Pain sensation and gut microbiota profiles in older adults with heart failure. 老年心力衰竭患者的痛觉和肠道菌群特征。

Objectives: Patients with heart failure (HF) experience severe pain and may have altered pain sensation; however, the underlying mechanisms of these symptoms are not yet fully understood. Identifying pain sensation and genomic biomarkers of pain in older adults with HF is a critical step toward developing personalized interventions to improve pain management and outcomes. This study aimed to investigate the differences in pain sensation, gut microbiota, self-reported pain, and symptoms in older adults with and without HF.

Methods: Twenty older adults with HF and age-matched healthy controls (HCs) were recruited in the Northeastern United States. Quantitative sensory testing and conditioned pain modulation were performed on the nondominant upper arm to detect the mechanical, thermal, and pressure pain thresholds and pain modulations. Stool samples were collected, and the 16S rRNA V4 gene region of stool samples was sequenced and processed using the Mothur 1.42.3 pipeline. Self-reported pain and symptoms were measured by the Brief Pain Inventory and the NIH Patient-reported Outcomes Measurement Information System. The associations between pain sensation, gut microbiota α-diversity indices, and pain and symptoms were explored using the Spearman correlations.

Results: The HF and HC subjects' mean ages were 73.50 (SD = 8.33) and 67.10 (SD = 7.64), respectively. The HF subjects reported significantly higher pain intensity and interference, sleep disturbance, fatigue, anxiety, and depression than the HCs. The HF subjects also had a significantly lower level of physical function and participation in social roles and activities. Compared with the HCs, the HF subjects had significantly altered conditioned pain modulation heat effect and gut microbiota compositions and predicted metabolic functions. The Statistical Analysis Of Metagenomic Profiles indicated that the HF subjects had a significantly decreased cardiac muscle contraction pathway compared with the HCs. The correlation analysis showed that the quantitative sensory testing profiles and gut microbiota diversity index were significantly associated with pain and symptoms in older adults with HF.

Conclusions: Older adults with HF had more severe self-reported pain and symptoms, altered pain sensation, and different gut microbiota composition and function compared with age-matched HCs. Pain sensation and gut microbiota may contribute to pain and symptoms in older adults with HF and could serve as biomarkers of pain and symptoms of HF. Further research with a larger sample size is warranted to confirm these findings.

Risk factors, theoretical models, and biological mechanisms of nonsuicidal self-injury: a brief review. 非自杀性自伤的危险因素、理论模型和生物学机制综述。

Nonsuicidal self-injury (NSSI) refers to the direct, deliberate infliction of harm to one's body tissue without the intention to die. The prevalence of NSSI has increased significantly globally in recent years and has become an important public health problem affecting the health of people, especially adolescents. The occurrence of NSSI in adolescents is the result of the interaction of different factors. Many scholars have proposed various theoretical models to explain the mechanism of NSSI behavior based on previous research on the influencing factors of NSSI. Moreover, advances have been made in genetic and neuroimaging mechanisms related to NSSI. Understanding the genetic and neuroimaging mechanisms of NSSI is important for both describing and treating the disorder. This literature review discusses the progress made on the risk factors, theoretical models, and biological mechanisms of NSSI.

A review of clinical practice guidelines for preventing chlamydial and gonococcal ophthalmia neonatorum 预防衣原体和淋球菌性新生儿眼炎的临床实践指南综述

