Even though allergic reactions to local anesthesia are rare and even more so among elderly people, the present case demonstrates that a medication can be tolerated for a significant period of time before causing an accident. This illustrative case details the true nature of complications observed following local anesthesia and suggests a way of approaching the same to arrive at a definitive diagnosis. This method consists of a rigorous analysis of the clinical manifestations and allergologic testing. Local anesthesia is indispensable for both the dentist and the patient and anyone with allergies is at risk at being allergic to general anesthesia as well.

Fearful children in the dental office is an old problem which the majority of dentists have to face in their daily practice. With the help of a brief review of the literature, diverse strategies employed to adjust to the different behavior patterns of children in the dental office are detailed. Furthermore, to illustrate several of the techniques proposed, we have during the month of May 1990, systematically filmed different behavior patterns of children in the waiting room, as well as, in the dental operatory. The material thus obtained, permitted the production of videocassettes on the following themes: "How to approach the child in the dental office?"; "How to treat the young anxious child or a child with a difficult behavior pattern?"; "The first visit to the dentist"; "How to present dental injections?"; "The presence of parents in the dental operatory"; and, "Preventive dentistry". These cassettes are meant for dentists, hygienists, dental assistants, as well as, dental students. Moreover, in the hands of parents, this material answers their numerous questions. Each individual will find something in these presentations, or at least some areas for future exploration.

As of last year, new Canadian dietary recommendations and nutritional requirements have been established. The combination of these two documents evolved as a result of a better understanding of nutrition, as well as, an awareness that the problems related to nutritional deficiencies have resulted in numerous chronic diseases. This article, after an initial review of the origin of these documents, highlights the main changes that have occurred and reviews how this information is used and interpreted by various health care professionals.

Three methods used to study the biocompatibility of dental materials are compared in this article. A glass-ionomer cement, Vitrabond, was studied, using gutta-percha as the control material. Specimens were standardized according to an original procedure. Unsterilized Vitrabond implants were used, because UV rays modify the material. Intraperitoneal Vitrabond implants increased the macrophage population in rats after 24 hrs. This was caused by the surgical trauma and indicates that this method is unreliable. The intramuscular placement of Vitrabond provoked well defined lesions after a week. The use of histochemical techniques on frozen muscle demonstrated that the concentration of succinic deshydrogenase and acid phosphatase enzymes were altered when compared to the control lesions. A comparison of enzymatic lesions and an evaluation of the areas of cellular growth inhibition during in vitro experiments using a computer image analyzer leads to quantitative conclusions that Vitrabond is a cytotoxic material. The simultaneous use of histochemical techniques on muscle tissue and cell culture in vitro enhances the validity of methods used to evaluate the biocompatibility of dental materials. These methods may be used to test the toxicity of various materials. Nevertheless, for a more complete evaluation of these materials, the allergenicity and carcinogenicity potential of these products should be evaluated, prior to the final verdict as to their biocompatibility.

The purpose of this study was to evaluate two variables associated with isolated cleft palate and cleft palate related to Pierre Robin syndrome, namely, sexual incidence and dental anomalies. The study demonstrated that there are more dental anomalies associated with the Pierre Robin syndrome and that girls are more affected than boys in both instances.

Using data from a cross-sectional survey on 570 fifteen year old students living in non-fluoridated public school sectors on the Island of Montreal in 1987, the effect of terminating the restorative aspect of public dental health care insurance for twelve to fifteen year old students is examined. This study also highlights the level and quality of dental caries treatment on the twelve to fifteen year old group. The main point to be made is that there was an improvement in the DMFT index values of fourteen and fifteen year old students in metropolitan areas. In reality, fourteen year old children had a DMFT index of 6.45 in 1983-1984, while the fifteen year old children in the 06A region (non-fluoridated area) had a DMFT of 6.07 in 1987. Since the metropolitan fluoridated area had been eliminated from the results in 1987, it is quite obvious that the 6.07 result obtained overestimates the actual prevalence of dental caries in the metropolitan area in 1987. On the other hand, the progress which had been recorded between 1977 and 1984 by francophones in relation to anglophones was partially lost in 1987. Furthermore, this study has demonstrated that the prevalence and level of dental caries treatment are highly associated with the language spoken at home and also with the family's socio-economic status.

A recent leaflet published for dental patients by the Canadian Association of Confectioners promotes the consumption of sweets, considering them on an equal basis, in regard to caries, as any other foods containing simple sugars, such as: fruits, vegetables, bread or pasta. This article intends to demistify such inference. While it is true that most foods containing simple sugars can lower plaque pH when considered alone, in the context of a total diet, the relationship between simple sugars and caries is much more complex. Also, in regard to total health, one can not equate all simple sugars since they differ considerably in terms of nutritional value.
