Background: The aim of this study was to analyze the demographic, ethyological and clinical aspects of patients with liver cirrhosis treated in the Chronic Liver Disease Center. Methods: a descriptive, cross-sectional and retrospective study based on the analysis of 580 pacients’ medical records with the diagnosis of hepatic cirrhosis treated from 2004 to 2014 in Belém, Brazil. Conclusions: the profile was predominantly of male patients, aged between 51 and 60 years, coming from the capital Belem, whose main etiologies of cirrhosis were C hepatitis and alcohol. Ascites was the most frequent clinical manifestation and Child-Pugh score A was the most prevalent.

Title: Evaluation of risk factors in cardiovascular patients with diagnosis of Systemic Lupus Erythematosus (SLE). Background: prevalence of cardiovascular risk factors in lupic patients.This study aimed to evaluate the presence of cardiovascular risk factors in patients diagnosed with SLE in a Rheumatology Ambulatory, comparing groups with and without SLE nephritis. Methods: a quantitative, observational and cross-sectional study was carried out during the period from November 2016 to June 2017. 86 patients were interviewed using specific protocol. Conclusions: among the cardiovascular risk factors evaluated, systemic hypertension was more frequent in SLE patients (33.7%), followed by smoking (17.4%) and diabetes mellitus (5.8%). There was, however, no difference between the frequency of these comorbidities in the groups with and without lupus nephritis.

Background: The association of thyroid disease and primary hyperparathyroidism is well described, with thyroid carcinoma being reported in 2-15% of cases. The most commonly associated carcinoma is non-medullary thyroid cancer. While the association of PHPT and medullary thyroid cancer (MCT) is well known, that of NMTC, despite its increasing incidence, is still not established. Our study is a review of incidence and underlying mechanisms of non-medullary thyroid cancer associated with PHPT. Also, best imaging tools for concomitant diagnosis is reviewed to ensure an adequate plan of care. Methods & findings: A search was done using two databases: Medline & Embase. The search conducted from the period of 2008 until April 2018 yielded a total of 142 studies. After an adequate screening, 26 studies were reviewed. Incidence of DTC in association with PHPT in the literature ranged between 0.91% and 17.6%. The main histological thyroid malignancy found is micropapillary carcinoma. Despite its less aggressive presentation, these microcarcinomas may grow or develop nodal metastases on follow up. Although bilateral neck exploration with hemi/total thyroidectomy carries the risk of temporary recurrent laryngeal nerve injury or hypoparathyroidism, permanent complications are rare especially when compared to re-do neck surgery. Recently, parathyroid surgeries are going towards minimal invasive procedures, requiring an adequate imaging tool to ensure diagnosis of both diseases. Multiple risk factors for concomitant diseases were hypothesized, the more robust are the common embryologic origin and activation of angiogenic growth factors. Conclusion: NMTC is frequently associated with PHPT especially in endemic goiter areas. With the high prevalence of micropapillary carcinoma and its risks, a partial/total thyroidectomy in addition to parathyroidectomy may be warranted. With the recent need of adequate diagnostic tools, combining both Technetium Sestamibi scintigraphy and thyroid ultrasound improved sensitivities and accuracy of diagnosis, but dual-isotope scintigraphy (I-123 sodium iodide/ 99mTc-sestamibi) seems an attractive modality in hyperparathyroid patients with concomitant suspicious thyroid nodules. Background: Protein supplements are one of the most frequently dietary supplements used by athletes and non-athletes in Saudi Arabia. Objective: To assess the knowledge, attitude and practice of Saudi population toward the protein supplements, examine the relationships of practice with education level, effect of protein on kidneys and lipid profile, assess the types and source of protein supplements, assess the correct use of protein supplements according to exercise intensity and determine protein content and heavy metals in protein supplements. Methods: A national cross-sectional survey was conducted during October-December 2017 on protein supplements users in Saudi Arabia. Moreover, samples of the best selling protein supplements were analyzed to determined total protein and heavy metals content. Results: A total of 398 filled protein supplements user questionnaire. The wrong practice of use associated with the effect of protein supplements on renal function and lipid profile.The percentage of protein content in isolated whey protein (68.18%) whiles by Kjeldhl analysis (80.55%). Concentrated whey protein contain (72%) and by kjeldhl analysis (77.2%). Hydrolyzed collagen containing (100%) protein and by kjeldhl analysis (91.25%).The amount of heavy metals in isolated whey protein contains arsenic (83 μg/kg), mercury (<10 μg/kg) and lead (18 μg/kg), while hydrolyzed collagen sample contains arsenic (101 μg/kg), mercury (<10 μg/kg) and lead (49 μg/kg). Conclusion: The current study reveals an evidence for lacking of the awareness, negative attitude and abuse of protein supplements, demonstrates necessity to further education regarding the use and purchasing of protein supplements to ensure the quality of these products.

