Background: Esophageal achalasia is an esophageal motility disorder of unknown etiology that results in impaired lower esophageal sphincter relaxation and loss of esophageal peristalsis. The aim of our study was to show our experience in the management of esophageal achalasia in a case treated by laparotomy in our service. Case Report: 21-year-old patient, student, admitted to our department for progressive dysphagia to solids accompanied by regurgitation, epigastric pain, nocturnal cough and weight loss, evolving for 5 years. ATCD of pulmonary tuberculosis. The physical examination noted a deterioration in general condition, stage (I) of the WHO; there was hypo coloration of the integuments and conjunctivae, TA=120/80mmHg, Pulse=70/min. The abdomen was symmetrical with presence of sluggish skin folds, supple and painless without a palpable mass, the digital rectal examination did not present any particularity. The thorax was symmetrical with a FR=20 c/mn. The biology showed a normo chrome and normocytic anemia of 8 g/l Group O+. The esogastroduodenal transit was in favor of an amputation (tight stenosis) extended on the cardia with megaesophagus upstream. We performed a cardiomyotomy according to Heller associated with a hemivalve according to Toupet. We performed a followed by preesophageal and subhepatic drainage. The postoperative course was simple, marked by a resumption of food on D6 postoperatively. The control test carried out with gastrographine 10 days after the surgical intervention noted a regression of the signs with passage of the product of contrast in the small intestine. The patient seen again 6 months after the operation showed no particularities. Conclusion: Achalasia is a chronic esophageal motility disorder characterized by the inability of the lower esophageal sphincter to relax. Heller cardiomyotomy by laparotomy associated with Toupet-type fundoplication retains its place in its management in our exercise context.

An inguinal hernia is a bulge or protrusion that is seen or felt in the groin area or the scrotum (for boys). Inguinal hernias occur in boys more frequently than in girls. Infants who are born prematurely are at an increased risk of having an inguinal hernia. Incarceration of hernia is a major problem in neonates, it causes significant pain and the content of this hernia is the intestine. If not diagnosed early it causes a cut in the blood supply and the infant became very sick. Treatment of inguinal hernia is surgical under general anesthesia, but if an inguinal hernia is present in preterm the surgical will be done before discharge. Bilateral inguinal hernia in females must be investigated very carefully, maybe ovaries are present in inguinal hernia.

Spirometry is a lung function test, whose main objective is to evaluate lung mechanics. It is a test that is currently easily accessible, but little used by anesthesiologists. One cause may be that they are not familiar with the analysis and understanding of said test or the lack of a pulmonary physiology laboratory in their hospitals. The proper analysis and understanding of this test by the general and thoracic anesthetist offers a range of possibilities both for the perioperative evaluation of the patient with pulmonary pathology or the one who will undergo thoracic surgery. Having a great impact by influencing the prognosis and management of these patients.

Gingival Plasma Cell Granuloma (GPCG) is a rare benign lesion, which is considered within the so-called pseudotumors. In other specialties it is known as plasma cell granuloma. Histologically characterized by dense inflammatory cells infiltrates, mainly plasma cells. This manuscript presents the case of a 75 year-old woman, with a tumor in the upper alveolar region of the oral cavity. The final diagnosis corresponds to GGCP. Successfully treated with surgery, without recurrences. It is important to know this pathological entity to differentiate it form other oral cavity tumors with malignant behavior.

FAS is associated with a traumatic event, especially neurologic injury. The literature suggests FAS is commonly associated with cerebrovascular accidents, though other precipitating neurological conditions are documented. To our knowledge, there is no reported case of FAS associated with cardiac arrest. Here we report a case in which a patient local to Louisiana developed a change in speech resembling a Haitian accent secondary to cardiac arrest.

Background: Many treatment options exist for the management of rotator cuff tears, varying from non-operative-benign neglect to complex arthroscopic partial repair using an interval slide technique. Although satisfactory results may be obtained with many of these options, it should be remembered that few prospective, randomized trials exist comparing these treatments. Thus, controversy remains as to the absolute indications for each, and individual patient factors may significantly affect the final treatment decision. Methods: Hospital-based observational study. Informed consent was taken from the patients. A total of 30 patients who got admitted to the Department of Orthopaedics, Government General Hospital, Kakinada from November 2018 to September 2020 were included in the study. Results: Twenty five cases showed excellent results four cases showed good results and one showed poor result using University of California LosAngeles shoulder scale. Twenty nine cases showed excellent results and one case showed good result with Constant Murley Score. Conclusion: In the era of arthroscopic rotator cuff surgery the mini open technique with open acromio plasty and rotator cuff repair adds to rural surgeon as an elective procedure and can produce clinically competing results that of other contemporary techniques.

Spinal neurostimulation is a promising approach for treating spinal lesions and has implications in various neurological disorders. It promotes axonal regeneration and neuronal plasticity to reestablish disrupted signal transduction pathways following spinal injuries or degeneration. This paper reviews the current technology and its differing utilities in various types of neurostimulation, including invasive and noninvasive methods. The paper also explores the efficacy of spinal compression and decompression therapy, with a primary focus on degenerative spinal disorders. Moreover, the potential of spinal neurostimulation in therapies for motor disorders, such as Parkinson's disease and demyelinating disorders, is discussed. Finally, the paper examines the changing guidelines of use for spinal neurostimulation following surgical tumor resection. The review suggests that spinal neurostimulation is a promising therapy for axonal regeneration in spinal lesions. This paper concludes that future research should focus on the long-term effects and safety of these existing technologies, optimizing the use of spinal neurostimulation to enhance recovery and exploring its potential for other neurological disorders.

Clinical presentation may vary in different patients and are nonspecific. Treatment is usually surgical intervention once diagnosed. We have presented one such case of a patient whose Computed Tomography (CT) scan showed a large aneurysm of descending branch of pulmonary artery.

Snake bite is often met emergency in Indian subcontinent especially in rural settings. We sent a case of a 26-year-old male, residing in North India who sented with complaints of swelling over left leg along with breathlessness. His renal functions deteriorated gradually. In view of various physical and laboratory findings, a diagnosis of vasculotoxic snake bite envenomation with acute kidney injury and pulmonary oedema was made. He was managed conservatively with IV fluids, parenteral antibiotics and haemodialysis. Acute kidney injury is an important complication in vasculotoxic snake bites and should be addressed early.
