Fifty adult patients of both sexes are selected for the study. Thirty patients were non-neurological pain among them knee joint arthoplasty were 33.3%, wrist joint fracture 33.35, shoulder joint surgery were 16.6% and mandible fracture were 16.6%. While neurological patients i.e Diabetic neuropathy having acute pain 3%, and chronic pain were 70%. Spinal cord injury having lumbar level injury were 60%, and cervical level of spinal cord injury were 40% (both were acute). As pregabalin is a non opoid drug, which has more effective in acute and chronic neurological pain rather than chronic arthitic pain. Moreover, perioperative administration of pregabaline causes least nusea, vomiting and pruritis and adverse effect include sedation, dizziness and visual disturbance. These findings are more or less in agreement with previous studies.

Myoclonus has been defined as sudden, brief, jerks, twitching or spasm of a muscle or a group of muscles that can be generated by any area in the central nervous system. Drugs administered through intrathecal and epidural routes and sedatives like benzodiazepines and opioids intravenously can occasionally cause myoclonus. The differential diagnoses of myoclonus following IV administration of midazolam are many. Inj midazolam may be responsible for acute myoclonus in our patient. The exact mechanism is not known. Extensive studies of the cause of myoclonus with midazolam have remained an enigma.

A common complication of trans-scleral fixation of posterior chamber intraocular lenses with prolene suture is high rate of spontaneous dislocation of the IOL due to biodegradation or breakage of suture. This is because of, at the place of suture knots, there is inflammation and continuous irritation, which leads to scleral melting and suture disintegration in due course of time. We report a new technique of suture tying and burying the knot of prolene suture in the scleral pocket instead of making a sclera flap and suturing the flap. We also tie the lens eyelet with double suture instead of single suture to ensure maximum post operative stability of lens. We achieved a very good centration and stability of S IOL with this technique. A good aspect was that there was no visible suture knot of any type, either of prolene suture or flap suture. This new technique is good modification of conventional technique to prevent knot and sclera flap related problems.
