Since over a century gender mainstreaming tries to take root in our society to ensure equality of all genders on all social levels. Nevertheless, the so-called gender gap is still omnipresent, especially the gender health gap. Men are still the main focus in (medical) research, the androcentric system and binary thinking are tenacious.
This can cause severe consequences for medical issues for non-male genders. Late or wrong diagnosis for heart diseases or pain syndromes in women happen a lot of times because of unawareness of early symptoms or wrongly blaming a psychosomatic cause. Non-binary people are not represented at all.
The medical knowledge of gender differences is also important for osteopathy, because we have to interpret those symptoms in a gender sensitive way in order to avoid missing contraindications for a treatment.

This article provides an overview of the specific thinking and action in psychosomatic osteopathy in general and the book “Psychosomatic Osteopathy” by Torsten Liem in particular. Basic assumptions, extended principles, dysfunction models, diagnostic and treatment approaches are explained as well as differences to conventional osteopathic approaches. Etiological factors, risk factors and mechanisms of action of complaints, healing responses and soma-physiology-experience-context dysfunction patterns (SPECD) are presented to develop an understanding of specific treatment approaches in the context of PSO. Thus, consideration of multiple interdependent and dynamically interwoven influences, holarchic levels of regulation, and systems of equilibrium in diagnosis and treatment is clinically relevant, as is identification and differentiation of genetic, epigenetic, and metabolic influences. Treatment indications and the treatment set-up in the context of PSO will be taught. In addition, the special importance of resources, coregulation and feedback loops in the treatment setting of PSO will be elaborated.

If a permanent load and stress exceeds the elastic limit of the bone, a slight plastic-elastic deformation of the osseous structures may occur. If the bone does not return to its original neutral shape a chronic tension in the trabecular matrix develops or remains. This in turn leads to permanent traction/pressure on the involved fascia, capsule, ligament and tendon structures, which have been shown to be non-extensible, with inevitable activation of the pain receptors interwoven within them. Chronic, sometimes therapy-resistant complaints in the bone area are the result.
This article introduces (Bone) Strain Relief, a new conservative treatment concept that can be used to manually bend/rotate/push pathological, invisible bone “deformities” back into their original anatomical position in order to minimize the permanent tension/shear/pull on fascia and tendons and thus very successfully reduce or even eliminate chronic pain.
