin(both P<0.001)in the spinal dorsal horn of the CCI+FMRP knockdown group were lower than those of the CCI group and the CCI+solvent group.Compared with the baseline FMRP overexpression group and the naive and negative control groups after adeno-associated virus(AAV)injection,the FMRP overexpression group after AAV injection showed decreases in 50%MWT and TWL(all P<0.001).After AAV injection,compared with the naive group and the negative control group,the FMRP overexpression group demonstrated up-regulated expression of FMRP(both P<0.001)and β-catenin(P=0.006,P=0.008)in the spinal cord.Conclusions This study confirms that spinal cord FMRP and β-catenin are involved in NP induced by CCI.Spinal cord FMRP may be one of the potential therapeutic targets for NP.

Objective To develop a diagnostic model combining the CT angiography(CCTA)-derived myocardial radiomics signatures with the CT-derived fractional flow reserve(CT-FFR)based on coronary CCTA and investigate the diagnostic accuracy of the hybrid model for hemodynamically significant coronary artery disease(CAD).Methods The patients presenting stable angina pectoris,diagnosed with CAD,and clinically referred for CCTA examination and invasive coronary angiography were prospectively recruited.Radiomics features of the left ventricular myocardium were extracted from the three main perfusion territories demarcated according to the coronary blood supply.The extracted features were first selected by the minimum redundancy maximum relevance feature ranking method.A least absolute shrinkage and selection operator Logistic regression algorithm with leave-one-out cross-validation was then employed to construct a radiomics model.The CT-FFR value was generated for each blood vessel.The area under the receiver operating characteristics curve(AUC_ROC),sensitivity,and specificity were adopted to evaluate the performance of each model against the reference standard invasive coronary angiography/FFR.Results A total of 70 patients[42 men and 28 women;(61±10) years old] were included in this study and complemented CCTA examination,with 175 vessels and the corresponding myocardial territories undergoing invasive coronary angiography/FFR.A total of 1 656 specific radiomics parameters were extracted,from which 14 features were selected to establish the radiomics model.The AUC_ROC,sensitivity,and specificity were 0.797(95%CI=0.732-0.861),77.1%,and 73.7%for the radiomics model,0.892(95%CI=0.841-0.943),81.4%,and 88.8%for the CT-FFR model,and 0.928(95%CI=0.890-0.965),83.3%,and 88.4%for the hybrid model,respectively.The hybrid model outperformed the radiomics model and CT-FFR alone(P=0.040).Conclusions The radiomics signatures of the vessel-related myocardium from CCTA could provide incremental value to the diagnostic performance of CT-FFR and improve vessel-specific ischemia detection.The hybrid model combining CT-FFR with radiomics signatures is potentially feasible for improving the diagnostic accuracy for hemodynamically significant CAD.

Objective To explore the research progress and hotspots of intelligent ultrasound imaging in the diagnosis of thyroid nodules and clarify the research directions via the bibliometric method.Methods The relevant research articles on intelligent ultrasound imaging in the diagnosis of thyroid nodules were retrieved from the Web of Science Core Collection,covering the period from January 2004 to August 2024.Python was used to analyze the number of annual publications.VOSviewer was used to create the co-occurrence network of authors and the keyword density map.CiteSpace was used to demonstrate the dual-map overlays of the journals,as well as the bursts and clustering of co-citations and keywords.Results A total of 1 179 articles were included.The annual number of publications increased steadily.The involved journals demonstrated high quality,and the publications showed a trend of cross-research.Chinese researchers were the core research force in this field.Haugen et al.'s study on the guidelines for thyroid nodules had the most citations.The clustering of co-citations and keywords indicated studies in multiple fields.Thyroid nodules,cancer,and deep learning were the representative keywords in this field.Conclusions The continuous enrichment of research topics promotes the rapid development of intelligent ultrasound imaging for thyroid nodules.Intelligent diagnosis methods based on deep learning can provide diagnostic suggestions,while there are still challenges such as interpretation.One of the research directions is the deep combination of intelligent diagnosis algorithms and medical knowledge.

Objective To investigate the correlations of circulating miRNA-205 expression with systemic immune-inflammation index(SII)and systemic inflammation response index(SIRI)in patients with severe obstructive sleep apnea(OSA). Methods The patients who attended the Hypertension Center of the People's Hospital of Xinjiang Uygur Autonomous Region from January to June 2023 and underwent complete overnight polysomnography were consecutively included in this study.Among them,30 patients had severe OSA,and 32 patients did not have OSA.Blood routine tests(white blood cells,neutrophils,monocytes,platelets,etc.)were performed and the expression of miRNA-205 was determined by quantitative reverse transcription PCR.Simple regression was adopted to analyze the correlations among miRNA-205,SII,SIRI,and OSA parameters.The potential regulatory effects of miRNA-205 on OSA and inflammation indices were further evaluated. Results The patients with severe OSA showed lower expression of circulating miRNA-205[1.910(1.240,2.403)vs.3.650(2.148,5.109),z=-3.874,P<0.001]and higher SIRI[1.090(0.775,1.573)vs.0.870(0.650,1.240),z=-2.031,P=0.041]and SII[555.200(451.780,936.350)vs.448.685(380.823,646.073),z=-2.029,P=0.042]than non-OSA patients.In the whole population,apnea-hypopnea index(AHI)showed a negative correlation with circulating miRNA-205(r=-0.391,P=0.002).Among severe OSA patients,each 1-unit increase in AHI was associated with a reduction of 0.030 in miRNA-205 and increases of 10.046 and 0.037 in SII and SIRI,respectively(SII:P=0.003;SIRI:P=0.037).Conversely,each 1-unit rise in miRNA-205 predicted a decrease of 121.093 in SII(β=-0.40,P=0.046).The low expression of miRNA-205 might have a negative moderating effect on elevated SII(β=-0.40,P=0.004). Conclusions Compared with the patients without OSA,those with severe OSA showed elevated SII and SIRI and down-regulated expression of miRNA-205.The low expression of miRNA-205 might have a negative moderating effect on the systemic inflammatory state associated with severe OSA.

Objective To investigate the value of ultrasonographic features combined with immunohistochemistry in predicting axillary lymph node metastasis in middle-aged women with breast cancer.Methods A retrospective analysis was conducted on 827 middle-aged female breast cancer patients who underwent surgical treatment at the Affiliated Hospital of Chengde Medical University from June 2017 to June 2023.Ultrasonographic and immunohistochemical information was collected,and the patients were randomly allocated into a training set(579 patients)and a validation set(248 patients).Univariate and multivariate Logistic regression analyses were performed to identify ultrasonographic and immunohistochemical risk factors associated with axillary lymph node metastasis in these patients,and a nomogram model was developed.Receiver operating characteristic curves and calibration curves were established to evaluate the performance of the nomogram model,and clinical decision curves were built to assess the clinical value of the model.Results The maximum diameter,morphology,boundary,calcification,and expression of human epidermal growth facor receptor 2 and Ki-67 in breast cancer lesions were identified as risk factors for predicting axillary lymph node metastasis in middle-aged women.The areas under the curve of the nomogram model on the training and validation sets were 0.747(0.707-0.787)and 0.714(0.647-0.780),respectively.Calibration curves and clinical decision curves indicated good consistency and performance of the model.Conclusion The nomogram model constructed based on ultrasonographic features and immunohistochemistry of the primary breast cancer lesion demonstrates high value in predicting axillary lymph node metastasis in middle-aged women with breast cancer.
