Case Report: Congenital Pseudoacorea in an Ocular Axenfeld-Rieger Syndrome: What is it? 眼部Axenfeld-Rieger综合征的先天性假橡子:是什么?

Summary: This is a descriptive summary of the case of a patient with Axenfeld-Rieger syndrome associated with a congenital malformation of the iris and consequent pupillary morphological alteration of an atypical characteristic reported. This anomaly is unique in scientific literature and exhibits a peculiarity that we have called pseudoacorea: Hidden pupil. Other associated abnormal clinical findings were posterior embryotoxon, astigmatism, amblyopia, and exotropia. Diagnosis was achieved by instilling ocular mydriatics into the cul-de-sac that revealed this peculiarity. It is necessary to make a differential diagnosis with other pupillary pathologies such as corectopia, acorea and microcoria. Early detection of pathology and surgical management is necessary, since it would lead to a better visual prognosis for both amblyopia and strabismus.

Background: Among the malformations of the pupil, we can find polycoria (more than one pupil), dyscoria (abnormal pupil shape), corectopia (abnormal pupil position) and acorea (absence of pupil). In addition, morphologically normal pupils can denote other anomalies such as the microcoria described by Holth in 1923. Acorea is a rare anomaly, congenital or acquired, characterized by an absolute absence of the pupil both at rest and in mydriasis. In our case we prefer to differentiate it and name it pseudoacorea, since although there is a total absence of the pupil at rest thanks to the application of ocular mydriatics, a micropupil with discoric and corectopic characteristics is achieved. It is worth noting that we have not detected in the scientific literature any case described as the one that we will develop here.

Conclusion: The case of a patient with Axenfeld-Rieger syndrome associated with a congenital malformation of the iris and consequent atypical pupillary morphological alteration is presented. This anomaly is unique in the scientific literature and presents a peculiarity that we have called pseudoacorea: Hidden pupil. Early detection of pathology and surgical management is necessary, since it would lead to a better visual prognosis for both amblyopia and strabismus.

Monkeypox virus (MPXV) is a zoonotic disease, with endemic circulation and it’s not considered a sexually transmitted infection. Recently, it has been reported a particularly high incidence among men who have sex with men (MSM). We report the first case of a male HIV-negative patient with HIV Pre-Exposition Prophylaxis (PrEP), and a simultaneous coinfection of Monkeypox Virus, COVID-19 and Syphilis.

Association between Provider-Patient Relationship, Communication, Accessibility Convenience, and Perceived Quality of Care from Patients Living with HIV before and during SARS-Cov-2 Pandemic 在SARS-Cov-2大流行之前和期间,艾滋病毒感染者的医患关系、沟通、无障碍便利性和感知护理质量之间的关系

OBJECTIVE The study aims to determine the risk factors for endometrial hyperplasia and endometrial cancer in patient with abnormal uterine bleeding and thus early diagnostic investigations to detect the disease and limit its development. DESIGN Primary data were collected from an infectious disease clinic outpatient setting using the PSQ-18 short form. The data were collected between February and March 2021. The study population included (a) non-institutionalized individuals, (b) individuals aged 18 years and older, (c) individuals living with HIV/AIDS, and (d) individuals who had had a provider visit in the past 12 months at an infectious disease clinic. METHODS Multiple regression was used to assess the relationship between the dependent and independent variables using a cross-sectional quantitative analysis. RESULTS The study revealed a statistically significant relationship between PPR and perceived quality of care from the perspective of patients living with HIV before and during the SARS-CoV-2 pandemic. CONCLUSION The study results indicated that overall provider-patient relationship (PPR), communication, accessibility, and convenience were related to perceived quality of care. The study findings also revealed that PPRs can uniquely impact perceived quality of care. Provider accessibility was also found to uniquely impact a patient’s perceived quality of care. Finally, the study results demonstrated that HIV patients who have a good relationship with healthcare providers and a high level of satisfaction tend to perceive high-quality healthcare.

Non-Pharmacological Interventions for the Hallucinations in Patients with Dementia. A Cross-Over Randomized Controlled Trial. 痴呆患者幻觉的非药物干预。交叉随机对照试验。

Objective: Hallucinations is a core characteristic symptom in Lewy Body Dementia (DLB) and Parkinson's Dementia (PDD). It may also appear at the late stages of Alzheimer's disease (AD). They are not easily managed, and they are associated with cognitive decline, earlier institutionalization, increased mortality, and increased caregivers' burden.

Design/setting: This is a cross-over RCT. The participants were randomly assigned in 6 different groups of 10 patients each.

Participants: 60 dementia patients (different types and stages of dementia).

Interventions: The three non-pharmacological interventions used are: A) Validation therapy (VT) in a Psycho-educational program, B) Reminiscence therapy (RT) and C) Music therapy (MT). Each intervention lasted for 5 days and there was an interval of two days, as a wash-out period (all the interventions had a duration of 3 weeks).

Measurements: The measurements which were used at baseline were: MMSE, ACE-R, GDS, FRSDD and NPI questionnaire at baseline and after each intervention.

