Management and Prevention of Sarcopenia in Era Covid-19 新冠肺炎患者肌肉减少症的管理与预防

Xanthogranulomatous pyelonephritis (XGP) is a rare and severe manifestation of chronic kidney inflammation that can be critical if not recognized and treated appropriately, often requires surgical intervention along with antibiotics. Most commonly presented in the fifth or sixth decade of life with a prior history of nephrolithiasis, obstructive uropathy, or recurrent urinary tract infections. Here, we discuss a 45-year-old male and a 73-year-old female who came with abdominal pain and weight loss for two months. An enlarged and distorted renal outline, with altered echotexture and calculus in renal pelvis was revealed on ultrasound examination. Abdominal computed tomography revealed staghorn renal calculi and thinning of renal cortex with involvement of adjacent structures.

Subtypes of the Human Immunodeficiency Virus Circulating in Professional Sex Workers Community: A Different Epidemiology 在专业性工作者群体中传播的人类免疫缺陷病毒亚型:不同的流行病学

Background: HIV-1 has a genetic diversity that is equal to the complexity of its management. Professional Sex Workers (PSW) have always been considered as a key population for Sexually Transmitted Infections around the world. They are often incriminated for transporting and introducing cosmopolitan subtypes into populations. Objective: The objective of this review was to present the different strains of Human Immunodeficiency Virus type 1 (HIV-1) which circulate in the population of Professional Sex Workers in Kinshasa, Democratic Republic of Congo (DRC) and their evolution. Methods: Various publications related to the identification of the different variants of HIV-1 among PSW in Kinshasa-DRC were the subject of this literature review. The research for these different works on the different variants of type 1 HIV was done on the internet from websites using specific keywords. The search was limited to published work and abstracts presented from 2000 to date. The manuscripts were selected according to the relevance of the methodology, the results as well as the representativeness of the samples. The socio-demographic information of the populations studied, the measurement methods and the objectives were taken into account in the evaluation of the articles. Results: From 2000 to 2021, 2 works were documented carried out mainly on the PSWs for Kinshasa-DRC meeting the various selection criteria. In 2012, according to the IBBS, subtype A was dominant with 4 PSWs (30.8%), followed by subtypes G with 2 PSWs (15.4%) and C with 1 PSW (7.7%). In 2014, according to Kamangu NE et al, the dominant subtype was K in 5 patients (25%), followed by subtypes A and G in 3 patients each (15%). Conclusion: It emerges from this work that this key population is a group with a complex and special epidemiology. The prevalence of subtype A has been declining over the years while other subtypes are on the rise.

Atrial Fibrillation with Ventricular Pre-Excitation: A Diagnosis That Must Not Be Missed 房颤伴心室预激:一个不容错过的诊断

Wolff-Parkinson-White (WPW) syndrome is caused by an accessory pathway that communicates between the atria and ventricles known as the Bundle of Kent. The development of atrial fibrillation, can result in the atrial impulses all being conducted via the accessory pathway and result in a sinister, board complex, irregular tachycardia, with varying QRS morphology (known as pre-excited atrial fibrillation) Adenosine is a potent atrioventricular node blocker, which can be used in the treatment of supraventricular tachycardias, but also has diagnostic utility, particularly in differentiating between supraventricular tachycardia with aberrant conduction (which would often terminate) and a ventricular tachycardia (which would not respond to adenosine). However, the administration of adenosine in pre-excited atrial fibrillation can precipitate 1:1 atrial to ventricular conduction, which can degenerate into life-threatening ventricular arrythmias. This case describes a patient who presented with pre-excited atrial fibrillation and received intravenous adenosine that resulted in development of broad complex tachycardia with haemodynamic compromise. In patients with pre-exited atrial fibrillation, AV nodal blocking agents should be avoided and direct current cardioversion should be utilised.

A Case Report: Electrophysiological Findings in Degenerative Myopia 退行性近视的电生理表现1例报告

We report electrophysiological findings in a patient that has degenerative myopia. The visual acuity of a 48 years old male patient was counting fingers from 0.1 with refraction (-11.00, -1.00 x115) in the right eye and 0.6 with refraction (-5.00, -1.50 x35) in the left eye. Bilateral anterior segment examination was normal however; on the fundus examination, there were atrophic scar area in the macula and pigmented degenerative areas in the inferior on the right, while there were chorioretinal atrophy and degenerative areas in the peripheric retina on the left. Normal latency response was obtained in VEP. In the right eye, a low amplitude and long latency response was obtained with the 120' pattern, but no response was obtained with the other patterns. In the left eye, there was a response to patterns shrinking to 15', the p100 wave was minimally low for age, and its latency was within normal limits. 7' pattern could not be answered. With ERG, bilateral rod b wave responses were subnormal for age, and cone b wave responses were low. Oscillatory responses were obtained bilaterally low.

Stem Cell Therapy Modulates Cerebral Palsy: Case Report 干细胞治疗调节脑瘫1例报告

A case of 9 years old Iraqi girl who had a moderate level of spastic cerebral palsy plus scissoring gait came to Al Zahrawy Center for Stem Cells in Baghdad in 2016. After proving diagnosis, the parents took their girl to distinctive pediatrics, orthopedics, physiotherapists, and neurologists. After a comprehensive check, we discussed the use of stem cells, the parents accepted and verbally consented. Later, after four sequential months of one injection per month of stem cells injections, the parents noticed gradual advancement of the mentioned girl state. Stem cell therapy modulates cerebral palsy, so we recommended it.

