Chronic constipation is a common condition seen in pediatrics. In recent years, new diagnostic and treatment measures have been applied, motivating the Asociación Mexicana de Gastroenterología to bring together a panel of national and international experts, for the aim of unifying concepts, providing recommendations on the use of complementary studies, and proposing the most appropriate treatment. A systematic search of the literature in English and Spanish was carried out for each statement formulated, utilizing Medline/PubMed, Cochrane Database, EMBASE (Ovid), and LILACS. The Delphi method was used for developing the consensus. Ten questions and 43 statements were discussed, establishing a recommendation grade and evaluating the quality of evidence. Different concepts, diagnostic methods, and pharmacologic and surgical therapeutic measures were discussed, and following three voting rounds, the recommendations based on current clinical evidence were established.

Introduction and aim
Celiac disease (CD) is an autoimmune enteropathy triggered by ingested gluten, with intra- and extra-gastrointestinal symptoms. Individuals with Down syndrome (DS) have a higher prevalence of autoimmune disorders, including CD. This study aimed to evaluate genetic predisposition (HLA haplotypes) and serological markers of CD in individuals with DS, from Northwestern Mexico.
Methods
Eighty-six participants with DS, 3–64 years of age, were included in the study. We assessed HLA-DQ2 and DQ8 haplotypes by duplex PCR and related symptoms, as well as IgG and IgA anti-gliadin antibodies, and IgA anti-tissue transglutaminase antibodies by an enzymatic immunoassay.
Results
Most participants (98.8%) carried risk alleles, with 52.9% having HLA-DQ2 and 45.9% HLA-DQ8. IgG anti-gliadin antibodies were positive in 8.1% of participants. In comparison, 6.9% were positive for IgA anti-gliadin antibodies, and 9.3% had a positive index for IgA anti-tissue transglutaminase antibodies. Four participants (4.6%) presented full or partial HLA-DQ2/DQ8 haplotypes, positive indexes for IgA anti-gliadin, and anti-transglutaminase antibodies related to CD.
Conclusions
The study reveals an almost complete genetic predisposition to autoimmunity and a very high risk of CD in individuals with DS. Enhanced diagnostic protocols and ongoing monitoring are recommended to improve the management of CD in this vulnerable population.

Hepatocellular carcinoma is the most frequent primary malignant tumor of the liver. In Mexico, it is one of the main causes of death. It tends to be detected at late stages of the disease, making its prognosis poor in most cases. It has a wide range of clinical presentation and histologic subtypes, which can also have prognostic implications. We conducted an autopsy study series, in which we identified a total of 34 cases of hepatocellular carcinoma over a 6-year period, accounting for 1.37% of all the postmortem studies. The disease was slightly more frequent in men, with a 1.42:1 ratio, and in the sixth decade of life. The conventional subtype was more frequent, and the main cause of death was hypovolemic shock due to gastrointestinal bleeding. A striking datum was that only 38.2% of the cases were diagnosed before the autopsy report, underlining the need to improve protocols for the early detection of hepatocellular carcinoma.

Cirrhosis of the liver is a major cause of death worldwide, with a recent increase in cases secondary to metabolic dysfunction-associated steatotic liver disease and alcohol use. Liver transplantation is currently the only definitive treatment but requires immunosuppression for preventing graft rejection.
Calcineurin inhibitors are the cornerstone of immunosuppression in transplanted patients. However, their use conditions metabolic and renal complications, such as metabolic syndrome, post-transplant diabetes, and chronic kidney disease. Minimizing or discontinuing immunosuppression has been shown to reduce those adverse effects in selected patients, especially those with potential immunotolerance. Predictors of immunotolerance and cardiovascular complications in transplanted patients have been identified in recent studies. Thus, there is a need for developing individualized strategies for balancing metabolic risk and graft rejection prevention.
