Detect Melanoma Skin Cancer Using an Improved Deep Learning CNN Model with Improved Computational Costs 使用改进的计算成本提高的深度学习CNN模型检测黑色素瘤皮肤癌

Melanoma cancer has been considered as one of the deadliest cancers. Melanoma is a highly malignant form of skin cancer that originates from melanocytes, the cells responsible for producing skin pigment. It is characterized by the uncontrolled prolife-elation of abnormal cells, which have the potential to invade surrounding tissues and spread to distant parts of the body. In this work, we aim to classify the skin disease into 7 classes. Our objective is to propose a deep learning CNN model to improve the accuracy of melanoma detection by customizing the number of layers in the network architecture and activation functions followed by reducing the computational cost. A comparative study is made between the improved model and the use of pre-trained Models like Resnet, Dense Net, Inception, VGG and Dense Net-II which has been giving impeccable accuracy. The HAM10000 dataset is used for research and we have got better results for the proposed model. Also, graphical results have been obtained for the same.

Quality of Life of Patients with Stiff Person Syndrome 僵硬人综合征患者的生活质量

Aim: This study aimed to identify the factors that significantly affect the quality of life (QoL) of patients with Stiff-Person syndrome (SPS) by examining their physical, psychological, and social aspects using questionnaires. Methods: This cross-sectional study included 23 patients with SPS. Data were collected using several questionnaires. Patient QoL was quantified using the EuroQoL-5-Dimention 5-Level; sociodemographic characteristics and other relevant variables were examined. Results: QoL was poorer for patients with SPS than those with moderate to severe Alzheimer’s disease. Several important factors for patients’ QoL have been identified, including difficulty in breathing, pain, mobility issues leading to difficulty performing daily activities, depressive symptoms inherent to SPS, and insufficient social support. Conclusion: Factors that significantly influenced the QoL of patients with SPS were related to multiple aspects of their lives. We evaluated all previously used methods. A disease-specific evaluation form is required to assess these patients thoroughly.

Choked for Over a Decade: A Rare Case of Huge Esophageal Fibro-vascular Polyp Causing Dysphagia 窒息十多年:罕见的巨大食道纤维血管息肉导致吞咽困难

Introduction: Esophageal fibro-vascular polyps (EFVPs) are rare benign tumors that can arise from the submucosa of the esophagus. They are usually asymptomatic but can cause obstruction and dysphagia in some cases. The diagnosis of EFVPs can be challenging due to their rarity and nonspecific symptoms. Delayed diagnosis can result in significant morbidity, as in the case of our patient who suffered from difficulty swallowing for over a decade before being accurately diagnosed. We present a case of a 42-year-old male patient with a 12-year history of difficulty swallowing, who was eventually diagnosed with a huge fibro-vascular polyp after undergoing thoracotomy esophagostomy and excision of the polyp. Case Presentation: A 42-year-old male patient presented to Hallelujah General Hospital, with a 12-year history of difficulty swallowing, associated with symptoms such as weight loss, fatigue, and asphyxiation while vomiting. The patient had visited several health centers but was unable to receive an accurate diagnosis or effective treatment. Initial investigations revealed a large, smooth, and mobile mass in the mid-esophagus, which was obstructing the lumen and causing difficulty swallowing. A CT scan of the chest and abdomen showed a large polypoid mass in the upper and mid-esophagus, measuring 25 cm in length and 10 cm in diameter, which was confirmed to be an esophageal fibro-vascular polyp upon surgical excision and biopsy. Conclusion: EFVPs are rare, benign tumors of the esophagus that can cause significant morbidity if not diagnosed and treated promptly. The diagnosis of EFVPs can be challenging, and imaging studies such as CT scans, MRI, and endoscopic ultrasound can be helpful in diagnosing the condition. However, a definitive diagnosis usually requires histological examination of the excised mass. Surgical excision is the treatment of choice for EFVPs, and the prognosis is usually excellent. This case report highlights the importance of considering rare conditions in the differential diagnosis of common symptoms, particularly if the patient does not respond to initial treatment. It also emphasizes the importance of early diagnosis and prompt surgical intervention in preventing complications and improving outcomes for patients with EFVPs.

Prominent Eustachian Valve Complicated by Thrombus 突出的耳咽管瓣膜合并血栓

The Eustachian valve is an embryological remnant of the valve of the inferior vena cava. It may be prominent in some individuals, but the echocardiographic appearance of a divided right atrium (RA) as a result of a large Eustachian valve is rare. Sometimes it presents a problem of differential diagnosis with other embryonic remnants of the right atrium, such as the Chiari network. It may be complicated by thrombosis or infective endocarditis. Cardiac imaging plays an important role in diagnosis. The aim of this review is to discuss a rare form of Eustachian valve complicated by thrombosis.

The risk of thromboembolism is high in patients with lung adenocarcinoma, especially in patients with ALK rearrangement. The mechanism of thromboembolism due to ALK rearrangement is still unknown. Here, we present a case of diffuse cerebral infarction due to massive thromboembolism in a 25-year-old young male patient with lung adenocarcinoma and ALK rearrangement. Thrombosis, apart from metastasis, should be kept in mind when cranial symptoms develop in patients with lung cancer with ALK rearrangement. In addition, these patients should be followed closely in terms of thrombosis both at diagnosis and during treatment.

