Background : Although the information on Metabolic Syndrome (MetS) in the general population from different parts of Bangladesh is available, the information regarding the same is scarcely available from physicians of Bangladesh. This study was aimed to assess the prevalence of MetS among physicians working at Chattogram Medical College Hospital, Bangladesh. Materials and methods : Data was collected for this cross-sectional study from 255 working physicians of Chittagong Medical College Hospital in 2018. The participants filled a structured questionnaire followed by physical and biochemical measurements including waist circumference, blood pressure, lipid profile, and fasting blood glucose. National Cholesterol Education Program’s Adult Treatment Panel III report (ATP III) defines criteria were used to identify physicians with the MetS. Results : Among 255 participants (males - 192 and females - 63), 100 (39.2%) were with MetS. Prevalence of same was higher in females (41.3%) than males (38.5%). The elderly physicians (>40 years) had a higher prevalence compared to the younger (£40 years) physicians [72 (50.3%) vs. 28 (25.0%)]. In the males, low High-Density Lipoproteins-Cholesterol (HDL-C) was the major contributor to the prevalence of MetS followed by high blood pressure. However, in females, low HDL-C and greater waist circumference contributed the maximum to MetS. Conclusion : This study showed a high prevalence of MetS among physicians of Bangladesh, and thereby warrants the urgent implementation of preventive health care strategies to reduce both morbidity and mortality related to this medical problem. Further studies ought to focus on the working conditions and lifestyles of individuals in these high-risk groups. Chatt Maa Shi Hosp Med Coll J; Vol.20 (2); July 2021; Page 65-68

Background: Lower segment caesarean section (LSCS) is a surgical intervention, which is carried out to ensure the safety of mother and baby when vaginal delivery is not possible or when danger to mother and baby would be greater with vaginal delivery. This study is designed to identify indication of LSCS and detect complications if any among the respondents. Materials and methods: Cross sectional descriptive study where 95 female doctors of Chattogram were selected by convenient sampling without knowing their mode of delivery and if it is LSCS then indication of LSCS , maternal and neonatal complications were noted. Results: Age of the respondents at the time of interview were noted and most of them were between 31- 40 years 52 (55%), regarding income most of them between 100000 -200000 42(44%) most of the delivery occurred in the private clinics 92 (70%) regarding mode of delivery, LSCS was 92 (70%) vaginal delivery 40 (30%) regarding indications of LSCS most common was repeat caesarean section 27 (29%) and second common was LSCS due to maternal interest 18 (20%) and others were severe preeclampsia 10 (11%) fetal distress 08(09%) placenta previa, Diabetes with big baby, cephalopelvic disproportion, PROM with non progression of labour. PPH was the most common complication 05 (63%), next was paralytic ileus 02 (25%) and left ventricular failure 01 (12%). Common neonatal comoplications were low birth weight baby 904 (40%), prematurity 03 (30%) and birth asphyxia 03 (03%). Conclusion: LSCS is a major surgical procedure for delivery. Inspite of its low maternal morbidity and mortality due to improved surgical technique, modern aneasthetic skill and availability of blood and blood products, still it carries a slightly greater risk in subsequent pregnancy, so we should be rational in doing primary caesarean section. Background: Currently the most favorable single stage procedure for management of long segment anterior urethral stricture is dorsal onlay technique of urethroplasty. This required circumferential extensive mobilization of the urethra, which might cause ischemia of urethra in addition to chordee. To evaluate the short term outcome of dorsolateral onlay BMG (Buccal Mucosal Graft) urethroplasty by unilateral urethral mobilization for treatment of long segment anterior urethral stricture Materials and methods: A prospective experimental study from January 2016 to December 2018 is carried out in Department of Urology, Chittagong Medical College Hospital (CMCH) Chattogram, Bangladesh. Total 32 patients of long segment anterior urethral stricture were treated by dorsolateral onlay BMG urethroplasty by unilateral urethral mobilization. Stricture >2cm were included; RGU, MCU and Uroflowmetry were the mainstay of assessment. Success was defined as maximum flow rate >15 ml/s, normal RGU and/ urethroscopy. Failure considered as presence of obstructive symptoms, Qmax < 15 ml/s, stricture on RGU and any post-operative urethral intervention. Results: Mean stricture length was 48.72mm (Range: 30-77mm) and mean follow up time was 22.4months (Range: 12-40 months). Three patients were found to develop stricture at proximal anastomotic site during follow up and required optical internal urethrotomy and considered as failure. 