[Infection after dental intervention. Iatrogenic or general medical cause? Case report]. 牙科干预后感染。医源性还是一般医学原因?病例报告)。

Whenever a dentist is dealing with abscess formation in the oral and maxillofacial region, it is mostly from dental origins. However, sometimes uncommon (co-)factors are present and responsible for major complications. Many general conditions or medications can significantly influence the course of an inflammation. It might spread faster and wider and also be resistant to "correct" therapy. This case report should raise awareness about general conditions supporting inflammation and demonstrate the importance of interdisciplinary treatment in these situations. A 76-year-old patient was referred to the maxillofacial surgery clinic after extraction of two teeth resulted in therapy-resistant painful swelling. Her dentist already had initiated "standard" therapy including Ponstan® (mefenamic acid) and Clamoxyl® (amoxicillin) without success. Initial blood testing came back with severe agranulocytosis. Immediately all potentially myelosuppressing drugs were stopped while myelosupporting drugs were prescribed. Under close interdisciplinary treatment conditions, healing was then uneventful without the necessity of surgical intervention. The challenge in inflammation treatment is to identify patients with uncommonly severe, fast-progressing, or therapy-resistant disease as early as possible. Further examination including blood workup for several medical parameters is indispensable in those patients.

Inhibition of enamel erosion by stannous and fluoride containing rinsing solutions. 含锡和氟的冲洗溶液对釉质侵蚀的抑制作用。

This in vitro study investigated the erosion-inhibiting properties of dental rinses during erosion in the presence of the salivary pellicle. The erosion inhibition by a Sn/F containing dental rinse (800 ppm Sn(2+), 500 ppm F(-), pH = 4.5) was compared with a fluoridated solution (500 ppm F(-), pH = 4.5) and water (control). Calcium release and enamel softening were significantly reduced among enamel samples exposed to the Sn/F rinse (group SF) compared to those treated with the fluoride solution (group F) and the control (p < 0.05). SEM showed slightly etched enamel interfaces in group SF, whereas the erosion was more pronounced in group F and even more severe in the control group. In conclusion, the Sn/F combination provided the best inhibition of erosion among tested solutions. This study demonstrates the application of different analytical tools for comparative erosion quantification. A strong correlation (r(2) ≥0.783) was shown between calcium release and enamel softening during demineralization.

Infected periodontal granulation tissue contains cells expressing embryonic stem cell markers. A pilot study. 受感染的牙周肉芽组织含有表达胚胎干细胞标记物的细胞。一项初步研究。

The commonly practiced removal of granulation tissue during periodontal surgery, aiming to eliminate infection and optimize healing conditions, may also remove progenitor stem cells that could otherwise support periodontal regeneration. The present study aimed to investigate if cells with embryonic stem cell properties are present in periodontal granulation tissue. During the course of flap surgery inflammatory granulation tissue was obtained from four patients and five periodontal defects. Tissues were processed in a collagenase/dispase solution to release the cells. Part of the resulting suspension was processed for bacteriological analysis (IAI PadoTest 4.5), whereas the remaining cell suspension was cultured and passaged once. Upon reaching confluence, total RNA was extracted, followed by cDNA synthesis. PCR was then performed (SYBR Green-based protocols) to measure gene expression levels of Collagen type I, and embryonic stem cell markers Nanog, Oct4, Rex-1 and Sox2. Results are expressed as 2⁻Δ(Ct) values of the target gene, calibrated against a house-keeping gene (GAPDH). A high total bacterial load up to 20.6 ± 11.0×10(6) counts/mg of tissue was found. Collagen type I was strongly expressed, confirming the predominance of mesenchymal/fibroblastic cells. Among the studied embryonic stem cells markers, Nanog was most highly expressed (2.3 ± 1.2), followed by Oct4 (1.1 ± 0.5), Rex-1 (0.6 ± 0.2) and Sox2 (0.3 ± 0.2). This is the first study that demonstrates the presence of cells expressing embryonic stem cell markers among infected granulation tissue. This knowledge needs to be considered when devising future strategies to improve periodontal wound healing and regeneration.

Prescription of antibiotics in the dental practice: a survey of dentists in Switzerland. 抗生素处方在牙科实践:瑞士牙医的调查。

In dentistry, antibiotics are prescribed both for prophylactic and therapeutic purposes. Their unwarranted use, however, may lead to the selection of resistant microorganisms. The aim of this study was to evaluate the indications and the extent of antibiotic prescription by dentists in Switzerland. A questionnaire was sent to 800 dentists during the winter of 2008/2009. Responses to the questionnaire were received from 20% of them. Many respondents indicated that they tried to prescribe antibiotics in a selective and cautious manner. Nevertheless, uncertainties existed regarding the indications. For example, the frequency of prescribing an antibiotic when it was not absolutely necessary was related to the respondents' uncertainty regarding the correct diagnosis and therapy. Not surprisingly, two-thirds of the respondents would prefer to have precise guidelines for the use of these drugs.

