Current Resources for Skin of Color Dermatology and Cultural Competency Training for Physician Assistants 当前资源的肤色皮肤病学和文化能力培训的医师助理

Medical education has long held inequities when educating on dermatology for skin of color. More recent awareness of these long-standing issues has highlighted a need to ensure better dermatology education. This review will outline additional resources that can be used for training students and practicing providers in skin of color dermatology. While there are mixed data on how confidence correlates with diagnostic accuracy, studies show that medical students and residents lack confidence in skin of color dermatology. Resources, including digital learning tools, books, and social media platforms, may provide avenues to increase exposure to skin of color dermatology and potentially increase the confidence of medical providers to care for all patients seeking dermatologic care.

Skin Color Matters 肤色很重要

Evaluating skin of color (SOC) demands that clinicians avoid solely relying on visual inspection, which can lead to diagnostic error. For example, redness, one of the most obvious signs of cutaneous inflammation, is subtle, appears violaceous, hyperpigmented, or is not apparent in SOC. Erythema in all skin tones is evidence of inflammation, but in darker skin, so is hyperpigmentation. Several inflammatory conditions hallmarked by erythema, such as atopic dermatitis (AD), psoriasis, or pressure injuries (PI), may be misdiagnosed in darker skin because the clinical presentation is inconsistent with textbook descriptions. Moreover, some cancers may go undiagnosed.

Dermatologic Drug Safety in Pregnancy 妊娠期皮肤药物安全

In 2014, the US FDA removed the Pregnancy Category Drug lettering system and enacted the “Pregnancy and Lactation Label Ruling.” This ruling required drug products to contain contact information for drug-specific exposure pregnancy registries, narrative-style sections summarizing the known effect of pregnancy, lactation counseling data, and data describing risks for females and males of reproductive potential. This new ruling has added more dialogue and discussion to the patient-provider decision-making process and equires clinicians to provide more individualized counseling based on the current medical literature. This article summarizes the recent evidence for the safety of the most common dermatological therapies for pregnant and lactating women.

Psoriasis Exacerbation Following Dupilumab Use In A Nine-year-old Black Male Patient 9岁黑人男性患者使用杜匹单抗后银屑病加重

Distinguishing between psoriasis and other inflammatory dermatoses in pediatric skin of color can pose challenges. As more topical and systemic treatments become available for pediatric patients, early and accurate diagnoses are needed to help guide treatments for those that progress to severe disease. Delays in diagnosis for this population often lead to worsening disease until a correct diagnosis is reached. The goal of this article is to improve the understanding of inflammatory dermatoses in skin of color and review differential diagnoses, by presenting a case of psoriasis in a pediatric patient who flared after receiving dupilumab for an incorrect diagnosis of atopic dermatitis.

From Daily to Rarity - A Case Report of Ulcerative Pyoderma Gangrenosum 从日常到罕见——溃疡性坏疽性脓皮病1例报告

Pyoderma gangrenosum (PG) is classified as a non-infectious neutrophilic dermatosis with poorly understood etiology. Pathergy, which is major trauma caused by a minor incident, has been associated as a preceding event for PG. The epidemiology of the disease reveals patients who present with PG have associated underlying conditions with the most common comorbidities reported as inflammatory bowel disease, rheumatoid arthritis, hematological and solid organ malignancies. Pyoderma gangrenosum is considered a diagnosis of exclusion. It is commonly a missed diagnosis leading to delays in treatment. A diagnostic guide which clinicians can use is the PARACELSUS score. The following case report will focus on a relatively healthy 50-year-old female with a past medical history of hypertension who is in complete remission from breast cancer. This case highlights the importance of keeping PG in the differential diagnosis in patients without a history of autoimmune disease.

Results from the Pennsylvania Dermatology PA Society 2023 Salary Survey 宾夕法尼亚州皮肤病协会2023年薪酬调查结果

The Pennsylvania Dermatology PA Society, a state affiliate chapter of the Society of Dermatology Physician Assistants, conducts an annual salary survey of its membership based on self-reported W2 earnings for the previous year. The 2023 salary survey collected responses from 84 out of 346 PAs who practice dermatology as their primary specialty in the state of Pennsylvania. Results from the survey yielded insights such as respondents’ geographic location, years of experience in dermatology, compensation amount and structure, and factors they consider most important when selecting a position. The Pennsylvania Dermatology PA Society presents results from this survey to add to the existing practice data and inform evidence-based discussion on fair compensation.

Lichen Planopilaris in Skin of Color: A Case Report 有色皮肤扁平苔藓1例报告

Lichen planopilaris (LPP) is an uncommon inflammatory scalp disorder which presents with a cicatricial alopecia with characteristic perifollicular erythema and perifollicular scale at the margins of the areas of alopecia. Classic LPP is usually seen in women between the ages of 41 and 50, although men may also be affected. White patients are more commonly affected,1 and the disease is potentially underdiagnosed in individuals of color. This case highlights a patient who was previously diagnosed with traction alopecia who went on to develop LPP. It is important to keep LPP on the differential for cases of alopecia in skin of color as the treatment varies from other forms of alopecia. The goal of treatment is to delay or stop progression and improve symptoms for the patient.

It is important to keep LPP on the differential for cases of alopecia in skin of color as the treatment varies from other forms of alopecia.
