That isolations of Pseudomonas aeruginosa have become less frequent in certain hospitals in the USA does not necessarily imply the same to be true or even likely in hospitals elsewhere-this organism was the second commonest cause of Gram-negative bacteraemia in patients with neutropenia in Addenbrooke's Hospital in the last three years. Further aspects of this article are open to similar criticism; we do not wish to labour the point, but simply to emphasise the importance for those who treat leukaemia of working in close collaboration with their own microbiological colleagues and establishing patterns of practice in conformity with their own local needs.

Acute otitis externa may be a diffuse inflammation or a furuncle. A furuncle is a very tender swelling in the outer ear canal (there are no hair follicles in the bony meatus). Hearing is impaired only if the meatus is blocked by swelling or discharge, and fever occurs only when infection spreads in front of the ear, as cellulitis or erysipelas. Tender enlarged nodes may be palpable in front or behind the ear, but the tenderness is superficial, unlike the tenderness on deep pressure in acute otitis media. The pinna is tender on movement, which, again, does not occur in acute otitis media. Any discharge is thick and scanty, unlike the copious mucoid discharge of middle ear infection.

Experience with 29 000 cases in which the oesophageal obturator airway has been used in cardiopulmonary resuscitation indicates its safety, efficacy, and ease of use. Blood gases, fractional inspired oxygen, and pH were measured in 18 patients given both the oesophageal obturator airway and the endotracheal tube; there was no significant difference between the two. The former was found to be inserted more rapidly and reliably; moreover, paramedical staff are quickly trained to use it. It is concluded that the oesophageal obturator airway provides the technique of choice whenever ideal conditions and facilities--and trained staff--for endotracheal intubation are not immediately available.
