Comparing New-onset Diabetes in Post Moderate-to-severe Coronavirus Disease 2019 Patients with Newly Diagnosed Diabetes Patients with Mild or without Prior Coronavirus Disease 2019 Infection 中重度冠状病毒病 2019 后患者新发糖尿病与轻度或未感染过冠状病毒病 2019 的新诊断糖尿病患者的比较

Systemic inflammation and various drugs used for treating coronavirus disease 2019 (COVID-19) including high-dose corticosteroids play a role in the development of new-onset diabetes (NOD). Patients of all ages and genders recovering from COVID-19 reportedly have an increased incidence and relative risk of developing diabetes. The present study thus tried to compare NOD in post moderate-to-severe COVID-19 patients with newly diagnosed diabetes patients with mild or without prior COVID-19 infection. This cross-sectional study included 100 newly diagnosed diabetes patients with a previous history of moderate-to-severe COVID-19 and those with no or mild history of previous COVID-19 infection (50 each in both groups). A detailed history was taken, and a clinical examination was done. Data on symptomatology, family history of diabetes, behavioral habits, anthropometry, and glycemic parameters was captured. Stress levels were assessed using the 5-point Perceived Stress Scale-10. The data obtained were statistically analyzed. Assessing for the glycemic indices – fasting blood glucose, postprandial blood glucose, and glycosylated hemoglobin – was significantly higher in the moderate/severe COVID-19 arm as in comparison to the no/mild COVID-19 arm (P < 0.0001). C-peptide was significantly higher in the moderate/severe COVID-19 arm (P < 0.0001). The stress score was as high as 23.54 for the moderate/severe COVID-19 arm in comparison to 15.00 for the no/mild COVID-19 arm. NOD following COVID-19 pneumonia differs to some extent from NOD without COVID-19 in terms of having increased insulin resistance and worsened glycemic parameters. Clinicians should be aware of this entity as more aggressive control of dysglycemia is warranted to avoid complications arising out of it.

Emergencies in Dengue Fever 登革热的紧急情况

Dengue is often seen as an acute infection with fever and thrombocytopenia where complications such as shock and hemorrhage need to be averted while the patient recovers with supportive care. However, dengue can also be a differential diagnosis in the emergency care setting. Acute encephalopathy, acute renal failure, hepatitis, liver failure, acute pancreatitis, noncardiogenic pulmonary edema, acute respiratory distress syndrome, compartment syndrome, intracranial bleeds, hemophagocytic lymphohistiocytosis are few of the emergencies that may be seen in the course of dengue fever. These complications need prompt recognition and management to ensure better outcomes.

A Rare Case Report of Pulmonary Hemorrhage in Electrical Injury 电击伤肺出血的罕见病例报告

We report a rare case of bilateral pulmonary hemorrhage with acute respiratory distress syndrome (ARDS), following high-voltage electricity exposure in a 29-year-old electric lineman while cutting an electrical wire for an uncertain duration. He was unconscious after falling from a tree following the exposure. His chest radiograph showed bilateral nonhomogeneous opacities, and high-resolution computed tomography revealed bilateral extensive ground-glass opacities, suggestive of pulmonary hemorrhage with ARDS. The patient was managed conservatively with intravenous fluids and oxygen. He recovered with near-complete resolution of chest X-ray abnormalities by day 5 of admission.

An Interesting Case of Takayasu’s Arteritis 一个有趣的高安动脉炎病例

Takayasu’s arteritis is an inflammatory and stenotic disease of medium- and large-sized arteries characterized by a strong predilection for the aortic arch and its branches. Takayasu’s arteritis is an uncommon disease with an estimated annual incidence rate of 1.2–2.6 cases per million. It is most prevalent in adolescent girls and young women. Although it is more common in Asia, it is neither racially nor geographically restricted. This is a case report of a 58-year-old male patient who presented with syncopal attack and weakness in the right lower limb. He had a history of cerebrovascular accident (CVA) 10 years back with right upper limb monoparesis. Patient had feeble pulses in bilateral upper limb and absent pulsations in the right lower limb and left lower limb. Carotid pulsations were absent on the left side. Upper limb Doppler showed dampened and monophasic flow in ulnar and radial artery on both upper limbs. Dampened and monophasic flow in the right leg arterial system and posterior tibial artery of the left lower limb.

A Rare Case of Cutaneous Metastasis of Osteosarcoma 骨肉瘤皮肤转移的罕见病例

Osteosarcoma (OS) is a primary bone tumor, usually seen in childhood or adolescence. With an estimated incidence of 5 in every 100,000 new cases detected per year, OS is an aggressive malignant mesenchymal tumor. The most common sites for this tumor are the distal end of the femur and the proximal tibia. Metastases from OS are most commonly seen in the lung or other bones. Patients with skin and subcutaneous tissue metastases are a relatively rare subset. Here, we report a rare case of an 18-year-old girl who presented to the hospital with an unusual site of metastasis from OS.

