Neonatal thrombocytopenia and its relation to maternal disease 新生儿血小板减少症及其与母体疾病的关系

Introduction: Platelets first appea r in the human fetus at five weeks post-concep tion, and increase in number during fetal life, reaching n ormal adult values by 22 weeks of gestation. Objectives: The aim of this work t o screen neonatal thrombocytopenia, its causes and its relation to maternal diseases. Patients and methods: This prospective study was carried out in the Neonatal Intensive Care Unit (NICU) at the Obstetrics and Gynec ology Department of MINIA University Hospital from the beginning of March 2017 till the June 2017. T his study was carried out on 150 neonates w o were admitted to the NICU and met the inclusion crit eria for enrollment into this study including 45 (30%)male neonates and 105(70%) female neonates ,th eir age ranged from (28 week -38 week) Who delivered to mothers which has maternal disease durin g pregnancy e.g ( Diabetes , Pre mature rup ture of membrane , TORCH , preeclampsia, placental ab normalities ). Results: The study included 150 newborn children, 102 (68%) of whom were premature infants and 48(32%) of them were term neonates & (70%)of them were females and(30%) of them were males& mode of delivery was cesarean section in(82.7%)and vaginal delivery was (17.3%). In our study there were significant increase in maternal diseases in preterm neonates when compared with full term with p.value (0.004, 0.0002, 0.001, 0.001,0.001) for each of them respectively ,while HTN diseases were common in full term neonates (p.0.004). There was significant correlation between birth weight and maternal illness that thrombocytopenia most common among ELBW. Conclusion: placental insuffici ency and perinatal asphyxia are relate d to early onset thrombocytopenia.
简介:血小板在妊娠5周后首次出现在人类胎儿体内,在胎儿生命期间血小板数量增加,在妊娠22周时达到正常成人值。目的:筛查新生儿血小板减少症,其原因及其与孕产妇疾病的关系。患者和方法:本前瞻性研究于2017年3月初至2017年6月在MINIA大学医院妇产科新生儿重症监护病房(NICU)进行。本研究纳入了150例符合入组标准的新生儿,其中男婴45例(30%),女婴105例(70%),年龄为(28周-38周),其母亲在妊娠期间有母体疾病(糖尿病、胎膜早破、TORCH、先兆子痫、胎盘异常)。结果:本研究共纳入150例新生儿,其中早产儿102例(68%),足月新生儿48例(32%),其中女性占70%,男性占30%,分娩方式为剖宫产(82.7%),阴道分娩(17.3%)。在我们的研究中,早产儿与足月相比,母体疾病的发生率均显著增加,p值分别为(0.004,0.0002,0.001,0.001,0.001),而HTN疾病在足月新生儿中较为常见(p.0.004)。新生儿出生体重与母体疾病有显著相关性,其中血小板减少症在新生儿中最为常见。结论:胎盘功能不全和围产期窒息与早发性血小板减少症有关。

Prognostic value of neutrophil CD64 in low birth weight neonates with sepsis 中性粒细胞CD64在低出生体重新生儿脓毒症中的预后价值

Introduction: Neonatal sepsis represents a major health problem with high mortality and morbidity rates. Although early diagnosis of neonatal sepsis is very important for proper management yet it remains a difficult task. Neutrophil CD64 (nCD64) is used as a marker for the diagnosis of sepsis, requiring a small sample volume, short turnaround time.Objectives: In this study we aimed to study the diagnostic performance of nCD64 against routine markers in low birth weight neonates (LBWN) with sepsis.Patients and methods: A case control study was conducted on 40 LBWN suspected clinically to have early onset neonatal sepsis against 20 neonates clinically free of sepsis as control. Investigations included CBC, CRP, blood culture andnCD64 expression.Results: among the studied markers of sepsis; immature neutrophil count, immature /mature ratio, immature/total ratio, CRP and nCD64 were significantly higher in suspected group than control (p value 0.007, 0.001, 0.002, 0.001, and 0.001 respectively). Among the group of neonates with suspected sepsis, blood culture of 11 cases (27.5%) did not show growth. nCD64 showed the highest sensitivity and specificity; 100% each. Immature neutrophil count and total leucocytic count showed the lowest sensitivity 40% and mature neutrophil showed the lowest specificity 45%. The expression of nCD64 in those neonates who died as a complication of sepsis was significantly higher than those who survived (p value 0.001).Conclusion: nCD64 is a reliable marker for the diagnosis of early onset neonatal sepsis in LBWN with a significant predictive value for disease course.

An Image for Differential Diagnosis 鉴别诊断的影像

It is the most common transient rash in healthy neonate, which is a benign, self-limiting, physiological rash affecting about 50% of term newborn. It is rarely seen in preterm infants. They usually begin at 1 to 2 days of age, but may occur at any time until about the fourth day. Blotchy erythematous macules 1 to 3 cm in diameter with a 1 to 4 mm central vesicle or pustule are seen in erythema toxicum neonatorum (ETN). Number of lesions can vary from one or two to several hundred. They are not only more profuse on the trunk, but also commonly appear on face and proximal limbs and spares palms and soles. The infant appears well, unperturbed by the eruption. Spontaneous recovery usually occurs within 3 to 7 days without any residual pigmentation . A smear of the central vesicle or pustule contents reveals numerous eosinophils on Wright stain preparations. No organisms can be seen or cultured. A peripheral blood eosinophilia of up to 20% may be associated with severe cases.  No treatment is indicated and apprehensive parents can be reassured about the benign nature of the eruption.
