Safety of Intranasal Insulin in Type 2 Diabetes on Systemic Insulin: A Double-Blinded Placebo-Controlled Sub-Study of Memaid Trial. 2型糖尿病鼻内胰岛素对全身胰岛素的安全性:Memaid试验的双盲安慰剂对照亚研究

Aims: To determine safety of intranasal insulin (INI) in MemAID trial participants with diabetes treated with systemic insulins.

Materials and methods: This randomized, double-blinded trial consisted of 24-week INI or placebo treatment once daily and 24-week follow-up. Safety outcomes were: 1) Short-term effects on glycemic variability, hypoglycemic episodes on continuous glucose monitoring (CGM) at baseline and on-treatment. 2) Long-term effects on glucose metabolism and weight on INI/placebo treatment and post-treatment follow-up. Of 86 screened subjects, 14 were randomized, 9 (5 INI, 4 Placebo) completed CGM at baseline and on-treatment, and 5 (2 INI, 3 Placebo) completed treatment and follow-up.

Results: INI was safe and was not associated with serious adverse events, hypoglycemic episodes or weight gain. INI administration did not acutely affect capillary glucose. Glycemic variability on CGM decreased with INI, compared to baseline. On INI treatment, there was a long-term trend toward lower HbA1c, plasma glucose and insulin. No interactions with subcutaneous insulins were observed.

Conclusions: INI is safe in older people with diabetes treated with systemic insulins, and it is not associated with adverse events, hypoglycemia or weight gain. Future studies are needed to determine whether INI administration can reduce glycemic variability, improve insulin sensitivity and thus potentially lessen diabetes burden in this population.

In an earlier study, it was proposed that the final consequence of hereditary anomaly results in the development of type 2 diabetes, which already emerges in the prediabetic phase. Factors Responsible for Diabetes Elevated Blood Pressure Among Bangladeshi Adults 孟加拉国成年人糖尿病血压升高的因素

The analytical results presented here were based on data collected from 960 adults of ages 18 years and above residing in both urban and rural areas of Bangladesh. The data were collected by some doctors and nurses from and nearby their working places according to quota sampling plan. The objective of the study was to investigate the impacts of different socioeconomic variables on simultaneous sufferings of adults from diabetes and high blood pressure and to identify the most responsible variables for this suffering. The analysis indicated that 5.4 percent adults were simultaneously suffering from both diabetes and elevated blood pressure. The most responsible variable for the diseases was occupation followed by gender variation, income and expenditure. The results of risk ratio and factor analysis indicated that the females and physically inactive adults had the more chance of affecting by the disease. Males, non- user of can food and skilled or unskilled working group had less chance of affected by the disease. The variables were identified by factor analysis.

Diabetes type 2 represents a global health problem, and it is expected to influence approximately 530 million people by 2030. Usually, diabetes type 2 is developed through more than one stage. Prediabetes is a preliminary stage of diabetes before developing full episodes of diabetes.Diabetes type 2 can be preventable following appropriate lifestyles. The main objective of this study is to describe a case with liver cirrhosis and the patient was recommended for liver transplantation. Following reviewing of his profile, it was apparent that the patient had prediabetes (glucose level was 126mg/dl). The patient was recommended to use tadalafil 5 mg daily to offer improvement of liver function based on previous experience. Glucose level was daily measured. It was interestingly found that glucose levels were restored to normal levels between 85-100 mg/dl. Taken together, prediabetes can be treated using non-classical therapeutic approaches and full events can be prevented accordingly.

Usually, diabetes type 2 is developed through more than one stage. Prediabetes is a preliminary stage of diabetes before developing full episodes of diabetes.Diabetes type 2 can be preventable following appropriate lifestyles. The main objective of this study is to describe a case with liver cirrhosis and the patient was recommended for liver transplantation. Following reviewing of his profile, it was apparent that the patient had prediabetes (glucose level was 126mg/dl). The patient was recommended to use tadalafil 5 mg daily to offer improvement of liver function based on previous experience. Glucose level was daily measured. It was interestingly found that glucose levels were restored to normal levels between 85-100 mg/dl.
