In 283 students in various branches of the health profession commonly involved in physical medicine and rehabilitation, experiences of sexual life were investigated together with parameters of sexual function and of sexual performance-orientation. The findings were related to attitudes towards sexuality of disabled subjects. Results indicate that sexual performance-orientation, frustration and dysfunction are common features for the students. Sexual performance-orientation was associated with alienation towards the sexuality and the sexual counselling needs of the disabled. It is therefore recommended that the curriculae of these students should include sexual education and possibilities for sexual counselling.

The effects of a rehabilitation programme one year after myocardial infarction (MI) were investigated in 171 patients under 65 years of age. These patients were allocated at random to rehabilitation and control groups before discharge from hospital. The groups were comparable with regard to age, sex and clinical data. The programme included physical exercise, counselling of patients and relatives, and social measures over a 3-month period during the convalescent stage. One year after MI patients in the rehabilitation group showed lower systolic blood pressure at rest and lower diastolic pressure on submaximal exercise than controls. No differences were found with regard to mean work capacity, days off work, return to work, psychological status, and understanding of the illness. At 12 months all patients were less physically and socially active than before MI, they were more dependent on their relatives than before, and they had poor understanding of their illness.

The effect of a comprehensive rehabilitation programme, including diet therapy, was studied in overweight male myocardial infarction (MI) patients randomly assigned to a rehabilitation group (n = 69) and a control group (n = 64). The results show a mean weight decrease of 3.8 kg in the rehabilitation group and a mean weight increase of 1.1 kg in the control group (t = -5.78, p less than 0.001). The proportion of patients reducing their weight more than 5 kg was 33% in the rehabilitation group and 5% in the control group (chi 2 = 15.6, p less than 0.001). Patient characteristics which significantly affected the weight change by univariate analysis were age, level of overweight, smoking habits, employment situation and social problems. Multivariate analyses yielded a two-variable model with moderate explanatory power in the rehabilitation group. The model includes the variables Body Mass Index and cessation of smoking before MI.

In this paper the analyses focus primarily on statements from those 62% (281 men and 294 women) of the participants who at the primary examination reported previous or present low back pain (LBP). In a subsample the most frequent location of the LBP was in the lower lumbar area. Pain radiating to the leg(s) was felt at some time by 36% of the men and 51% of the women. Intense pain was more frequently reported by men, and a feeling of weakness or fatique more frequently by women. Most of the participants felt that the LBP became worse during the day. The most common aggravating factor was stooping, reported by 65% of the participants, followed by the sitting position, reported by 30%. Factors of highest importance for the relief of LBP were lying down (52-54%) and walking around (34-39%). A stated history of pain radiating to the leg(s) was the best indicator for occurrence of LBP in the follow-up year.

A group of 21 patients with total knee joint replacement was followed-up ten years after the operation. The clinical assessment including pain, range of motion, muscle strength and knee function was related to an objective gait analysis which included ground reaction force patterns and joint angular motion. This method of gait analysis not only confirmed in an objective way the impression from the clinical assessment but also provided a more accurate gradation of the results. The method is applicable to patients with weight bearing problems.

Intellectual Function Training (IFT) is an occupational therapy method for remediating cognitive functions in patients with acquired brain damage and has been presented in a previous paper. It has been evaluated by comparing a group of trained patients (n = 13) using the IFT method with a control group (n = 13) which underwent conventional rehabilitation. The trained group received IFT for 40 min each day, 5 days a week for about three months. Age, education and neurological status did not differ between the groups. The measurement methods of evaluation were Intellectual Function Assessment (IFP) and three psychometric test batteries. At the beginning of the study there was no significant difference in any subtest between the two groups. After the training period there was a significant difference of at least p less than 0.05 between the trained and the control group in the IFP battery, except for the Long-term Memory subtest. The improvement for the trained group was evident six months later at the time of the follow-up measurement, clearly indicating a significant difference between the groups. In one psychometric subtest a significant difference of p less than 0.01 was found. Within the experimental group over the study time there was a slight increase in performance which was notable in seven of the psychometric subtests p less than 0.05-p less than 0.001. The positive effect of IFT is considered to be specific for the type of task in which the patients were trained, while evidence of the effect on general intellectual function is inconclusive.(ABSTRACT TRUNCATED AT 250 WORDS)

The wrist motion in 31 healthy probands was measured by four observers with a simple goniometer under standardized conditions. The INTRA-observer variation was calculated to 5 to 8 degrees and the INTER-observer variation to 6 to 10 degrees. No difference between experienced and non-experienced observers was found. The difference between the right and the left wrist was negligible, indicating that the opposite wrist can be used as reference when evaluating restrictions in motion.

Isokinetic plantar flexor peak torques (PT) and contractional work (CW) of the triceps surae muscle have been measured in 24 patients with peripheral arterial insufficiency and intermittent claudication and in 15 controls. Tests were performed both during non-fatiguing (30-180 degrees/s) and fatiguing (200 repeated plantar flexions at 60 degrees/s) conditions. The electromyographic signals (iEMG) from all three heads of the triceps surae were measured. The patients were significantly weaker (PT) and produced significantly less contractional work (CW) than the controls. In contrast, similar iEMGs of the triceps surae heads indicated similar levels of activation. At 40 contractions the majority of the patients had already given up and the remainder showed significantly greater declines in PT (50%) and CW (55%) than did the controls (13% and 18%, respectively). The decline in muscular excitations was similar in both groups. The ratio CW/iEMG showed a dramatic decline in the patients but was virtually constant in the controls. These results indicate a fatigue of low-frequency type in the patient group. There were close correlations between maximum walking tolerance and total work production.

A method for clinical studies of ground reaction forces during gait is presented. A walkway consisting of two force measuring platforms, each five meter long, allows the recording from several consecutive steps from both feet. Signals from photocells and goniometers have been added and a microcomputer system for on line recording and processing have been developed. The accuracy of the system is tested.