Perception of the Health-care Providers toward Telemedicine Practice: A Cross-sectional Study from Karnataka, India 医疗服务提供者对远程医疗实践的看法:印度卡纳塔克邦横断面研究

The major advantages of telemedicine can be saving time, cost, and effort, especially for rural patients, during challenging times such as COVID for obtaining consultation and treatment. Telemedicine, if properly implemented, can reduce gaps and strengthen the health-care delivery system in India. The objective of the study was to assess the perception and practice of health-care providers toward telemedicine. We conducted a cross-sectional study among 141 health-care providers who ever practiced telemedicine consultation. Utilizing online Google Forms, we used snowball sampling. Four sections made up the pretested study questionnaire: demographic information, perceptions, and practices. Statistical Package for the Social Sciences (SPSS) version 21 was used for data analysis. Among them, majority were aged between 35 and 44 years (49.6%), males (56%), had M.D./M.S. degree (57.4%), postgraduates (33.3%), worked in private sector (56.7%), and were from clinical specialty (80.8%) We had majority health-care workers who used telemedicine application for the first time after COVID (80.1%), used only mobile (82.2%), used only one application (83.6%), and gave 5–10 consultations per day (57.4%). Most of our study participants feel teleconsultation takes less time (61.7%), reduces waiting list at medical center (92.9%), has better communication (83.6%), gives better aid during emergency (53.9%), and saves money on transportation (97.1%). In this study, we could conclude that most participants started practicing telemedicine post-COVID. Health-care professionals still feel challenging to provide effective patient counseling through telemedicine; moreover, they are benefitted from telemedicine practice.

Among them, majority were aged between 35 and 44 years (49.6%), males (56%), had M.D./M.S. degree (57.4%), postgraduates (33.3%), worked in private sector (56.7%), and were from clinical specialty (80.8%) We had majority health-care workers who used telemedicine application for the first time after COVID (80.1%), used only mobile (82.2%), used only one application (83.6%), and gave 5–10 consultations per day (57.4%). Most of our study participants feel teleconsultation takes less time (61.7%), reduces waiting list at medical center (92.9%), has better communication (83.6%), gives better aid during emergency (53.9%), and saves money on transportation (97.1%). In this study, we could conclude that most participants started practicing telemedicine post-COVID. A Study of Diabetes in Young Individuals with Special Reference to Type 2 Diabetes Mellitus 关于年轻人糖尿病的研究,特别是关于 2 型糖尿病的研究

Type 2 diabetes mellitus (T2DM) in young individuals is a serious concern for the society. Literature suggests that the incidence of T2DM is not only increasing, but also there is a shift of age of onset of T2DM toward younger age groups in India and worldwide. However, data about the frequency of T2DM among young subjects below 25 years of age are sparse in the Indian subcontinent. Hence, the current study was undertaken to find out the frequency and clinical profile of young individuals with T2DM in the departments of internal medicine and pediatrics in a tertiary care hospital. The aim of the study was to study the frequency and clinical profile of T2DM among young individuals with diabetes mellitus. The study was a cross-sectional observational study conducted in the Departments of General Medicine and Pediatrics at JSS Hospital, Mysuru, Karnataka, India, over a 2-year period. The subjects with age of onset of diabetes mellitus below 25 years were included in the study. They were categorized according to the C-peptide values as type 1 diabetes mellitus (T1DM) and T2DM. Maturity-onset diabetes of the young (MODY) was diagnosed with the age of onset of diabetes below 25 years with a strong family history of diabetes for three generations and no insulin requirement for metabolic control for 5 years after diagnosis. The statistical analyses were performed using the Statistical Package for the Social Sciences (SPSS version 25). Of the 250 patients included in the study, 208 (83.2%) subjects were T1DM, 38 (15.2%) T2DM, and 4 (1.6%) with MODY. The mean age of onset of T1DM was 13.7 ± 6.201 years and T2DM was 23.28 ± 3.68 years. Body mass index (BMI) and waist circumference of young T2DM patients were more compared to T1DM, which were statistically significant P < 0.0001, and family history showed strong positivity in patients with young T2DM compared to T1DM. Low-density lipoprotein cholesterol (LDL-C), triglycerides (TGs), and total cholesterol (TC) were more in young T2DM patients compared to T1DM which were statistically significant with P = 0.003 (LDL), P = 0.0015 (TG), and P = 0.03 (TC), respectively. T2DM, previously considered a middle-aged or elderly condition, is now commonly occurring in young adults and children, probably due to a growing epidemic of childhood obesity and lifestyle. Obesity and dyslipidemia were more in T2DM compared to T1DM among young diabetics.
