Complementary and Integrative Health Use in Binge Eating Disorder: A Cross-Sectional Mixed-Methods Study of Binge Eating Disorder Experts' Perspectives. 补充和综合健康在暴食症中的应用:暴食症专家观点的横截面混合方法研究。

<p><strong>Introduction: </strong>Binge Eating Disorder (BED) has high lifetime prevalence rates, low treatment success rates, and high rates of treatment dissatisfaction, early discontinuation of care, and recurrence. Complementary and integrative health (CIH) interventions (non-mainstream practices used with conventional approaches for whole-person treatment) hold potential to overcome many treatment barriers and improve BED treatment outcomes. Some CIH interventions have empirical support for use in eating disorders. However, little is known about the current state of CIH use in BED.</p><p><strong>Methods: </strong>This mixed-methods cross-sectional study collected information from BED experts about CIH use in adult BED treatment. Fourteen expert BED researchers and clinicians were identified based on federal funding, PubMed-indexed publications, practice in the field, leadership in professional societies, and/or popular press distinction. Anonymously recorded semi-structured interviews were analyzed by ≥2 investigators using reflexive thematic analysis and quantification.</p><p><strong>Results: </strong>Expert opinions and experiences on/with CIH use were generally positive/supportive (64%) with mixed views (36%) varying by intervention and empirical support. The interventions most commonly described were mindfulness (71%), yoga (64%), and supplements/vitamins/pre-/probiotics/herbs (64%). Supplements/vitamins/pre-/probiotics/herbs had mixed views; all other interventions were generally viewed positively. The benefits most commonly associated with specific interventions (e.g., mindfulness, yoga, supplements) were: regulating/tolerating emotions/mood/stress/anxiety (50%); healing the relationship with the body/body image/movement/exercise-trauma (29%); biological/physiological benefits (29%); and directly supporting treatment ("space for self-separate from treatment," behavior change, "tolerating treatment") (29%). Intrinsic self-healing (e.g., patient-driven healing that comes from the patient's innate desire to heal based on the patient's lived experience(s)) and investigative research were also associated with CIH use broadly. Most experts (57%) expressed familiarity with existing literature/research for ≥1 CIH intervention; 50% identified a need for empirical testing. Half (50%) spontaneously described using ≥1 CIH intervention in their own clinical practice/center. The most used interventions were yoga (43%), meditation/mindfulness (29%), and acupuncture (21%). Eight experts (57%) endorsed the importance of correct implementation; 43% acknowledged CIH use in conventional treatments (2nd-wave CBT, 3rd-wave behavior therapies).</p><p><strong>Discussion & conclusions: </strong>CIH interventions can complement current BED treatments to improve clinical outcomes, particularly managing anxiety/stress/mood, healing the relationship with the body, addressing biological/physiological deficiencies, and tolerating treatment (thus reducing treatment d

Effects of Bed Height on Balance during Ingress and Egress from a Hospital Bed. 病床高度对进出病床时平衡性的影响。

Background: Patient falls from hospital beds are a major safety concern in hospitals, with bed ingress and egress alone responsible for most falls. Despite previous insights, no research has systematically assessed how bed height affects balance during ingress and egress.

Aims & objectives: Our study aimed to quantify individuals' balance during hospital bed ingress and egress at different heights by analyzing the components of ground reaction forces.

Methods: Based on the experiment conducted in a previous study, ground reaction forces were collected from twenty-four healthy adults during ingress and egress from a hospital bed at different heights. Component force vectors for each foot were evaluated, and data was statistically analyzed using a two-factor Analysis of Variance (ANOVA) with follow-up post-hoc Tukey tests.

Results: Significant imbalance was found in the medial-lateral direction for both feet at all bed heights during ingress and egress trials. A more balanced approach between the two feet was observed for the longitudinal direction, with mid-range heights demonstrating more optimal ground reaction forces.

Conclusion: Our results revealed that individuals had better balance in the longitudinal direction for medium bed heights (51 to 66 cm) during ingress and egress. We provided further insights into the specific balance impairment during ingress and egress trials, indicating some left-right feet differences. In summary, medium bed heights (51 to 66 cm) resulted in the most stable balance with fewer differences between the right and left feet, suggesting that low and high bed heights may place patients at a higher risk of falling.

