The value of a compression bandage applied for 4 days following ankle sprains was assessed in a comparative study. The effect of the treatment was evaluated by changes in an inflammatory score based on a combination of subjective (pain, function) and objective (swelling, limitation of movement, pain on passive movement) factors. The study included two consecutive series each of 50 patients treated with a compression bandage, and untreated. There was no significant difference in the reduction of the inflammatory score from start to the fourth day (0.5 less than p less than 0.6) or from start to the eighth day (0.1 less than p less than 0.2) between the group treated with compression bandage and the untreated group. Analysis of the individual factors included in the inflammatory score demonstrated no difference in the course of pain, function, swelling, or limitation of movement between the two groups.

Sick-role and attitude towards disease and work two months after a first myocardial infarction (MI) were studied in relation to social, psychological and somatic factors prior to, during and after the MI in 201 consecutive male patients. Questionnaires and a brief interview covered psychological and social data. Somatic data were registered in a standardized medical examination. New concepts were introduced after factor analysis of the psychological variables. Two out of three patients displayed avoidance behaviour. Seventy per cent had restricted their everyday activities. The leisure time was dominated by passivity. Over-protection was frequent. Sick-role behaviour was significantly related to emotional upset, preoccupation with the health and self-reported coronary symptoms while the shortage of relationships to somatic and cardiac factors was notable. The patients attributed the onset of MI to work-related factors. Work dissatisfaction was an aspect of a general negative attitude towards life and was more frequent among patients with emotional symptoms. The psychological care ought to be improved in order to prevent long-term disability.

The rehabilitation outcome of seventy-eight consecutive patients with nailed hip fractures admitted to a hospital for chronic care from an orthopaedic department during 1977 through 1980 was followed for two years. They comprised only 13% of all hip fracture patients, the others being discharged to their own homes or old people's homes. Thirty-four of the admitted patients came originally from their own homes, 23 from old people's homes and the others from hospitals for chronic care. Each had been selected for long-term care by the orthopaedic surgeon and his staff as the likelihood of rehabilitation in his/her original habitat was considered poor. Because of an active rehabilitation at the hospital for chronic care, one-third of the surviving patients were discharged to their own homes or old people's homes within one year after the fracture. Most of these patients came originally from their own homes. Once discharged, the patients did not need to return to the hospital for chronic care.

Seven spinal cord injured (SCI) patients with clinical signs of knee-joint spasticity were tested with the Wartenberg pendulum test and an electrogoniometer. All patients were subjected to four channel rhythmical electrical stimulation of the knee muscles for three consecutive days. In five patients some improvement of spasticity was achieved. No increase of spasticity was observed in any patient. Combining results from two separate but similar studies it is contended that about one-half of randomly selected SCI patients with knee joint spasticity might benefit by electrical stimulation.

The aim of this study was to assess the cognitive skills and visuo-motor performances of patients with type-2 diabetes. An ADL questionnaire, neurophysiological EEG recordings and a comprehensive battery of neuropsychological tests were administered to 33 type-2 diabetics and to 33 matched controls. Neither neuropsychological nor neurophysiological EEG findings differentiated type-2 diabetics as a group from their controls.

A simple alarm circuitry for unilateral diaphragm pacer transmitters has been constructed. The system, which is attached to the 9 V battery powered transmitter, will trigger an alarm sound when the battery voltage decreases to 8 V. The system may diminish or abolish the need for special supervision of patients with phrenic nerve stimulators.

Eleven aphasia patients and seven family members participated in a five-day residential course led by a speech pathologist, a psychologist and a neurologist. The aim of the course was to give the aphasics and their relatives information on the etiology, treatment possibilities and prognosis of aphasia; work on personal and interpersonal problems through psychological counseling; improve language function through comprehensive and intensive stimulation; examine the psychological, linguistic and neurological effects of the intensive course. The course participants met again one year after the course and were found to have changed positively both psychologically and interpersonally but, as expected, only to a minor degree linguistically and neurologically. Through this intensive course it was possible to assess the problems of the families more easily than in the hospital. The patients and their relatives reported that they had learnt how to identify and to deal with their psychological problems in a more constructive way. They also felt that they had support from the other families and were able to share their experiences. We found that this type of intervention had many positive effects on the ability of the participants to cope with the chronic disability.

Rehabilitation in patients with bilateral high above elbow amputation presents a considerable prosthetic problem. A patient with high upper arm amputations after a high-voltage injury is presented. He was successfully fitted with multifunctional myoelectric hand prostheses. The problems in rehabilitation of adult bilateral arm amputees are discussed and the value of fitting these patients with electrically powered prostheses is assessed. The balance between technical and clinical aspects is discussed in relation to patient acceptance. In our case good acceptance and functional benefit was noted. The fact is stressed that the bilateral upper extremity amputee can regain considerable physical function with the fitting of suitable prostheses, even if the limb remnants are short and provide little or no function. An extensive team approach at specialized centres will favour the results.

Forty-five of 52 consecutive patients with chronic low back pain were screened for presence of HLA-B27 antigen one year after they were included in a rehabilitative program. Six (13.3%) were positive and, when re-examined radiographically, 2 had signs of ankylosing spondylitis. The proportion of antigen-positive individuals is similar to that found in a population study of healthy Swedish blood donors, and within the range of other populations of healthy controls. It is concluded that HLA-B27 is of limited diagnostic value as a screening test for ankylosing spondylitis in a patient group with chronic low back pain.

The load on the hip joint during lifting was studied. Healthy subjects lifted a 12.8 kg box from floor to table level with straight and flexed knees in four different ways. The loading moment of force about the bilateral hip axis was calculated by means of a computerized static sagittal plane model. The highest load moment, 124 Nm, occurred initially in the straight knee lift and the compressive joint reaction force was 2.7 times body-weight. The lowest load moment, 82 Nm, occurred in the flexed knee lift with the burden moved close to the trunk, the compressive force was 3.2 X bw. The load moment was also discussed in relation to the strength capability. The EMG levels of seven different hip muscles were normalized and expressed as a percentage of the recorded level of each muscle group during an isometric maximum voluntary contraction. The initial activity in the hamstrings was higher in the straight knee lift compared to flexed knees. The gluteus maximus was activated to a moderate level.