Results: The most effective combination for the reduction of the hallucinations is: VT/Psycho-educational program (p = 0.005), MT (p = 0.007) and RT (p = 0.022). The same combination applies for the caregivers' distress: VT/Psychoeducational program (p = 0.010) - MT (p = 0.023) - RT (p = 0.036).

Conclusion: VT/Psycho-educational program followed by MT, followed by RT is an effective combination of non-pharmacological interventions that can reduce hallucinations in patients with dementia and caregivers' burden. Non-pharmacological interventions should be further examined as an effective alternative for the reduction of the hallucinations in dementia.

Cardiac Tumor Masquerading as ST Elevation Myocardial Infarction: A Case Report and Literature Review. 心脏肿瘤伪装成ST段抬高型心肌梗死1例报告及文献复习。

Cardiac tumors are rare, with primary tumors much rarer than secondary. They can present with a variety of symptoms, including cardiogenic shock, arrhythmias, tamponade, and systemic embolism. There have been cases reported of patients having cardiac tumors presenting with ST elevations. While the exact pathophysiological mechanism for ST changes in patient with tumors is not known, proposed theories include tumor emboli to coronary artery, external compression of coronary arteries, stretching of cardiac muscle fibers, inflammatory reactions, and electrolyte transfer from necrotic tumor tissue to adjacent myocardium. We present a case in which the patient had no prior history of malignancy that are presented with cough, shortness of breath, lower extremity edema, ST elevation on electrocardiogram, and was found to have epithelioid tumor in his left ventricle. This case raises awareness of wide differential for ST changes on electrocardiogram besides myocardial infarction, especially in patients who do not present with classic ischemic symptoms.

While the exact pathophysiological mechanism for ST changes in patient with tumors is not known, proposed theories include tumor emboli to coronary artery, external compression of coronary arteries, stretching of cardiac muscle fibers, inflammatory reactions, and electrolyte transfer from necrotic tumor tissue to adjacent myocardium. We present a case in which the patient had no prior history of malignancy that are presented with cough, shortness of breath, lower extremity edema, ST elevation on electrocardiogram, and was found to have epithelioid tumor in his left ventricle. The COVID-19 quarantine is creating a threat to the intellectually disabled community. There is a need for community outings to sustain the psychological wellbeing of the intellectually disabled. This is why their caregivers include community outings in their care. However, during the COVID-19 quarantine this is suddenly missing in their lives. Therefore, this is causing an increase in psychosis for the intellectually disabled. Mental health professionals must proceed with caution when providing medication to ensure the holistic wellbeing of the patients is met.

Private school teachers among Higher Education Institutions (HEIs) continue to strive to sustain quality education despite the psychological impact of the COVID-19 crisis. This study aimed to assess the emotions, stressors, coping strategies, and motivational factors of private HEI's teachers in Southern Philippines. Both negative and positive teachers' emotions were observed. Low salary scheme was the primary teachers' stressor. But, teachers utilized helpful strategies to cope with stress. Several motivational factors were seen as significant steps for their institutions to implement and follow. The high level of stressors of teachers during the pandemic influences the emotions towards the community and work situations. As they increasingly practice varied coping strategies and augment motivation to continue to work despite the situation, the better emotional state with lesser adverse feelings they will have. Government support is needed to aid private schools to continue the quality delivery of higher education. It is necessary to address stress-related problems of teachers during the pandemic, and help them strengthen their coping practices and motivation at work.

Pandemic Control COVID 19: The Results of Analytical Studies of Ten Health Systems in the Word. Scopus Review COVID - 19大流行控制:世界十大卫生系统分析研究结果斯高帕斯审查

OBJECTIVES Description of the epidemiological situation of COVID-19 and analyze and compare between ten-health systems control epidemic around the word for use in public health system settings. METHODS This study was a descriptive, exploratory analysis of all cases of COVID-19 in the world especially in the ten countries. We conducted a Scopus review of the literature. Thirty articles are been recolted. RESULTS The estimates of human transmissibility (R0) range from 2 to 3. Pre-symptomatic transmission has been reported. The Incubation period is 5.1 days; the overall global case fatality rate is currently approximately 6%. The countries are characterized into three categories of pandemic control: excellent level of control fo r Germany, South Korea, Hong Kong and turkey a medium level for China, Morocco and a low level for the USA, France, Spain and Italy. CONCLUSION The present descriptive, exploratory analysis offers important new information to the international community on the Corona virus Pandemic in world. Important questions remain including determination of infectiousness period, identification of transmission routes, and effective treatment and prevention methods including further test development, drug development, and vaccine development.

The immune system protects the human body from pathogens like viruses and bacteria. People need to maintain a strong immune system during the COVID-19 pandemic. Unfortunately, the fearmongering news reports on death rates increase fear. This could become a health risk because fear could weaken the immune system by in creasing the release of corticosteroid in the body. Modern medicine includes the mind, body, and soul strengthening the immune system.
