In four cases of intracranial suppurative disease, studied in the laboratory of Anaerobic bacteria of Institute Pasteur of Algiers, three cases are caused by anaerobic bacteria and one remain sterile. Two of them are empyema with unknown origin and the others are brain abscess with bilateral chronic ear infection to one. In the three cases, gram negative anaerobic bacilli were grown in culture: Bacteroides asaccharolyticus, Bacteroides vulgatus and Bacteroides ruminicola brevis. In two cases, the bacilli are associated with gram positive cocci: Streptococcus intermedius (anaerobic bacteria) and Streptococcus NG (facultative anaerobic bacteria).

We have used spore isolation by the sodium thioglycolate-lysozyme technique on collected stools. Of the 51 stools studied, we found 41% of Clostridium perfringens with an average ratio of 10(4) germs/gr. 15 strains were typical double hemolysis and trehalose positive-5 presented only one hemolysis and were trehalose negative. We only found a single strain of Clostridium difficile with a rate of 10(4) germs/gr in the stools of a 10 months infant.

573 strains of Klebsiella were isolated in Geneva (239 strains) and in Algiers (334 strains). The complete work with bacteriologic features (culture, biochemistry and antigenicity) will be published later. Two investigations took place in surgical cardio-vascular service in Geneva cantonal hospital and in Algiers hospitals: Pediatric (Beni-Messous) and intensive care service (Mustapha). High resistance percentages are found with Penicillins (greater than 60% by C.M.I.) in Geneva and Algiers. The strains are susceptible to aminoglycosides: in Algiers, 8.68% strains are resistant to Gentamicin and 5.99% to Tobramycin. In Geneva, all the strains are susceptible to Aminoglycosides. All the strains are susceptible to Trimethoprim and the Furans resistant strains are rare. The C.M.I. test is more precise than disc diffusion technique, especially for Penicillin and Furans. Cephalosporins and aminoglycosides are the best choice for treatment of Klebsiella infections.

The monoclonality of myeloma proteins is usually demonstrated by their electrophoretic homogeneity and their reactivity with monovalent antisera directed against isotypic determinants of a single heavy chain and a single type of light chain. The absence of precipitation with anti-sera to immunoglobulin kappa and Lambda light chains is a constant character of heavy Chain Disease Proteins (HCDP). However, homogeneous M-components present in the sera of some patients and reacting only with anti-heavy-chain antisera were identified as IgA and IgD myeloma proteins bearing unreactive Lambda chains. In this study, the electrophoretic pattern of a patient serum showed a paraprotein with heterogeneous electrophoretic mobility and precipitation reaction limited to anti-IgD antiserum. The failure to react with anti-light chain antisera was observed by immuno-electrophoresis, immunofixation and rocket-immunoselection. Further analysis by crossed-immunoelectrophoresis revealed that IgD paraprotein contained two separate populations of molecules, one of them being retained when anti-Kappa and Lambda light chains anti-bodies were incorporated in the first dimension gel. It soon became obvious that the observed pattern was generated by enzymatic cleavage of native IgD myeloma protein.