Background: Nurse leaders influence policy, change, and outcomes on the part of patients and junior nurses, and they are critical in resolving conflicts and in decision making. A clear understanding of whether high qualifications or experience impact more on a nurse’s leader’s outcomes is necessary. Aims: To examine whether an advanced practice registered nurse with higher qualifications is considered by themselves and by junior colleagues to have better position outcomes than one who is in the same job level but who has lesser experience and less qualifications. Methods: This review followed the methodological framework for systematic reviews described in the PRISMA statement and PRISMA checklist and the Cochrane Collaboration procedures to review quantitative studies published between 1t January 2015 and 31 December 2023. Results : This review included thirteen articles after screening articles from six different databases. The settings for the studies included a private hospital (n=1, Turkey), medical centers at University of California (n=1, USA), general hospital setup (n=3; Egypt, Taiwan, Ethiopia), trauma hospital (n=1; Iran) and clinics (n=2, Sweden, South Africa), ICU (3 Saudi Arabia, Sweden, Ethiopia). In terms of study designs, all the studies were interventional with five described as quasi-experimental studies (pre-test and post-test studies designs) and 8 Cross-sectional study design studies (completed through surveys questionnaire). Key findings highlighted the impact of nurse academic qualification and experience on position outcomes. Conclusion: Quality of nursing care is dependable on nurse staff qualification as nurses with higher academic qualifications have better position outcome than those with inferior qualifications.

Fall Prevention Knowledge, Attitudes, and Practices among Nurses in Saudi Arabia 沙特阿拉伯护士的防跌倒知识、态度和做法

Objectives: To comprehensively assess the knowledge, attitudes, and practices related to fall prevention among nurses in multiple healthcare settings, emphasizing the crucial role of preventing falls in ensuring patient safety and well-being. Methods: A correlational cross-sectional design, involving the recruitment of participants through convenience sampling from sixteen government hospitals in Hail, Saudi Arabia. Data on nurses’ knowledge, attitudes, and practices regarding falls were collected using a self-reported questionnaire. The findings are reported using descriptive and inferential statistics. Results: The analysis included 295 participants. The knowledge scores ranged from 0 to 14, with a mean of 8.54, the attitude scores ranged from 19 to 54, with a mean of 39.50, and the practice score ranged from 25 to 65, with a mean of 57.02. Significant differences existed in knowledge scores among hospitals (p < .001), while attitude and practice scores were statistically significant for multiple factors, including gender, place of work, experience of falling accidents, and the fall prevention training (frequency, duration, source) (all p < .001). Conclusion: This study reveals that nurses generally possess moderate knowledge and positive attitudes towards fall prevention, with high reported adherence to preventive measures. However, variations among hospitals and the influence of factors like gender, workplace, experience with falls, and training characteristics emphasize the importance of tailored training programs to enhance patient safety.

Respiratory Repercussions Due to Mandibular Advancement in Children and Adolescents with Mandibular Class II Malocclusion: A Retrospective Longitudinal Study 儿童和青少年下颌骨II类错颌的呼吸反应:一项回顾性纵向研究

Abstract To test the improvement in airway patency after mandibular advancement with functional appliances as a treatment for growing patients with skeletal Class II caused by mandibular retrognathism. Determining which functional mandibular advancement appliance has the greatest effect on the augmentation of the upper and lower pharyngeal airways. A series of measurements were made according to Arnett’s analysis in the lateral teleradiographies of the skull using the Nemoceph program to evaluate how the upper and lower airways are modified at sagittal level pre- and post-treatment, with different types of mandibular advancement appliances and in growing patients with age ranges from 8 to 15 years. There are statistically significant differences in craniometric points A, MCI and Pg in contrast to other studies with the same cephalometric analysis, where craniometric point B presented a greater increase of mm after advancement. There is an improvement in mandibular Class II skeletal malocclusion after the use of mandibular advancement appliances, and more changes are observed in the A, MCI and Pg points, confirming improvement in upper air patency.