Background: The great variability of the extrahepatic bile duct (EBD) has clinical-surgical implications. The objective of this study was to characterize the morphological expressions of EBD. Methods and findings This descriptive study, done by injecting a semi-synthetic (Palatal GP40L 85%; styrene 15%) impregnated with mineral green dye into the gallbladder, to determine the anatomical characteristics and the biometrics of EBD in 33 blocks formed from the supra-mesocolic floor.The gallbladder presented a length of 66.9 ± 1.7 mm. The Hartmann´s Pouch was observed in 16 specimens (50%). The lengths of the cystic duct (CD), common hepatic duct (CHD) and common bile duct (choledoch duct) were 27.8 ± 1.6 mm, 28.6 ± 11.39 mm and 60.6 ± 11.6 mm respectively. The presence of accessory hepatic ducts (AHD) was found in three samples (9.1%). In 29 specimens (87.9%) the cystic duct presented medium length, while in 4 cases (12.1%) the CD was long (P < 0, 05). The trajectory of the lateral oblique of CD was present in 23 cases (69.7%), with statistically significant differences in relation to the other trajectories of the CD (P <0, 05). In 18 samples (55%) the cystic-hepatic union appeared at the level of the middle third of the EBD, while in 15 (45%) cases the union of the CD was low (P<0,05). Conclusions The mathematical distribution of the segments of the EBD, carried out in this study, provides reliability to the assessment of the cystic-hepatic junction level. The presence of CHA and the level of the cysto-hepatic junction are important anatomical references, especially in emergency room procedures.

BACKGROUND: describes clinical aspects of patients with chromoblastomycosis treated at the dermatology department of the Para State University. The present study aimed to describe clinical and therapeutic aspects of patients with chromoblastomycosis treated at the dermatology department of Para State University. METHODS: An observational, descriptive, case series study was performed, comprising 12 patients with the diagnosis of chromoblastomycosis. A clinical interview and a dermatological protocole were applied, classifing these patients in variables as gender, age, time of illness, type of lesion (size, shape, appearance, location and evolution), treatment and complications. Those unable to attend the clinic in person or unable to be represented by someone legally, a Term of Consent for Data Use was applied. CONCLUSIONS: most of the patients were male, rural workers, with vegetative lesions according to the classification of Carrión, graduated as moderate, with less than 10 years of evolution.

