An algorithm for treatment of a patient with an infected wound using povidone-iodine in the outpatient setting 在门诊使用聚维酮碘治疗感染伤口患者的算法

The high prevalence of skin and soft tissue infections (SSTI) urges healthcare professionals to improve the traditional (operative and pharmacological) methods of their treatment, and to find new methods that are applicable in outpatient practice, which can reduce the period of disability and reduce the number of complications. The choice of the right drug for such infections to influence the local infectious process is one of the most important objectives of the local wound therapy. The paper presents data on the effectiveness of Povidone-Iodine (Betadine) for external use with a broad spectrum of action, intended for the treatment of SSTI, including abscesses, furuncles, carbuncles, infected traumatic lesions of the skin and trophic ulcers in Phases 1 to 2 of the wound process. The drug has a pronounced combined effect: antimicrobial (bactericidal, bacteriostatic), wound healing, regenerating, local anesthetic and protective. This brings us to the conclusion about the advantages of Betadine and provide a rationale for its use to treat surgical infections of the skin and soft tissues in the outpatient setting.

Topical use of antibiotics in the treatment of wound infections as exemplified by the effects of neomycin 局部使用抗生素治疗伤口感染,例如新霉素的效果

The problem of effective treatment of the local surgical soft tissue infection has not yet been completely solved. The use of topical antibiotics, in particular neomycin, is one of the possible ways to solve this problem. The use of topical drugs that contain neomycin or combinations of neomycin and other antibiotics showed high efficacy of these drugs in the comprehensive treatment of wounds in various pathologies in combination with a low incidence of undesirable reactions.

The 19th Annual Meeting of the European Venous Forum held in the city of Athens in 28-30 June 2018 presented the long-awaited updated guidelines for the diagnosis and treatment of chronic venous diseases (CVD), the full text of which can be found in the June issue of International Angiology Journal. This publication aims at acquainting practitioners with one of the key sections of the new European guidelines devoted to the pharmacotherapy of CVD.

Possibilities of minimally invasive treatments of hemorrhoidal disease 微创治疗痔疮病的可能性

The article presents the results of surgical treatment of patients with stages III to IV hemorrhoids followed by an observation period of 45 days, which is designed to improve the results of surgical treatment of this category of patients. The surgical treatment methods included doppler-guided transanal hemorrhoiddearterialization with mucopexy (DDM) (Group I) and harmonic scalpel hemorrhoidectomy (HSH) (Group II). Operated patients underwent all necessary examinations according to the “per protocol” principle. Study Design: single-center controlled randomized prospective. The effectiveness of DDM was comparable to that of hemorrhoidectomy (HE), which made it possible to significantly reduce the duration of the operation (DDM 17.9 ± 6.1 min, GE 34.5 ± 10.1 min (p <0.01) ), reduce the level of pain in the postoperative period (DDM an average of 2.5 points, HE 4.8 points (p <0.01)), reduce the frequency of narcotic analgesics (DDM an average of 1.3 doses, HE an average of 6.1 doses (p <0.01)) and shorten the period of disability (DDM 14.4 ± 5.2 days, HE 30.3 ± 5.4 days (p <0.01)) patients with stages III to IV disease.

Experience of outpatient treatment of trophic foot ulcers complicating the course of diabetes 糖尿病合并营养性足溃疡的门诊治疗体会

The work was aimed at analysing the annual results of outpatient observation of patients with trophic ulcers (TU) on the background of diabetes mellitus while applying modern principles of general and differentiated local treatment in the Diabetic Foot Room (DFR) environment. The authors carried out a retrospective analysis in a cohort of patients with diabetic foot syndrome (DFS), who visited a practitioner to receive advice and treatment in the DFR in 2015–2017 (n = 570). Of which, only 308 people have been treated on an outpatient basis. Neuropathic form (n = 194): diabetic osteoarthropathy (DOAP) (A0) = 63, DOAP + TU = 34; TUs without DOAP = 131 (AI 105, BI 26). Neuroischemic form (n = 114), all had TUs (CI 107, D I 7). The patients received glucose-lowering therapy correction, feet relief and differentiated local effects on TUs in accordance with the wound process stages. All options of modern wound dressing were used for indications. As a result, out of 83 patients with neuropathic DFS form, who were regularly observed in DPT, 65 (78.3%) patients achieved healing within 1 year. Of 103 patients with the neuroischemic form regularly observed in DFR, 76 (73.8%) patients achieved epithelialization, 19 patients (18.4%) reported unhealed TUs, 3.9 and 3,9% of patients had small and high amputations, respectively. 96.1% of patients achieved preservation of the supporting limb. Differentiated treatment and dynamic observation in the DFR environment made it possible to reduce the number of high limb amputations in patients with diabetic TUs within 1 year of observation to the minimum.