Abstract Objective: The objective of this study is to evaluate and analyze clinical practice guidelines pertinent to the prevention of ophthalmia neonatorum in newborns after delivery. The aim is to offer guidance to medical and nursing staff regarding newborn eye care within the clinical context. Methods: WHO, Guidelines International Network, National Institute for Health and Care Excellence, Scottish Intercollegiate Guidelines Network, Queensland Clinical Guideline, Joanna Briggs Institute, Canada Registered Nurses’ Association of Ontario, New Zealand Guidelines Group, and Chinese Medlive, professional societies focusing on newborn care, and bibliographic databases, such as PubMed, Embase, Web of Science, CNKI, WanFang Database, VIP, and SinoMed, were systematically searched. The International Guidelines Evaluation Screening Tool and Appraising Guidelines Research and Evaluation II were utilized to assess the quality of the guidelines. Content analysis methodology was used for summarization, and the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) approach was applied to determine the quality of evidence and the strength of recommendations. Results: A total of 8 guidelines pertaining to neonatal ophthalmia were included in the analysis. Among them, guidelines from the National Institute for Health and Clinical Excellence and WHO obtained higher scores (≥80%). Seven guidelines recommended universal topical ocular prophylaxis. In addition, 2 guidelines recommended prescreening and pretreatment of pregnant women at risk of exposure to chlamydia and gonococcus. In total, 17 recommendations were summarized. While the quality of specific evidence ranged from “very low” to “low,” the majority (12/17) of the recommendation strengths were classified as “strong.” Conclusions: In the clinical context, both universal topical ocular prophylaxis for neonates and screening and treatment of pregnant women and their partners for sexually transmitted diseases are recommended.

Stakeholders’ perceptions and experiences of an outdoor activity project on children myopia prevention: a multiperspective qualitative study 利益相关者对儿童预防近视户外活动项目的认知与体验:多视角定性研究

Abstract Objective: The current investigation sought to examine the experiences and perspectives of the project coordinators, participating parents, and children to identify strategies for enhancing children’s engagement and adherence to outdoor physical activities. Methods: A descriptive, qualitative study was conducted from October 2019 to December 2019 through in-depth interviews with 32 families who participated in the Community Child Myopia Prevention Project, 15 project volunteers, and 8 community organizers. Results: Five themes and 10 subthemes were identified to enhance children’s participation rate and adherence to the project. The 5 themes were as follows: (1) overcoming parental barriers, (2) attracting parental participation, (3) active support from parents, (4) creating a positive experience for children, and (5) integration into children’s daily lives. Conclusions: Empowering parents and children by meeting their needs, creating innovative activities, and connecting them with community social networks support the successful integration of the outdoor activity program into their daily lives. The sharing and integration of community resources through community empowerment can provide a framework for developing future community interventional projects.

Abstract Background and Objective: Reducing the number of falls in nursing facilities is crucial to prevent significant injury, increased costs, and emotional harm. However, current fall detection systems face a trade-off between accuracy and inference speed. This work aimed to develop a novel lightweight fall detection system that can achieve high accuracy and speed while reducing computational cost and model size. Methods: We used convolutional neural networks and the channel-wise dropout and global-local attention module to train a lightweight fall detection model on over 10,000 human fall images from various scenarios. We also applied a channel-based feature augmentation module to enhance the robustness and stability of the model. Results: The proposed model achieved a detection precision of 95.1%, a recall of 93.3%, and a mean average precision of 91.8%. It also had a significantly smaller size of 1.09 million model parameters and a lower computational cost of 0.12 gigaFLOPS than existing methods. It could handle up to 20 cameras, simultaneously with a speed higher than 30 fps. Conclusion: The proposed lightweight model demonstrated excellent performance and practicality for fall detection in real-world settings, which could reduce the working pressure on medical staff and improve nursing efficiency.

A new approach of estimating insertion length for intermittent orogastric tube feeding in adult patients 一种估算成人间歇口胃管喂养插入长度的新方法

Abstract Objective: To determine the association between human external anthropometry and the in vivo length between the incisor and cardia and develop a new approach for estimating the insertion length of the orogastric tube in adult patients. Methods: A cross-sectional study was conducted among participants from the digestive endoscopy department of a tertiary hospital in China. The in vivo length between the incisor and cardia was measured through a digestive endoscopy examination. External anthropometric data were obtained using a weighing scale and a soft ruler. The univariate analysis, correlation analysis, and multiple regression analysis were performed to establish a prediction model of the in vivo length between the incisor and cardia. Results: A total of 327 participants were recruited in this study. The in vivo length between the incisor and cardia ranged from 35 to 46 cm (average 40.57 cm). The highest correlation coefficient was observed for height (r=0.71, P<0.001). Multiple regression analysis showed that weight, sex, height, sitting height, and the nose-umbilicus distance could predict the in vivo length between the incisor and cardia, and the prediction equation was as follows: 6.990−0.066×weight−1.049×sex + 0.124×height + 0.135×sitting height + 0.128×nose-umbilicus distance (sex=0 if the participant was male; sex=1 if the participant was female). Conclusion: The prediction equation for the in vivo length between the incisor and cardia proposed in this study can provide an evidence-based approach for estimating the insertion length of the orogastric tube.

