Some Trace Element Contents and Ratios in Prostatic Fluids as Ancillary Diagnostic Tools in Distinguishing between the Benign Prostatic Hyperplasia and Chronic Prostatitis 前列腺液微量元素含量及比值作为鉴别良性前列腺增生与慢性前列腺炎的辅助诊断工具

Chronic prostatitis (CP) and benign prostatic hyperplasia (BPH) is an internationally important health problem of the man. The association between CP and BPH is significant. This warrants the need of reliable diagnostic tool which has ability to differentiate CP from the BPH. The aim of this exploratory study was to evaluate whether significant difference in the contents of Zn and some other trace elements as well as in the Zn/trace elements ratios of prostatic fluid exist between the inflamed and hyperplastic prostate. Prostatic fluid levels of Br, Fe, Rb, Sr, and Zn were prospectively evaluated and Zn/Br, Zn/Fe, Zn/Rb, and Zn/Sr ratios were calculated in 52 patients with BPH and 33 patients with CP. Measurements were performed using energy dispersive X-ray fluorescent microanalysis. It was found that in the prostatic fluid samples of BPH group the levels of Zn/Br, Zn/ Fe, and Zn/Rb are 3.39, 2.56, and 2.69 times, respectively, higher than levels of these parameters of patients with CP. It was supposed that the changes of Zn/Br, Zn/Fe, and Zn/Rb ratios in the prostatic fluid samples can be used as markers in distinguishing between CP and BPH.

Accelerated Atherosclerosis in the Setting of Chronic Kidney Disease 慢性肾脏疾病的加速动脉粥样硬化

Between 35% and 50% of deaths among patients with chronic kidney disease (CKD) can be attributed to cardiovascular disease. Even after adjusting for traditional cardiovascular risk factors, cardiovascular mortality risk is substantially increased in a linear fashion with decreasing glomerular filtration rate in CKD. Uremic toxins, oxidative stress and inflammation are critical factors found in CKD that can accelerate the atherosclerotic process. Although the precise mechanistic link(s) between CKD and cardiovascular disease are not yet fully defined, this review will discuss the current state of our knowledge. Lack of effective treatment for cardiovascular disease in CKD is a major unmet clinical need that can only be resolved with greater insight into the unique molecular and cellular mechanisms underlying cardiovacular disease pathogenesis in CKD.

Intraglomerular Lesions of Renal Angiomyolipoma in Tuberous Sclerosis Complex: A Case Report 结节性硬化症并发肾血管平滑肌脂肪瘤肾小球内病变1例

The association between tuberous sclerosis complex (TSC) and renal angiomyolipomas (RAML) is widely recognized, but the evidence of intraglomerular microlesions of angiomyolipoma in TSC is extremely rare. We report a 36-year-old woman with TSC, presenting as gross hematuria and hypovolemic shock. The patient was diagnosed with bilateral RAML and spontaneous rupture of right RAML. She underwent right nephrectomy. Microscopic examination revealed multiple bilateral RAML composed of dysmorphic blood vessels, smooth muscle, epithelioid cells and adipose tissue, as well as intraglomerular microlesions of these components in continuity with the glomerular basement membrane but without adhesion to the Bowman’s capsule. In this report we describe the histopathologic features and immunohistochemical markers of TSC-associated RAML with multiple parenquimatous lesions and isolated micro foci in the glomerular tuft. We also review recent discoveries related to possible pathogenesis of renal and intraglomerular lesions.

Renal Biopsy in the Elderly in Senegal: Indications and Outcomes 塞内加尔老年人肾活检:适应症和结果

Introduction: Renal biopsy (RB) is poorly performed in sub-Saharan Africa. It is even less practiced in the elderly. The objective of this study was to determine the indications of RB in the elderly and to describe the histopathological aspects of nephropathies of this age group. Patients and methods: This is a descriptive retrospective cohort study over a 5-year period (1 st January, 2011 to 31 st December, 2015). Patients older than 60 years who had RB during the study period were included. This RB had to be cortical and contain at least 10 glomeruli. All RB were echoguided. Results: Of the five hundred and ninety-nine (599) RBs performed, one hundred and five (105) were performed in elderly subjects, for a hospital prevalence of 17%. Only 75 had an interpretable histology. The mean age of the patients was 66.5±5.03 years old. With a sex ratio of 2.4. Nephrotic syndrome was the main indication in 26.7% of cases. Glomerular lesions accounted for 63% of cases, tubulointerstitial lesions 5.2%, vascular lesions 25.36% and unclassified lesions 6.5%. Among the 47 glomerular lesions, there were 13 cases (27.65%) of membranous nephropathy (MN), 12 cases (25.53%) of focal segmental glomerulosclerosis (FSGS), 5 cases (10.63%) of amyloidosis, 4 cases (8.51%) of minimal change disease (MCD), 3 cases (6.38%) of extracapillary glomerulonephritis (EGN), a case (2.12%) of membranoproliferative glomerulonephritis (MPGN) and a case (2.12%) of endocapillary glomerulonephritis. Conclusion: Our data, similarly to the literature worldwide, shows that MN is the most found lesion in the elderly.
