The Positive Impact of Registered Nurse Sedation Teams 注册护士镇静小组的积极影响

Background: Sedation care documentation and patient safety on general care nursing units need improvement. Purpose: To assess the impact of implementation of a registered nurse sedation team model on patient safety and thoroughness of documentation in patients receiving moderate sedation on general care units.Methods: In 2010 a 3-month retrospective chart review determined adverse patient outcomes and incompleteness of documentation for patients receiving sedation on general care units. After implementation of the registered nurse sedation team model for 3 months, patient’s sedation documentation and outcomes were assessed. The registered nurse sedation team model was implemented into practice and further outcome data from implementation through 2017 also were assessed.Results: There was clear improvement in both required documentation during moderate sedation and patient safety (P<0.001).Conclusion: The registered nurse sedation team model in this study improved both completeness of required sedation documentation and patient safety.

Effects of Singing Songs about Preoperative Care for Tonsillectomy on the Anxiety of Children Undergoing Tonsillectomy 唱扁桃体切除术术前护理歌曲对儿童扁桃体切除术焦虑的影响

Introduction: Surgery creates anxiety for children, whose control and reduction are among the objectives of nursing care. Nurses have always sought scientific evidence for the use of innovative techniques appropriate for the level of understanding, recognition, and need of children, such as the use of poetry to reduce children's anxiety. Therefore, the present study aimed to investigate the effects singing songs about preoperative care for tonsillectomy on the anxiety of children undergoing tonsillectomy.Method: This quasi-experimental non-random study recruited 76 children aged 7-12 years old presenting to the hospital for tonsillectomy were selected with a convenience sampling method. In addition to the routine care, some songs were sung to children in the intervention group about pre- and post-operative procedures before, on the day of, and after the surgery. Hamilton Anxiety Rating Scale (HAM-A) was filled out before and after the surgery and on the day of discharge in the intervention and control groups in order to measure children's anxiety. Data were analyzed in SPSS software using Chi-square, Mann-Whitney, and Fisher’s exact tests.Results: The results showed that the mean and standard deviation of anxiety in both groups (control=19.57 ± 3.65; intervention=19.78 ± 4.87) were not significantly different before the intervention (P=0.48), while a significant difference (P<0.001) was observed on the day of surgery between the two groups (control=34.28 ± 5.68; intervention=26.97 ± 4.6). Conclusion: Nurses can sing songs about pre- and post-operative procedures as an easy, inexpensive way appropriate for children’s level of development to reduce their anxiety. Furthermore, the family’s engagement in pre-operative training in an innovative manner can be a step forward in the development of family-based care.

Case Report of a Phototoxic Reaction after Application of a Footbath with Herbal Traditional Chinese Medicine (TCM) 中药足浴后光毒性反应1例报告

This case report indicates a complication of a footbath with herbs, self-applied by a patient at home. A 60 year old female patient presented herself at the TCM (Traditional Chinese Medicine) hospital, Bad Kötzting, Germany with the main symptoms of persistent coughing fits and disturbing smells especially during meals. At admission, she was administered a high dose Chinese herbal therapy complementary to her already prescribed pharmaceuticals. During her sojourn in the hospital she started suffering from numbness in both feet, preferred on the dorsum, and up to and including the toes, accompanied with a sensation of cold. Therefore TCM herbal footbaths containing 5g Psoraleae fructus semen (Buguzhi) were administered, twice a day, for 2 weeks, during the hospital stay without any complications. One week after discharge from the hospital she went on with her footbaths, prescribed for outpatient treatment. Buguzhi is suspected to cause phototoxic reactions under sunlight. Unfortunately, the patient applied the prescription-based footbath under a parasol in her garden. This seems to be the reason, why the patient developed an erythema bullosa grade three. She was treated with antibiotics for 10 days and ointment bandages until remission. To avoid similar adverse events in future, patients have to be informed to not expose their skin to solar irradiation during or after footbaths containing Buguzhi. Alternatively it has to be considered to omit Buguzhi for the use of footbaths applied as outpatient treatment.

Nursing Education and its Cultural Congruency in the Sultanate of Oman: Case Study 阿曼苏丹国护理教育及其文化一致性:个案研究

Background: The purpose of this study is to explore whether the cultural aspects of client care as included in the nursing curriculum in the Sultanate of Oman are satisfactory. This concept will be examined by reviewing the Omani Ministry of Health’s (MoH) approach to designing, developing, and implementing a general nursing curriculum. Method: This case study was explored qualitatively to determine whether nursing education addresses culturally congruent care adequately through the review of educational policy documents, semi-structured qualitative individual interviews, and focus groups. The data was collected from the MoH’s nursing education decision-makers, educators, students, and service users (public) over a period of six months. To identify themes, the data was analyzed using Ritchie and Spencer’s five-stage analytical framework. The study used purposeful sampling to select study participants from the MoH nursing education decision-makers, educators, students and service users. Results: The study revealed that the MoH’s current approach to nursing curriculum falls short of what Denis Lawton considers a culturally competent curriculum. Conclusion: The study proposes to reexamine the cultural care content in the nursing education in Oman.

