End-Stage Renal Disease: A Milieu for Neurological Derangements in Addition to Metabolic Disturbances 终末期肾病:除了代谢紊乱外,还有神经紊乱的环境

Short Communication This article highlight a very important aspect of neurologic consequences associated with End-stage renal disease (ESRD) and would like to add our reviews regarding ESRD and how its early and proper management can affect the overall mortality and morbidity associated with ESRD as well as how early management will lead to improved cognitive status in these patients. The very finding of decreased cortical thickness, as well as declining cognitive function in patients with ESRD, should prompt us to think about managing kidney problems as soon as they start since they can lead to impaired cognitive status and debilitating mental conditions overtime which ultimately leads to death [1, 2]. It is therefore very important for us to start evaluating and detecting brain abnormalities in patients with chronic kidney disease (CKD) who have no apparent neurological symptoms which will help us improve the prognosis of patients with ESRD and may lead to a significant reduction in death rates associated with it. [1, 2].

Penile Paget's Disease: A Case Report and Review of the Literature. 阴茎佩吉特病:1例报告及文献复习。

Extramammary Paget's Disease (EMPD) is a rare cutaneous, slow growing, intraepithelial adenocarcinoma that can be either primary (intraepithelial arising within the epidermis) or secondary (intraepithelial spread of a visceral carcinoma). Here we present the case of a 63-year-old male with EMPD of the glans penis stemming from underlying urothelial carcinoma. Our treatment decision elected for management with chemotherapy and local treatment with radiation therapy. Subsequent, review of the literature demonstrated a rare disease with a variety of underlying malignancies causing this secondary pathology. Caregivers should be aware of the association of Paget's disease and urothelial cancer and should have a high index of suspicion that erythematous penile lesions may represent Paget's disease and that penile biopsies should be performed early in this setting.

Chronic Kidney Disease-Mineral and Bone Disorders (CKD-MBD) 慢性肾脏疾病-矿物质和骨骼疾病(CKD-MBD)

The kidney plays a vital role in the metabolism of minerals and bone health. It is not only the target organ of several regulating hormones such as parathormon (PTH) and fibroblast growth factor-23 (FGF-23), but it is also the main organ that activates vitamin D. CKD-MBD was further expanded to include cardiovascular diseases (CVD), left ventricular hypertrophy (LVH), hypertension, immune dysfunction, inflammation and iron deficiency anemia, and thus its treatment is still a major challenge for the nephrologist that necessitates further pushing for the development of new agents with high specificity to the treatment of CKD induced MBD.

Impact of Smoking on Prognosis and Survival of Patients with Bladder Cancer 吸烟对膀胱癌患者预后和生存的影响

Cigarette smoking is the established risk factor for the development of urothelial carcinoma of bladder (UCB). However, the impact of smoking on the outcomes of urothelial carcinoma patients remains poorly investigated. Current evidence proves that cigarette smoking increases the risk of disease recurrence and potentially disease progression in patients with urothelial carcinoma of bladder. Current and heavy long-term smokers seem to be at the greatest risk. The majority of retrospective studies found an association between smoking status as well as cumulative exposure and disease recurrence in patients treated with TURB. The evidence for association with disease progression was less abundant. There are insufficient data investigating the impact on cancer-specific and overall survival in patients with bladder cancer. The impact of smoking with outcomes of T1 UCB or high risk disease, patients receiving intravesical therapies is limited because of a lack of data and controversial findings. Long-term smoking cessation seems to mitigate the detrimental effects of smoking in non-muscle-invasive and muscle-invasive bladder cancer.

Congenital Atrichia with Recurrent Episcleritis-A Rare Case Report 先天性不足症伴复发性巩膜炎1例罕见报告

Congenital atrichia is a rare disorder and is often associated with various systemic diseases. Ocular involvement has also been reported. We report a rare case of congenital atrichia with recurrent episcleritis. To the best of our knowledge and after an extensive internet search, this is probably the first reported case of congenital atrichia with episcleritis. The association may be a co-incidental finding or may have a broader perspective. Hence, further studies are warranted.

Lower ureteric strictures are a common cause for unilateral hydroureteronephrosis, commonly resulting from previous surgeries, weather endoscopic, laproscopic or open. Devascularisation of the ureter resulting in fibrosis forms the underlying pathophysiology of stricture formation. We report a case of ureteral stricture resulting from angioembolization done for a bleeding arteriovenous malformation (AVM) of the uterus.

Survival Pattern of Patients on Maintenance Haemodialysis for End Stage Renal Disease in a Nigerian Dialysis Centre 尼日利亚透析中心终末期肾病患者维持血液透析的生存模式

Background: The increasing incidence and prevalence of chronic kidney disease (CKD) is a serious health challenge globally. The need to examine the longevity of patients with CKD can never be over emphasized. We investigated the survival experience and factors that may contribute to the longevity of chronic kidney disease patient on haemodialysis. Methods: Data for the study were extracted from the record of CKD patient on haemodialysis. Kaplan-Meier survival analysis was done to assess both short- and long-term survival. The impact of six covariates on survival chances were separately investigated using Log-rank test and collectively examined using both Cox and Weibull models. Akaike Information Criterion was then employed for determination of a better model between variables. P-value of <0.05 was considered as statistically significant. Results: The overall median survival time was 182 days. Only 66.3% of all the patients survived their 90th days after starting dialysis and approximately 25% survived to 366 days. The hazard ratios for those patients with family history of chronic kidney disease was 0.45; 95% CI 0.23-0.90 and for those with urinary symptoms was 0.59; 95% CI 0.35-0.99. Model generated imply hi(t)= –5.1499 exp{–0.7850Family His. Of CKDi –0.5353Urinary symptomi}. The age of the patient was also found to be statistically significant when separately investigated. Conclusion: Family history of chronic kidney disease and urinary symptoms were found to influence the survival of patients on haemodialysis. Early identification of at-risk family and prompt treatment of urinary symptoms is advocated.

We investigated the survival experience and factors that may contribute to the longevity of chronic kidney disease patient on haemodialysis. Methods: Data for the study were extracted from the record of CKD patient on haemodialysis. Kaplan-Meier survival analysis was done to assess both short- and long-term survival. The impact of six covariates on survival chances were separately investigated using Log-rank test and collectively examined using both Cox and Weibull models. Akaike Information Criterion was then employed for determination of a better model between variables. P-value of <0.05 was considered as statistically significant. Results: The overall median survival time was 182 days. Only 66.3% of all the patients survived their 90th days after starting dialysis and approximately 25% survived to 366 days. The hazard ratios for those patients with family history of chronic kidney disease was 0.45; 95% CI 0.23-0.90 and for those with urinary symptoms was 0.59; 95% CI 0.35-0.99. Introduction Prostate cancer (PCa) is the second leading cancer in both sexes and the most common cancer in males. Prostate cancer preferentially spreads to regional lymph nodes and bone. Significant metastases to lung, liver or other visceral organs are less common [1]. Patients have a good survival rate if the cancer is diagnosed at an early stage [2]. Generally the patients contact the doctors when the malignancy almost spread in other parts of the body. This is an advanced stage of cancer which is called Metastatic disease. At this stage is incurable and causes significant mortality. Symptoms of advanced prostate cancer are bone pain, spinal cord compression, fatigue, depression, obstruction, weight loss, constipation and anaemia.
