Caffeine Citrate in Neonatal Intensive Care Unit 咖啡因柠檬酸盐在新生儿重症监护病房

Apnea is a common condition in premature infants due to the immaturity of respiratory control mechanisms. Incidence increases with younger gestational age and lower birth weight, afflicting 25% of infants under 2500 g and 80% under 1000 g . Recurrent apnea can lead to respiratory failure, pulmonary hemorrhage , abnormal lung function, intracranial hemorrhage, abnormal neurodevelopment and even sudden death. Caffeine is the most frequently used medication in the neonatal intensive care unit. It is used for the prevention and treatment of apnea, although this drug has been associated with lower incidence of bronchopulmonary dysplasia (BPD) and patent ductus arteriosus as well as intact survival at 18-21 months of life. The mechanism of action of caffeine on prevention of apnea and activation of breathing seems to be through central inhibition of adenosine receptors. Caffeine

Associated Cardiac Anomalies Detected by Echocardiography in Infants and Children with Congenital Malformations. A Prospective-Analytical Study 先天性畸形婴儿和儿童超声心动图检测相关心脏异常。前瞻性分析研究

Background: Infants with congenital malformations are more common to have cardiac malformations. Aim : To detect the frequency of associated congenital heart diseases and other congenital malformation of different systems. Subjects and methods: sixty patients who were admitted at pediatric and neonatology department at Minia health insurance hospital & Al-Azhar University-Assuit hospital aging from 1 day to 48 months (4 years) old will be included in the study between first of January 2022 and the end of November 2022. Results : A total number of 60 patients with known congenital malformations, 10 patients with Genitourinary congenital malformations (16.6% ), 9 patients with renal malformations (15% ), 8 patients with digestive malformations (13.3% ) , 9 patients with biliary atresia (15% ), 10 patients with genetic malformations (16.6% ), 14 facial congenital malformations (23.3 %). The echocardiography showed in seventy patients mild ASD (28.3%) while 5 patients showed moderate ASD (8.3%), 5 patients showed mild TR (8.3%), 17 patients showed PDA (28.3%), 6 patients showed PFO (10%), 2 patients showed mild MR (represent 3.3%), 18 patients showed VSD (represent 30%) . 29 patients showed normal PASP (48.3%), 4 patients had severe PASP (6.7%), 10 patients with moderate PASP (16.7%), 17 patients with mild PASP (28.3 %) with mean 40.3±13. Conclusions: Regarding the correlation between cardiac anomalies and other congenital malformations, there were significant correlation between cardiac anomalies and renal system, biliary system, genetic abnormalities and facial malformations, so more attention and careful cardiac examination using echocardiography in those patients is advised.

Perinatal Care and Threshold of Viability; A Great Dilemma

Premature babies born at 21 weeks gestation or earlier have a very poor chance to survive. Babies born between 22 and 24 weeks gestation, may be able to be supported with intensive care, but have a high risk of dying despite treatment or permanent impairment in survivors. This period is sometimes referred to as the “threshold of viability”. For infants born around the threshold it may be appropriate to provide only comfort care after birth, to provide full intensive care support, or to provide a trial of treatment with management adjusted to the response to resuscitation and intensive care. Periviability is the earliest stage of fetal maturity where there is a reasonable chance of extrauterine survival. This period is generally between 22 and <26 weeks gestational age. Most infants born at ≥26 weeks gestational ages have a high chance of survival. Where there is a possibility of preterm labor around the threshold of viability, expert obstetric and neonatal teams should be consulted. Where possible and safe, early transfer of the mother should be arranged to a center with tertiary level neonatal care. At least 2 expert neonatologists should be present at the birth of any infant around the threshold of viability whether or not active resuscitation is planned. The obstetric and neonatal decision-making is derived from local and national consensus statements.

The Role of Absolute Monocytic Count in Early Diagnosis of Nercotizing Enterocolitis in Premature Neonates. A Prospective Analytical Study 绝对单核细胞计数在早产儿Nercotizing小肠结肠炎早期诊断中的作用。前瞻性分析研究

Background: Necrotizing enterocolitis (NEC) is characterized by macrophage infiltration in the intestine. As intestinal macrophages are derived from recruitment of blood monocytes into gut mucosa, it in turn reduces blood monocytes concentration which can be helpful marker for NEC. Aim : To evaluate the contribution of absolute monocytic numbers in separating NEC from other potential causes of preterm feeding intolerance. Subjects and methods: This was a prospective research which was conducted (after exclusion of 44 (25.1%) neonates after their death) for 131 neonates (70.2% with feeding intolerance and 29.8% with NEC). All the included neonates were subjected to full history taking, clinical evaluation including also Ballard score , and full investigations including absolute monocytic count . Results : Statistically substantial variation between preterm with FI and preterm with NEC regards nutritional history with p-value <0.001 most of FI preterm was on trophic feeding while most of NEC preterm were on TPN. Statistically significant decrease of Ballard score in NEC preterm than feeding intolerance with p-value=0.014 as infants in feeding intolerance group developed feeding intolerance at an earlier postnatal age than those with NEC. AMC after 3-5 days after FI had statistically significant difference between NEC and FI with p-value 0.000. Conclusions: Absolute monocytic count in 3 rd to 5 th day after signs of feeding intolerance has predictive role in differentiation of NEC from other causes of feeding intolerance, but it had no significant difference between different stages of NEC.