Background: The popliteus muscle fulfills a function of rotational stabilizer of the knee. Its particular shape and innervation pattern are the interest of anatomists and surgeons. The aim of this work was characterizing the anatomy and innervation of the popliteus muscle of knees from a sample of mestizo-raced population, predominant in Latin America. Methods: This cross-sectional study was made in 23 knee segments 14 right knees and 9 left knees coming from 14 men and 9 women aged 67 ± 22 years. Results: The popliteus muscle presented a scalene-like triangle shape and its medial base was covered by the superficial expansion of the semimembranosus tendon. The length of its base was 79,2±12,8 mm with a superior and inferior side that measured 58.0±7.4 mm and 101.7±11.7 mm respectively. The popliteus muscle innervation was supplied by the tibial nerve (TN) by the arising of two branches in 22 cases (95.7%). The first branch arose proximal to the inter articular line and presented a length of 91,1 mm. The second branch arose distally to the inter articular line with a length of 48.4±1.3mm. The third was observed in just one case (4.3%) Its length was 108±1.1mm and it arose distally to the inter articular line. Conclusions: The morphological characteristics of the popliteus muscle and its innervation patterns found in our study differ from what was reported in previous studies. The presence of these findings can be considered during the planning of surgical procedures in the posterior aspect of the knee.

The length of its base was 79,2±12,8 mm with a superior and inferior side that measured 58.0±7.4 mm and 101.7±11.7 mm respectively. The popliteus muscle innervation was supplied by the tibial nerve (TN) by the arising of two branches in 22 cases (95.7%). The first branch arose proximal to the inter articular line and presented a length of 91,1 mm. The second branch arose distally to the inter articular line with a length of 48.4±1.3mm. The third was observed in just one case (4.3%) Its length was 108±1.1mm and it arose distally to the inter articular line. \u0000 \u0000Conclusions: The morphological characteristics of the popliteus muscle and its innervation patterns found in our study differ from what was reported in previous studies. INTRODUCTION: During the 20th century, psychiatry began to use the cinema as didactic-pedagogical help resource in the study of psychopathology. The Films of Alfred Hitchcock usually present as main themes mental disorders and the psychoanalysis. However, these films were not created with academic goal and it is natural that there are distortions. OBJECTIVES: To evaluate the importance of Hitchcock films as didactic resource in the study of psychopathology, identify possible distortions and seek to justify them on the basis of the historical context. METHODOLOGY: The abstracts of the 53 feature-length films of the director were read and deleted the movies that did not have the mental disorder as a central theme; they were dumb; or those who had only the antisocial personality disorder. It was performed a narrative review. RESULTS: Six films were selected through an intentional sampling. When he speaks of the Heart (1945) shows a picture of amnesia with dissociative fugue. The psychoanalysis is represented in a very superficial. The Indiscreet (1954) describes a voyeuristic disorder and should not be indicated. The Wrong Man (1956) depicts a severe depressive episode with psychotic symptoms. A body that falls (1958) presents the themes of specific phobia of height and the acute stress disorder. Psychosis (1960) represents a framework of dissociative identity disorder, but the scenes of violence may cause a negative view of the mentally ill. Marnie (1964) has as themes the specific phobia and sexual aversion disorder. The psychoanalysis is presented in distorted form and caricature. CONCLUSION: The films directed by Alfred Hitchcock can be useful in the study of psychiatry and should be considered the distortions that can worsen the stigma.

Suicide has been a growing problem all around the world, with around 1 million deaths registered per year. This scenery affects the medical population more frequently and it has been a concern for more than one hundred years. It has been reported that the work situation to which the physicians are submitted causes distress, burnout and other symptoms that help to develop suicidal ideation. Based on previous works, it was found that among the physicians, surgeons have a higher tendency to be a victim of these disorders, due to work related harassment, sickness presenteeism, unfulfillment feelings and, mainly, the stigma of seeking psychological help. Although, some studies indicates strategies to overcome this situation as regular meetings provided by the work management to discuss stressful work related events and the clarification of suicidal ideation treatment.

The evidences highlight that 13.5% of the echocardiographic studies performed in children with congenital Zika virus infection presented amendments of congenital heart disease (CHD). Several recent studies have shown that the pathogenesis of neurological complications of the Zika virus, as Guillain-Barre Syndrome, in relation to other clinical manifestations there are still no conclusive findings. Scientific studies have observed the possibility of cardiac involvement be underdiagnosed due to the mercy of the infection per ZIKV, in their majority.