01 patients developed Surgical Site Infection (SSI) which resolved after regular dressing. Success rate was 90.7% Conclusion: Dorsolateral onlay BMG urethroplasty by unilateral mobilization is feasible, safe and easily adoptable for long segment anterior urethral stricture with good short term outcome. Background : Supracondylar fractures of the humerus is one of the most common fractures in children. Failure to treat properly leads to malunion of the fracture site. For that, closed reduction and percutaneous pinning is considered to be the golden choice for treatment. There are many methods in which percutaneous k-wire fixation can be done. The aim of this study is to report the advantages of percutaneous fixation using cross pinning from both medial and lateral sides. Materials and methods : A total number of seventy pediatric patients with Gartland type II and type III supracondylar fracture of the humerus were recruited from the outpatient department of two different hospitals between January 2018 and September 2020. All of them were treated using closed reduction and internal fixation using percutaneous crossed k-wires. The treatment outcomes were evaluated using Flynn’s criteria and were compared with other similar studies. Results : The mean age of study subjects was 8.14 ± 2.8 and the male to female ratio was 1.6:1. In 27(38.6%) cases the left arm was involved while in 43(61.4%) cases the right arm was involved. Preoperative complications included 1(1.4%) case with radial nerve palsy and 4(5.7%) cases with pulseless pink hand. When evaluating cosmetic outcome using Flynn’s criteria, there were 57(81.4%) excellent, 10(14.3%) good and 3(4.3%) fair outcomes. On evaluating outcome according to range of motion deficit outcomes were excellent in 35(50%), good in 22(31.4%), fair in 5(7.2%) and poor in 8(11.4%) children. Post-operative complications were 1(1.4%) ulnar nerve neuropraxia and 5(7.2%) superficial pin tract infections. Conclusion : Closed reduction and internal fixation using percutaneous crossed kwires placed from the medial and lateral side gives satisfactory cosmetic and functional outcomes in majority of the patients with Gartland type II and III supracondylar fractures of the humerus. Background : On a daily basis, orthodontic patients present with malocclusions and orofacial imbalance. Often times, teeth must be extracted in order to obtain proper esthetics, occlusion, and stability as well as extraction of teeth is often a need to close residual space, after the initial decrowding and aligning. Pre adjusted fixed orthodontic appliances commonly utilize sliding mechanics for space closure with different types of force delivery systems. A variety of materials have been used as force delivery systems to close spaces between teeth as in the case of canine retraction after the extraction of premolars. Elastomeric auxiliaries are relatively consistent in producing tooth movements. The purpose of the study to evaluate the rate of mandibular canine retraction by elastomeric power chain. Materials and methods : This obervational study was designed as split mouth study. A total number of 20 patients (40 quadrants),who required canine retraction into first premolar extraction sites as part of their orthodontic treatment in the Department of Orthodontics, BSMMU as study population. The quadrants were affixed by elastomeric power chain (Dentaurum) with 200gm force measured by Correx Tension Gauge. Space closure was measured by means of direct measurement from the mesial surface of mesial wing of the 2nd premolar bracket and the distal surface of distal wing of the canine bracket with digital Vernier Calipers. Results: Mean space closer rate in mandibular canine retraction was 0.99 ± 0.08 mm/month in elastomeric power chain method with p value<0.001. Conclusion : This study revealed that space closer rate in mandibular canine retraction was 0.99 ± 0.08 mm/month by elastomeric power chain. Background: Hospital services are the amenities provided by a hospital both for the patients and the workforces. Periodical assessment of hospital service quality discovers the demands and needs thus ensuring the development of the institution. The aim of the study to find out the problems regarding hospital services and its solution. Materials and methods: This observational study was conducted among the health personnel of Chattogram Maa-O-Shishu Hospital from 01 July to 30 August 2019 and 01 January to 29 February 