The influence of prolonged antithrombotic therapy on the development of post-thrombotic syndrome in patients with proximal deep vein thrombosis of the lower limbs 长期抗栓治疗对下肢近端深静脉血栓形成患者血栓后综合征发展的影响

The article is devoted to the incidence and severity of post-thrombotic syndrome (PTS), the odds ratio (OR) of PTS development in patients with previous proximal deep vein thrombosis (DVT) against the background of prolonged antithrombotic warfarin and sulodexide (SD) therapy (ATT) with due account for safety and development of clinically significant bleeding (CSB) during the year. A total of 130 patients aged 18 to 69 years with acute proximal DVT were enrolled in the comparative prospective study. Group I included 64 patients (31 men and 33 women), who received vitamin K antagonists (warfarin) therapy. Group II included 66 patients (37 men and 29 women), who received sulodexide in 3 months after completing a course of standard ACT (heparins in the acute period with the transition to AVK). The studied parameters included OR, incidence and severity of PTS in patients according to the Villalta scale, and the incidence rate of PTS against the background of prolonged warfarin and sulodexide ATT in 12 months’ time. It was established that the prolonged sulodexide ATT within one year after completing a course of standard ACT in patients with proximal DVT reduced the development of clinical signs of PTS by 22.2% compared to the standard warfarin therapy. At the same time, there is a lower incidence of severe forms of PTS in patients, who underwent proximal venous thrombosis, 17.8% against the background of sulodexide treatment. Thus, the prolonged sulodexide ATT is associated with a 1.7-fold decrease in the risk of the development of both PTS and its severe forms. Therefore, the prolonged use of sulodexide for one year in patients, who underwent proximal DVT, provides an alternative to warfarin and is associated with a lower incidence of PTS, including its severe forms. The prolonged sulodexide ATT is safe and does not cause the development of clinically significant hemorrhagic complications during the year.

Group I included 64 patients (31 men and 33 women), who received vitamin K antagonists (warfarin) therapy. Group II included 66 patients (37 men and 29 women), who received sulodexide in 3 months after completing a course of standard ACT (heparins in the acute period with the transition to AVK). The studied parameters included OR, incidence and severity of PTS in patients according to the Villalta scale, and the incidence rate of PTS against the background of prolonged warfarin and sulodexide ATT in 12 months’ time. It was established that the prolonged sulodexide ATT within one year after completing a course of standard ACT in patients with proximal DVT reduced the development of clinical signs of PTS by 22.2% compared to the standard warfarin therapy. At the same time, there is a lower incidence of severe forms of PTS in patients, who underwent proximal venous thrombosis, 17.8% against the background of sulodexide treatment. Thus, the prolonged sulodexide ATT is associated with a 1.7-fold decrease in the risk of the development of both PTS and its severe forms. Therefore, the prolonged use of sulodexide for one year in patients, who underwent proximal DVT, provides an alternative to warfarin and is associated with a lower incidence of PTS, including its severe forms. Effectiveness and safety of different schemes of perioperative prophylaxis withheparins in spine surgery 脊柱外科围手术期不同肝素预防方案的有效性和安全性

To study of effectiveness and safety of different schemes of perioperative prophylaxis against thromboembolism with heparins 36 patients who has been undergone spinal surgery under general anaesthesia were divided into three equal groups. Heparin, enoxaparin, and bemiparine were administrated to patients of h-group, e-group, and b-group, respectively, in routine regimen. Large ecchymosis, thromboembolic events, and bleeding were registered for first 5 postoperative days. Only the former has shown statistically significant differences between hand b-groups (p=0.01). In addition, emergence of large ecchymosis was higher in e-group compared with b-group (p=0.047). Thus, our data have suggested effectiveness and safety of different schemes of perioperative prophylaxis with heparins in spine surgery.