Intraoperative awareness with recall during General Anaesthesia in Damascus University Hospitals in Syria 叙利亚大马士革大学医院全麻期间术中意识与回忆

Background: Awareness with recall during general anesthesia is one of the frightening complications of general anesthesia, which indicates the failure of anesthesia. Awareness is an underestimated complication despite the fact that it sometimes carries devastating psychological risks for patients. To our knowledge this is the first research in Syria that reports the incidence of awareness with recall during general anesthesia. Aims: to assess the incidence of awareness with recall during general anesthesia in Damascus University teaching hospitals. Design: Multi-central prospective cohort study. Setting: Three main Teaching Hospitals of Damascus University: Al-Assad, Al-Mowassat & Maternity Hospitals during the time period between January and November 2021. Study Sample: 3500 patients aged ≥ 18 years were included. Results: the incidence of Awareness with recall during general anesthesia in Damascus University Teaching Hospitals was 0.66% in low risk of awareness surgery and 1.8% in high risk of awareness surgeries. The most frequent manifestations of awareness were auditory perceptions, paralysis and pain (100%, 100% & 97.4% respectively). Conclusions: Awareness with recall rate was somehow high in Damascus University Teaching Hospitals in comparison to the figures reported by American and European studies, this difference was especially noted in a low risk of awareness surgery. The rates were similar in the high risk of awareness surgeries. Lack of premedication and low concentration of inhalational agent were the main reasons for awareness.

Hematological malignancy is an increasing burden on the Tanzanian healthcare system accounting for around 10% of all cancer diagnosis [1]. Acute myeloid leukemia (AML) is particularly challenging in the context of limited diagnostic services, restricted blood product availability, high cost of chemotherapy drugs and the complete absence of bone marrow transplantation. Despite this Kilimanjaro Christian Medical Centre (KCMC) has attempted to overcome these barriers and has commenced induction therapy with daunorubicin and cytarabine in a number of suitable candidates in an attempt to treat their disease. However, to date treatment during nadir poses an unresolved threat despite the use of prophylactic antiviral, anti-fungal and antibiotic therapies and broad-spectrum antibiotics in the case of infection.

Technological developments have taken place in the modern world. Internet Addiction has even been championed as an actual disorder, notably by psychologists Kimberly Young. This paper throws light on facts about Internet addiction and mental health of adolescence have a strong connection. The data collected from secondary sources as books, journals, e-books and various Google sites. There is strong connection between Internet addiction and mental health of adolescence. Due to excess use of internet has harmful detrimental effects on adolescence.

Aim and Background: Pancytopenia is a reduction of all the three cells of peripheral blood lineage: leukocytes, erythrocytes and platelets. The bone marrow can be primarily hypocellular in pancytopenia either due to reduction of hematopoietic cell production or ineffective haemopoiesis. Generally, infections, toxins or malignant transformation of the marrow are responsible for most of the cases of pancytopenia.1 After the incidence of Covid-19 pandemic, pancytopenia has been seen to occur as a rare complication secondary to SARS-COV 2. Here, we report a unique case of pancytopenia in a previously healthy child with SARS-CoV-2 infection.

Traditional Treatment by Arabic Acacia Seeds Exterminating Microbial Biofilms for Wounds Healing 阿拉伯刺槐种子消灭微生物生物膜的传统治疗伤口愈合

Microbial biofilms exist in wounds resulting in long-term non-healing infected wounds. Herbal products as ancient traditions treatment and scientists in developing countries using in wounds healing over years. Arabic Acacia present in KSA, it's using in production of some medications. It has gallic-acid, methyl-gallate, quercetin, rutin, myricetin 3-O-(3'''-O -galloyl), β-D-rutinoside and catechin. It has a significant antimicrobial agent. Its extract provided fast wounds healing related to antimicrobial activities. The aim was to prove "Traditional Treatment" by Arabic Acacia Seeds exterminate microbial biofilms for wounds healing. Materials and methods were included Arabic Acacia aqueous extract, biofilm isolates included (Staphylococcus sp., Streptococcus sp., Pseudomonas sp., Proteus sp., Escherichia coli, also Klebsiella sp., and Candida sp.)) The applied tests were anti-biofilms formation and biofilms elimination, then biofilm estimation by Tissue Culture Plate method (TCP). Results were for estimation of anti-biofilms formation after exposed to Arabic Acacia aqueous extract. It was eliminated by simple Arabic Acacia aqueous extract early by crude dilution in overnight time were Streptococcus sp., and Proteus sp. Then by crude dilution in day time were Escherichia coli, also Klebsiella sp. Staphylococcus sp., Pseudomonas sp., and Candida sp., they had reached the weak point only. Estimation of biofilms elimination after exposed to Arabic Acacia aqueous extract. The effects of all dilutions were faster than anti-biofilms formation. This was giving none for Streptococcus sp., and Proteus sp., by half dilution for overnight. While Streptococcus sp., Proteus sp., Escherichia coli also Klebsiella sp. were resulted none by half dilution for a day. Crude dilution gave none for all biofilms elimination in both times. It was concluded that the Arabic Acacia aqueous extract had microbial anti-biofilms formation. That was by their antimicrobial contents exterminating microbial biofilms for wounds healing. This indicated the using from ancient Arab customs to treat wounds as traditional treatment. It was recommended that for the "Traditional Treatment" of Arabic Acacia extract according healthy doses as simple treatment instead of chemical treatment for wounds healing.