2020. Purposive sampling technique was followed to collect data from 100 randomly selected samples. They were interviewed with pretested questionnaire and data were managed manually. Results: Different findings were drawn. These are: Lack of work dedication, responsibility, loyalty and internal co-operation among the staff (60%). Lack of manpower including doctors, nursing staff, cleaners and few recruitments are not so experienced as to fulfill the demands of their duty (56%). The number of beds is less compared to the number of patients who need them e.g. ICU, CCU, NICU, Paediatric ICU Adult medicine ward, etc. Hence patients are referred outside (46%). Patient visiting hours and number of visitors not properly maintained. Visitors make noise, take food on patient’s bed and sometime physically assault employees when they are asked to obey rules (40%). However, overall job satisfaction of the respondents was 78%. Conclusion: Ascertaining the issues regarding the improvement of hospital services is mandatory. Quick detection and early solution would gain confidence by employees and beneficiary which would ultimately boost up the reputation of the hospital. Background: Thalassemias are the most common inheritable blood disorders requiring regular blood transfusions and iron chelating therapy. Non-adherence to iron chelation therapy increases complications and is a problem in treating thalassemia. To assess the reasons of non-adherence to iron chelating drug in treating thalassemia. Materials and methods: This descriptive cross-sectional study was carried out in the thalassemia ward of Chattogram Maa Shishu-O-General Hospital, Chattogram from July, 2013 to June, 2014. 70 thalassemia patients aged 2-18 years previously treated with iron chelating drugs were included. Parents were interviewed according to a formulated questionnaire based on discontinuation of iron chelating drugs and its reasons. Data were analyzed by both manually and by SPSS-18. Results: About 48.6% patients needed blood transfusion >10 units/year and 62.9% patients were prescribed with iron chelating drugs. Near about half patients (47.7%) did not continued iron chelating therapy till full prescribed period. Deferiprone (31.8%) and combination of deferipronc & desferrioxamine (31.8%) was the most commonly prescribed drug. Deferiprone is the drug to which most of the patients (70%) were adherent and a good number of patients (65%) discontinued desferrioxamine. Financial problem (100%) was the only reason for discontinuation of oral chelator. In case of parenteral chelator, besides finanacial problem (38.5%), time consuming natures (38.5%), need of hospital admission (23%) are the other causes for non-adherence to iron chelation therapy. Conclusion: Financial problem is the main cause of non-adherence to iron chelation therapy. Iron chelating drugs should be available at low cost. Background : Vitamin D is an essential nutrient and deficiency of it leads to devastating disorders. This study aimed to find out the associated factors with selected variables of low vitamin D among adult females of Chattogram. Materials and methods : Cross-sectional observational study was conducted from September 2019 to February 2020 at a diagnostic complex of Chattogram. Participants were adult females of Chattogram. All participants gave oral informed consent and answered a questionnaire that included 16 questions covering demographic information, monthly income, educational status, occupation, body weight, duration of sun exposure, dress pattern (Using hijab or burkha, regular dress-up) drug and disease history. Blood samples were collected by cautious aseptic procedure. Serum vitamin D levels were measured by using immunoassay method. Adult females of Chattogram with no religious restrictions, n= 150,aged 31 to 70 years. Results: Overall, 88% had low vitamin D levels with deficient 60.66% and insufficient 27.34%. Sufficient level of vitamin D was found only among 12%. The prevalence of vitamin D deficiency was much higher in females with house-wife status 73.33% (p value of 0.0001) wearing hijab 46.66% (p value of 0.001). It could be due to interference with UVB radiation into skin, short duration of sun exposure, use of sunblock cream or less supplementary intake. Conclusion : Vitamin D deficiency is an emerging but neglected health issue in modern time, particularly more vulnerable are female population. Ironically, abundant sunlight of Bangladesh seems failed to protect the self-imposed concrete prisoner, classically known as home-makers, and practitioner of indoor life style that leads to avoidance of sun light. However, large sample size is mandatory to boost-up the findings of current study. Chatt Maa Shi Hosp Med Coll J; Vol.20 (2); July 2021; Page 74-78