Evaluation of the Difference Between non-COVID-19 Pneumonia and COVID-19 Pneumonia Nursing Care 非COVID-19肺炎与COVID-19肺炎护理差异评价

Aim This study was aimed to contribute to the nursing care of COVID-19 pneumonia by determining the similarities and differences in nursing care for COVID-19 pneumonia and non-COVID-19 pneumonia. Methods This study aimed to retrospectively evaluate the nursing care applied to 84 pneumonia patients who were hospitalized in Sivas Numune Hospital Chest Diseases Service between 2018-2019 and 81 in patients with COVID-19 pneumonia between May and November 2020. Results COVID-19 pneumonia patients were older than non-COVID-19 pneumonia patients (p<0.001), and the majority of patients were hypertensive and diabetic (p<0.05). The most common symptom for COVID-19 pneumonia patients is dyspnea (p<0.05) while for non-COVID-19 pneumonia patients it is fever (p<0.001). 77.8% of COVID-19 pneumonia patients but non-COVID-19 pneumonia patients had bilateral pneumonia with the feature of multiple mottling and ground-glass opacity on CT images. LDH, CRP, PCT, AST/ALT, and D-dimer levels increased significantly higher in COVID-19 pneumonia patients than in COVID-19 pneumonia patients (p<0.001). 26.2% of non-COVID-19 pneumonia patients, all of COVID-19 pneumonia patients' liver function tests, and coagulation factors were regularly monitored. COVID-19 pneumonia patients received %100 of patients antiviral drugs, 74.1% of patients corticosteroids, 92.5% of patients anticoagulant, 39.5% psychological support different from non-COVID-19 patients. Conclusion D-dimer and procalcitonin elevation in patients who develop pneumonia due to COVID-19 may be indicators of the need for intensive care in nursing care. Besides, it was concluded that liver functions and DVT follow-up in COVID-19 pneumonia should be included in nursing care.

A Challenging Case of Pneumonia and Vasculitis 一例具有挑战性的肺炎和血管炎病例

An elderly (76 years old) immunocompromised Iraqi male had carcinoma of colon stage 3c without metastasis operated for seven years before, plus chemotherapy for one year. In winter 2022, this man sorrowed from a sore throat, voice change, and a rise in temperature (37.4 C). Later, dry cough developed in subsequent hours and deteriorated quickly to yellow-colored sputum, plus an increment in his temperature (37.8 C). After that, the patient had non-specific, non-itchy, follicular erythematous papules. The rash varies in size and shape, distributed in the abdomen, lower back, and legs. Two physicians proved pneumonia by clinical means, plus two successive chest x-rays, but the patient refused the laboratory investigations for Covid. He took two doses of the Chinese vaccine (Sinopharm) six months before recent symptoms. After strict treatment, the cough became less frequent, white-colored sputum, fever dropped gradually, at the end of the week. On the tenth day from the beginning of the man's symptoms, the skin rash faded slowly due to prednisolone usage.

The main element that supports lymph and blood circulation in the body - the processes of cell metabolism, are musculoskeletal structures. This is indicated by human physiology, but this is not taken into account in the treatment - rehabilitation of the body. The skeleton must be considered not only as a system of levers and muscles that form our posture, but also as a set of movements that are laid down by nature to support the functioning of lymphatic and venous-muscle pumps. The paired muscles hold the bones of the skeleton in the so-called neutral stable position. This achieves the relative stability of the body, in which the General Center of Gravity (GCG) of the body constantly makes oscillations in the X-Y planes within 2-4 cm from its neutral position. So in a state of rest, cell nutrition is continuously maintained, and their decay products are excreted. Without muscle contraction, the respiratory, digestive, and thermoregulation systems would not work.

Abdominal complications related to ventriculoperitoneal shunting are an entity described in the literature. Among these is evisceration. In this article we present a case of evisceration in a 5-month-old child, six days after DVP placement. We will discuss the probable etiology and prevention of such a complication.

Many people affected by disability require assistive devices, particularly mobility aids, in order to live independently and fully integrate into their communities. Throughout the world, numerous methods have been implemented in facilitating the provision of these devices to people affected by disabilities. This case study intends use the Rajanagarindra Institute of Child Development Wheelchair Project, located in Chiang Mai, Thailand, to evaluate the viability and efficacy of mobility-provision programs which adapt wheelchairs and other assistive devices not originally intended for the end user. Programs which must “retro-adapt” equipment, like the Wheelchair Project, rely heavily on medical and mechanical expertise to identify the recipients’ needs and safely modify the device accordingly. By examining the distribution statistics from the Wheelchair Project over its 20 years of operation and the needs of the disability community reported by the National Statistics Office of Thailand alongside a cost-benefit analysis of the declared value provided versus operational cost, this case-study systematically details the scope and financial viability of this approach. While not without constraints, this model succeeds in effectively capitalizing on available resources (both in regard to physical equipment as well as professional expertise) to meet the needs of those affected by disabilities in Thailand and Southeast Asia. Although still limited in size and reach, the Wheelchair Project demonstrably shows its viability and the potential this approach presents given the right circumstances.
