Quadriceps tendon rupture is an uncommon injury but may result in long term disability if not adequately repaired. Many techniques are described for repair of acute quadriceps tendon rupture, including tendon-totendon repair, trans-osseous tunnels, synthetic augmentation, tendon plasty and the use of suture anchors. There is no single accepted surgical treatment. This study's objective was to assess the efficacy of a double-crossed suture anchor repair in the management of quadriceps tendon rupture. Materials and methods 85 patient attended our institute for surgical management of quadriceps tendon rupture over eight years (2012-2019). Twenty patients were treated with the use of a double-crossed suture anchor fixation. These patients were allowed to weight bear in a hinged knee brace for six weeks following surgery. Eighteen out of twenty patients had one or more predisposing comorbidities, including obesity, diabetes, renal failure, quinolone and steroid use. Results Clinical and functional outcomes were recorded during follow up visits prospectively for a mean of one year (10-14 months). The mean knee flexion was 124 degree (120-130). This retrospective study was conducted to evaluate the clinico-pathological pattern of nodular goiter among patients who underwent thyroidectomy. This study included 50 patients following selection criteria. All patients were properly evaluated by history, clinical examination along with blood chemistry, ultrasonography, nuclear scanning and histopathology [fine needle aspiration cytology (FNAC)]. Out of 50 patients 38 were females and 12 were males, the male to female ratio was 1:3.12. Majority of the patients nodule and rest 3 cases were carcinoma. Among total study patients 46 were cold nodules and 4 were warm nodules. Histopathological reports of excised tissues revealed that 45 cases were papillary adenoma, 3 cases were papillary carcinoma and 2 cases were follicular carcinoma. The study concluded that solitary thyroid nodules are frequent at the middle age and predominant among females. Most of the patients of thyroid goiter are in euthyroid state and majority of nodules are benign lesions. Papillary adenoma is the most frequent benign nodular tumor followed by papillary carcinoma and then follicular carcinoma.

Introduction: This study aimed to characterize and compare the fecal microbial profiles between post pull-through Hirschsprung disease patients and healthy children aged younger than five years. Method: Fresh fecal samples were collected from 10 post pull-through Hirschsprung disease patients and age range-matched 10 healthy children. Bacterial DNA obtained from fecal samples were analyzed using 16S rRNA Illumina MiSeq platform. Results: Our findings demonstrated the significantly increased Firmicutes in Hirschsprung disease group compared to the healthy group (corrected p-value=0.007) at the phylum level. In addition, the Firmicutes/ Bacteroidetes ratio in Hirschsprung disease group was 4.8 times higher than that of its control counterpart. Bacilli were also significantly increased (corrected p-value=0.004), while Erysipelotrichi and Actinobacteria were significantly decreased in Hirschsprung disease patients (corrected p-value=0.04 and 0.03, respectively) at the class level. Moreover, functional analysis demonstrated that 20 enzymes and 18 Kyoto Encyclopedia of Genes and Genomes pathways were significantly different between groups (corrected p-value<0.01). Conclusion: A distinct dysbiosis even when the aganglionic segment had already been removed was remarkably evident in young children with Hirschsprung disease, with a significant increase in Firmicutes and a nearly five-fold increase in proportion of Firmicutes/Bacteroidetes which may potentially be employed as the dysbiosis-related biological indicator.

Appendicitis is the most common cause of surgical abdomen. Delay in the diagnosis and appropriate management may lead to potentially life-threatening complications. Bilirubin is associated with the assessment of the severity of appendicitis. The objective of the study is to determine the role of hyperbilirubinemia in predicting appendiceal perforation. An online search was conducted in PubMed, Google Scholar, and PubMed Central to find relevant publications relating to hyperbilirubinemia in perforated appendicitis. Only studies conducted on humans were included. Out of the 1287 articles found, only seven were included in our final review. Of the seven articles included in the review, hyperbilirubinemia is confirmed as a useful predictor for perforated appendicitis or a marker of the severity of appendicitis. Preoperative bilirubin levels can be used as a diagnostic tool in perforated.

Primary Malignant Brain Tumor Trends 2001- 2017: An Alarming Increase In Glioblastoma. Journal of Surgery and Research 5 (2022): 284-295. Abstract Introduction Primary Brain tumors which include a 1.7 percent of all cancers among Data was obtained from Population-based according to WHO 2007 classification of CNS tumors was used for grouping the tumors. Using SPSS, Total Growth Rate (TGR) and Incidence Rate (IR) were calculated to assess the changes overtime. Results A total number of 2130 (male: 890, 41.8% and female: 1240, 58.2%) brain tumor cases from 2001 to 2017 were included in this study. The highest Incidence Rate (IR) was found in ‘unspecified tumors of the brain’ (2.74 per 100000) between 2011 and 2015 followed by ‘Astrocytoma’ (1.34 per 100000) between 2006 and 2010) and ‘Glioblastoma’ (1.25 per 100000) between years 2011 and 2016. Additionally, the Glioblastoma showed the highest IR in those aged between 30 and 59 of years (0.42 per 100000). An upward trend in the rate of brain tumors in the province could be an for more studies to be done especially from the point of view.

Wilms tumor is the commonest childhood tumor. Extra renal Wilms' tumor is a nephroblastoma arising in locations other than kidneys. The tumor arises from misplaced metanephric blastema or is contemplated to arise from pluriotent cells. A metstasis, origin of tumor in kidneys and supernumery kidneys have to be ruled out to assign a diagnosis of extrarenal Wilms' tumor. It has been commonly reported to occur in retroperitoneum but also in inguinal region, pelvis, testis and colon. The age of occurence has been reported as Wilms; tumor in literature. It is mostly diagnosed post operatively. A preoperative biopsy combined with immunohistochemistry can help in diagnosing tumors in cases where neoadjuvant chemotherapy is anticipated. The WIlms'tumors in extrarenal location largely have favourable histology and are treated on same guidelines as Wilms' tumor. Here we present a case of extra renal Wilms' tumor arising in left lobe of liver. Liver as location for an extra renal Wilms' tumor was not found in literature.

We examined characteristics trends in early breast cancer patients receiving neoadjuvant chemotherapy (NAC) over a 16-year period. Our primary objective was to analyze variations in tumor stage and subtype over time. Secondary objectives included analyses of type of surgery and pathological response, from January 2005 to May 2021, 1623 patients receiving NAC were identified. Three periods were determined: 2005-2009 (P1), 2010-2014 (P2), 2015-2021 (P3). Correlations between periods and patient features with cT stage, pathological breast and axillary node response, pathological complete response (pCR), and type of surgery were assessed in univariate and multivariate analyses. We observed a significant increase in cT0-1 and N0 stages with periods (from 6.8% at P1 to 21.2% at P3, and from 43.2% at P1 to 55.9% at P3, respectively) and in the proportion of HER2+ and triple negative (TN) subtypes. In a multivariate analysis, a decrease of cT2-3-4 tumors during P3 was observed for HER2+ (OR:0.174; p=0.004) and TN tumors (OR:0.287; p=0.042). In-breast pCR and pCR were observed in 40.8% and 34.4% of all patients, respectively, with strong association with tumor subtypes, but not with tumor size in multivariate analysis (37.0% pCR for cT0-1 tumors, 36.4% for cT2 tumors, 29.1% for cT3 tumors (cT0-1 versus cT≥2; p=0.222)). pCR was negatively associated with cN1 stage (OR:1.499; p<0.001 for cN1 patients compared to cN0). We observed an increase in the proportion of small cT0-1 and N0 stages treated with NAC, especially in HER2+ and TN subtypes. No significant impact of tumor size on pCR rates was found.