Metabolic signatures and potential biomarkers in the progression of type 2 diabetes mellitus with cognitive impairment patients: a cross-sectional study 2型糖尿病合并认知障碍患者进展中的代谢特征和潜在生物标志物:一项横断面研究

Abstract Background: Type 2 diabetes mellitus (T2DM), a growing global chronic disease, can increase the risk of cognitive impairment. The microbiota-gut-brain axis has a crucial role in the development of neurological pathologies. Therefore, it is necessary to examine host-gut microbiota metabolites associated with diabetic cognitive impairment (DCI) progression. Objective: This study aimed to describe metabolic signatures, identify potential biomarkers in the progression from T2DM to DCI, and analyze the correlation between the potential biomarkers and clinical characteristics. Methods: A cross-sectional study involving 8 patients with T2DM and 8 with DCI was carried out between May 2018 and May 2020. The characteristic clinical data of the patients, such as demographics, hematological parameters, Mini-Mental State Examination, and Montreal Cognitive Assessment, were collected. Metabolomics profiling measured the host-gut microbiota metabolites in the serum. The potential biomarkers were found by getting intersection of the differential host-gut microbiota metabolites from multidimensional statistics (Orthogonal Partial Least Squares-Discriminant Analysis and permutation plot) and univariate statistics (independent-sample t test and Mann-Whitney U test). In addition, we examined the relationship between potential biomarkers and characteristic clinical data using the Spearman correlation coefficient test. Results: A total of 22 potential biomarkers were identified in the T2DM and DCI groups, including 15 upregulated potential biomarkers (such as gluconolactone, 4-hydroxybenzoic acid, and 3-hydroxyphenylacetic acid) and 7 downregulated potential biomarkers (such as benzoic acid, oxoglutaric acid, and rhamnose) in DCI group. Most of the potential biomarkers were associated with clinical characteristics, such as Mini-Mental State Examination, Montreal Cognitive Assessment, and glycated hemoglobin A1c. Conclusion: This study showed that metabolic signatures in the serum were associated with DCI development and clinical severity, providing new ideas for extensive screening and targeted treatment.

Abstract Background: Although several prediction models have been developed to estimate the risk of obstetric anal sphincter injuries (OASIS) among laboring women, none have been used in clinical practice because of controversial or unavailable predictors included in the prediction models and the format used to present them. Thus, it is essential to develop evidence-based prediction models for OASIS using known antenatal and modifiable intrapartum factors and to present them in user-friendly formats. Objective: The objective of this study was to develop evidence-based prediction models for OASIS and a risk calculator to present prediction models. Methods: Models were developed based on a systematic review and meta-analysis in which risk factors for OASIS were identified, and the pooled odds ratio for each risk factor was calculated. A logistic regression model was used to develop the prediction models, and MATLAB with a graphical user interface was used to develop the risk calculator. Results: Two prediction models for OASIS were established: Model I and Model II. Model I included 7 known antenatal variables: maternal age, parity, prior cesarean delivery, prepregnancy body mass index, gestational age, estimated birth weight, and fetal position. Model II added 5 modifiable intrapartum variables to Model I: epidural analgesia, labor induction, labor augmentation, episiotomy, and operative vaginal birth. The risk calculator developed by writing the parameters in the logistic regression models into MATLAB scripts included 2 interfaces, each consisting of risk factors for OASIS and the possibility of OASIS occurring. Conclusions: This study developed 2 prediction models and a risk calculator for OASIS based on a systematic review and meta-analysis. Although the models were more scientific in model development methods and predictors included in the prediction models, they should be externally validated and updated to ensure better performance before they can be widely applied to guide clinical practice.