Objective: To determine the prevalence of latent tuberculosis infection among Healthcare Workers (HCWs) in Al- Thawra Modern General Hospital. Methods: We carried out cross sectional study to determine the prevalence of Latent Tuberculosis Infection (LTBI) among HCW. Two-steps Tuberculin Skin Test (TST) was performed among health care workers (HCWs) in Al -Thawra Modern General Hospital (TMGH) Sana'a- Yemen during the year 2016. We included all health care workers in the hospital. Out of 466 total HCWs 426 were fulfilled the inclusion criteria. Questionnaire was distributed to HCWs and information related to demographic data, profession, and duration of work, individual and in the family history of Tuberculosis (TB) was recorded. TST was done by a single investigator using the standard Mantoux test. The reaction was read 48 to 72 hours after injection, and the widest axis of indurations was measured by a standardized palpation method. Those with negative result were advised to come, after1- 2 weeks for second step TST. Results: The total number of health workers in the hospital were 466, Eligible cases who fulfilled the including criteria were 426. The remaining either excluded or not present at the time of study. Of them 232 (54.5%) were males and 194 (45.5 %), were females with a ratio of 1.2:1.269 (70%) were positive for TST. The positive result was highest among radiology assistant and laboratory worker represented 91%, 80% respectively, while 76% of doctors found positive for TST. There was an increase in TST reactivity with an older age, and there is a positive correlation between work duration and TST reaction. Conclusion: Latent tuberculosis infection is prominent among HCW who work in high-risk departments. This suggests that some TBI develops via in-hospital infection

Forensic Nursing Education and Practice in Lebanon: Current Status and Future Perspectives 黎巴嫩的法医护理教育与实践:现状与未来展望

Background: Civil war followed by endless socioeconomic hardships and political instabilities have taken their toll on the Lebanese people. As a result, their physical, mental, and psychosocial wellbeing have been significantly jeopardized. This has been demonstrated in the significant increase in the stress-related disorders, many of which are underreported. Mental health and forensic medicine specialists are still in low number in the country, and their services are too expensive to be approached by the people in need. As such, establishing Forensic Nursing Education and Practice in Lebanon seems to be a necessity. Purpose: The aim of this study is to survey Forensic Nursing education and practice in Lebanon. Method: We conducted two separate comprehensive PubMed and Medline searches using MeSH terms “Forensic Nursing education” AND “Lebanon”, and “Forensic Nursing practice” AND “Lebanon”. In parallel, we reviewed the catalogues and the related description of courses of the nursing programs currently offered by the various universities, colleges, technical institutions, and hospitals in Lebanon in relation to Forensic Nursing education. Finally, we reviewed the Lebanese Law for the existence of legislations governing the practice of Forensic Nursing in Lebanon. Results: Our results revealed that Forensic Nursing education still lacks in the nursing degree programs offered by the various universities, colleges, technical institutions, and hospitals in Lebanon. Moreover, legislations governing the practice of Forensic Nursing still do not exist. Conclusion and implications: Introducing Forensic Nursing in the nursing programs offered in Lebanon seems to be an urgent need. This should be followed by establishing a track in Forensic Nursing licensed practice that is accompanied by proper government legislations.

Establishing a Rural School of Nursing in Lebanon: A Practical Model 在黎巴嫩建立农村护理学校:一个实用的模式

Background: Rural nursing practice and education remain a difficult task to achieve in first world countries, let alone in a third world country like Lebanon. The latter sustained 15 years of civil war, followed by ongoing political and economic instability. North and South Lebanon, and Bekaa are rural sites, and are considered the most socioeconomically-disadvantaged geographic locations in the country. This includes severe shortage in Nursing practice and education. Purpose: The aim of this study is to share the experience in the provisional establishment of a School of Nursing in rural Lebanon, hoping that such an initiative would help in lessening the severity in the shortage of qualified nurses rurally, and, thus, in improving health care. Method: The model followed is based on four main pillars, namely approaching the locals, establishing the matrix, designing the curriculum, and setting-up research priorities. Each of these pillars consists of various components at different levels. Results: Approaching the locals and establishing the matrix are essentials and prerequisites for the other two main pillars. The former is time-consuming, requires well-trained human resources, and takes a big proportion of the time allocated to the project. Establishing the matrix, designing the curriculum, and setting-up research priorities are equally-important, and each has its own peculiarities and requirements that are summarized in this manuscript. Conclusion and implications: Setting-up a rural School of Nursing in Lebanon is not a privilege. It is rather a necessity, and requires careful planning and allocation of significant human and non-human resources. However, the experience is very much enjoyable, has a unique flavor, and provides the best solution for the severe shortage in qualified nurses from which the local villages suffer.

Lipoid Pneumonia Due to Aspiration of Oil Products: Two Case Reports 油品误吸致脂质性肺炎2例报告

Lipoid pneumonia which is a rare disorder, results from accumulation of lipids in the alveoli. Lipoid pneumonia has two types; exogenous pneumonia which results from deposition of inhaled oil in the lung and endogenous pneumonia which results from breakdown of lung distal to obstructed airway. Tumor or bronchiolitis obliterans or following chemotherapy or radiotherapy lead to release of cholesterol and other lipids in alveoli. We report clinical and radiological findings of two cases with exogenous lipoid pneumonia due to aspiration of oil products.