Orofacial pain often causes special difficulties to patients and dentists. Numerous differential diagnoses require the utilization of a coordinated diagnostic concept. Often, multiple causes lead to the need for a complex treatment plan. Impacted third molars are a potential cause of a variety of complications. Caries, pulp necrosis, and periapical infection are some of the infrequent causes of such pain. The presented case shows just such a constellation, resulting in primarily "unclear" orofacial pain. A diagnostic sequence generally leads to the correct diagnosis and thereby allows for fast and effective therapy. This shows how important structured diagnostics are, especially in cases of "unclear" pain.

Introduction: Even if a mortal incident occurs rarely in a dentists daily routine there is a chance of it. The entire team has to face a great challenge then. Stress and incertainty are leading to mistakes and cost precious time which is not given for treating due to emergency.

Material: Usual emergencies helped developing emergency kits which should make it easy for the practise team to act right and aim orientated in case of emergency. Therefore the combination for an as much efficient as possible state of those kits has priority. Through these the acting procedures turn out easier and more secure.

Result: The management in case of emergency could be highly improved through the developing of emergency kits. The acting procedures turn out easier and more secure. The possibility for mistakes decreases.

Stress and incertainty are leading to mistakes and cost precious time which is not given for treating due to emergency.</p><p><strong>Material: </strong>Usual emergencies helped developing emergency kits which should make it easy for the practise team to act right and aim orientated in case of emergency. Therefore the combination for an as much efficient as possible state of those kits has priority. Through these the acting procedures turn out easier and more secure.</p><p><strong>Result: </strong>The management in case of emergency could be highly improved through the developing of emergency kits. The acting procedures turn out easier and more secure. Infected periodontal granulation tissue contains cells expressing embryonic stem cell markers. A pilot study. 受感染的牙周肉芽组织含有表达胚胎干细胞标记物的细胞。一项初步研究。

The commonly practiced removal of granulation tissue during periodontal surgery, aiming to eliminate infection and optimize healing conditions, may also remove progenitor stem cells that could otherwise support periodontal regeneration. The present study aimed to investigate if cells with embryonic stem cell properties are present in periodontal granulation tissue. During the course of flap surgery inflammatory granulation tissue was obtained from four patients and five periodontal defects. Tissues were processed in a collagenase/dispase solution to release the cells. Part of the resulting suspension was processed for bacteriological analysis (IAI PadoTest 4.5), whereas the remaining cell suspension was cultured and passaged once. Upon reaching confluence, total RNA was extracted, followed by cDNA synthesis. PCR was then performed (SYBR Green-based protocols) to measure gene expression levels of Collagen type I, and embryonic stem cell markers Nanog, Oct4, Rex-1 and Sox2. Results are expressed as 2⁻Δ(Ct) values of the target gene, calibrated against a house-keeping gene (GAPDH). A high total bacterial load up to 20.6 ± 11.0×10(6) counts/mg of tissue was found. Collagen type I was strongly expressed, confirming the predominance of mesenchymal/fibroblastic cells. Among the studied embryonic stem cells markers, Nanog was most highly expressed (2.3 ± 1.2), followed by Oct4 (1.1 ± 0.5), Rex-1 (0.6 ± 0.2) and Sox2 (0.3 ± 0.2). This is the first study that demonstrates the presence of cells expressing embryonic stem cell markers among infected granulation tissue. This knowledge needs to be considered when devising future strategies to improve periodontal wound healing and regeneration.

[The stamp technique for direct composite restoration]. [直接复合修复的图章技术]。

The indications for direct resin composite restorations are nowadays extended due to the development of modern resin materials with improved material properties. However, there are still some difficulties regarding handling of resin composite material, especially in large restorations. The reconstruction of a functional and individual occlusion is difficult to achieve with direct application techniques. The aim of the present publication was to introduce a new "stamp"-technique for placing large composite restorations. The procedure of this "stamp"-technique is presented by three typical indications: large single-tooth restoration, occlusal rehabilitation of a compromised occlusal surface due to erosions and direct fibre-reinforced fixed partial denture. A step-by-step description of the technique and clinical figures illustrates the method. Large single-tooth restorations can be built-up with individual, two- piece silicone stamps. Large occlusal abrasive and/or erosive defects can be restored by copying the wax-up from the dental technician using the "stamp"-technique. Even fiber-reinforced resin-bonded fixed partial dentures can be formed with this intraoral technique with more precision and within a shorter treatment time. The presented "stamp"-technique facilitates the placement of large restoration with composite and can be recommended for the clinical use.
