Topical phenytoin in a case of severe fibromyalgia: ex juvantibus proof for fibromyalgia as a localized non-length dependent small fiber neuropathy 局部应用苯妥英钠治疗严重纤维肌痛:纤维肌痛为局限性非长度依赖性小纤维神经病的证据

We present a case of a 52-year old woman suffering from severe fibromyalgia, unable to manage even the lightest of housework, she gradually became crippled and her life became a ‘couch potato’ in patient’s own words. Treatment with topical phenytoin (5% titrated up to 15%) transformed her life, pain nearly vanished, and she became active, looking for a job, doing housework and she can walk rapidly. One explanation of this phenomenon could be that a number of patients suffering from fibromyalgia and having tender points suffer from localized length non-dependent small fiber neuropathy, and this pathology can be found in the skin above the tender points.

Ocular open globe injuries are often disabling for both the victim and society, as most of these eyes may remain unsalvageable. Many initial variables have been identified which can help in predicting the final visual outcome in these eyes. Also, various prognostic tools have been validated by previous studies. This article aims to review all these factors.

A turning point in the HIV epidemic was the introduction of protease inhibitors. It was clear to some that treatment of HIV infected individuals with these potent antiviral agents was needed as early as possible, and with this paradigm shift, HIV became a livable chronic disease (1). There are some lessons learned from the HIV epidemic relevant to the current synthetic opioid epidemic.

Adolescent Internet Gaming Disorder: A Narrative Review 青少年网络游戏障碍:叙述性回顾

This narrative review is based on a literature search on PsycINFO and PubMed that involved entering the terms adolescent internet gaming for papers published during the last five years. Following exclusion criteria, 72 papers could be classified as internet gaming or internet gaming disorder/addiction studies including research on the prevalence, effects/comorbidities, risk factors and interventions for those problems. The prevalence of both internet gaming and internet gaming disorder has varied by culture. The effects/comorbidities have included hyperactivity, inattentiveness, cyberbullying, depression, anxiety and substance use. The risk factor studies have focused on impulsivity, sensation-seeking, and aggressivity. Altered autonomic and central nervous system function have also been notable including decreased heart rate variability and fMRI data showing less activation of the prefrontal cortex, ventral striatum and amygdala (areas that are involved in modulating impulsivity, reward-seeking and aggression respectively) as well as reduced gray and white matter. Surprisingly, given the prevalence and severity of internet gaming disorder, very little prevention/intervention research appears in this recent literature. Research is also missing on peer relationships/rejection as potential risk factors. Like other literature on adolescent problems, this research is limited by primarily deriving from self–report and parent report and by the absence of longitudinal data that might inform whether the behavioral and brain data being reported are effects of or risk factors for internet gaming addiction.

Evidence shows that successful treatment of nicotine addiction improves mortality, despite age at cessation. The extreme hazards of smoking stress the importance of patient-physician discussion that is a significant factor in tobacco cessation. Discussion alone and other methods such as “cold turkey” have proven to have low efficacy at cessation which has led to the development nicotine replacement therapy to help augment cessation.

Today nearly almost every part of the world has been impacted by the corona virus COVID-19 pandemic. COVID-19 has been unrelenting in its impact as thousands and thousands of people have died from its infections and resulting complications. Some countries have experienced severe effects of the virus than others. However, in both situations the virus has been vicious and many of those who have recovered and survived its infections have testified of how deadly this virus is. In the wake of this pandemic nations around the world have taken measures to curb its impact and spread in communities and societies. Preventative measures taken have disrupted in most cases schools and the workforce with many people being forced to work remotely from their homes and with schools going virtual. However, progress in fighting this virus is in the offing as promising vaccines are on the horizon. This paper discusses a preliminary intervention research project whose goal is to mitigate the impact of COVID-19 in Nairobi, Kenya. Results of this study may have promise of being replicated in other parts of Africa affected by this pandemic.
