Reports on oral cancer date back to ancient times, with descriptions made by Egyptians and Hindus1. The Sanskrit medical text Sushruta-sahmita, written between the 4th and 3rd centuries B.C.E., had a section dedicated to HNS, Otorhinolaryngology and Ophthalmology – Shalakya-tantra2. Although this text was more dedicated to the diagnosis of infectious diseases, it included citations for the diagnosis of oropharyngeal cancer as well as descriptions of surgical techniques2. A 42-year-old male patient with a congenital facial cutaneous hemangioma on the right side that continued into the ipsilateral external auditory canal and predisposed to the development of cholesteatomatous chronic otitis. The involvement and thickening of the skin of the external auditory canal probably caused the accumulation of debris and consequent formation of cholesteatoma. The patient complained of fetid otorrhea and evolved with progressive hearing loss over time and complications of otitis, requiring a surgical approach to resolve the condition. The description of this clinical case is relevant as no similar cases of cholesteatoma with extensive facial hemangioma as the primary cause have been found in the literature to date.

Brasil. abstract Introduction: Thyroid surgery has been performed by hospitalization with the admission period exceeding 24 hours. Surgeries that allow same-day discharge reduce spending, promote greater turnover and utilization of hospital beds, and generate high patient satisfaction. However, in the thyroidectomy, the risk of hematoma and hypocalcemia are factors that cause resistance toward early discharge. Objective: The aim of this study is to describe a series of patients undergoing thyroidectomy with same day discharge, focusing on the procedure safety and patient satisfaction. Methods: A prospective study of patients undergoing thyroidectomy with same day discharge, compared with who were discharged on the first postoperative day or after, assessing clinical characteristics, type of surgery, weight of the gland, complications, and patient satisfaction. Results: A total of 105 patients were discharged on the same day, whereas 157 on the first postoperative day or after. There were no significant differences observed in the two groups’ complication rates. No patient who was discharged on the same day required hospital readmission. In the group of patients who were discharged on the same day, patient satisfaction was 98.1%. Conclusion: The same-day discharge procedure, in selected cases, proved to be safe and had a high satisfaction

Several techniques are studied for a more targeted surgery. One of them consists of intraoperative PTH measurement (IO-PTH), which can be collected from a Peripheral Vein (PV) or from a Central Vein (CV), using the Internal Jugular Vein (VJI). Objective: The aim of this study was to analyze the peculiarities of the collection sites. Methods: It is a prospective study of patients undergoing parathyroidectomy by HPTP, with PTH values in the peripheral collection and from the ipsilateral VJI at times: initial (T0) and 10 minutes after removal of the diseased gland (T10). Results: 61 participants were evaluated. The median PTH at baseline was 147.9 in the peripheral vein and 476.58 in the central vein. The median at T10 was 36 in the peripheral vein and 33 in the central vein. The central vein values showed a greater reduction, with the peripheral values showing a median decay of 74.35%, while the central values dropped by 82.38%. In the initial collection, PTH values were higher in the central vein, while they tended towards homeostasis after the removal of the diseased gland, regardless of the collection site. Thus, the average decay values from the central vein collections were higher and more significant than those from the peripheral veins. Conclusion: The use of central values thus implies greater reliability for to intraoperative monitoring, with long-term comparative studies still being necessary to determine more targeted and effective surgical approaches.

Multiple Symmetric Lipomatosis (MSL), also known as Madelung’s Disease, is a rare condition characterized by the presence of diffuse, symmetrical, unencapsulated lipomas commonly located on the face, neck, and in the proximal region of the upper limbs. Lesions are generally asymptomatic, grow slowly and progressively, but can lead to cervical mobility limitation and involve deep mediastinal structures. The disease is more prevalent in men of Mediterranean origin aged 30-60 years and is associated with chronic alcoholism. Diagnosis is based on clinical findings and complementary imaging tests. Generally, the most accepted treatment approach is surgical excision of the lesions, although several alternative treatments have been described. This study reports the clinical case of an MSL patient treated with multiple surgical resections. It describes the diagnosis and compares the clinical findings and treatment of the patient with the main correlated comorbidities in the literature.
